The IgA-binding beta antigen of the c protein complex of Group B streptococci: sequence determination of its gene and detection of two binding regions.

The beta antigen of the lbc protein complex of Group B streptococci is a cell-surface receptor which binds the Fc region of human immunoglobulin A (IgA). Determination of the nucleotide sequence of the beta antigen gene shows that it encodes a preprotein having a molecular weight of 130,963 daltons and a polypeptide of 1164 amino acid residues that is typical of other Gram-positive cell-wall proteins. There is a long signal sequence of 37 amino acids at the N-terminus. Four of the five C-terminal amino acid residues are basic and are preceded by a hydrophobic stretch that appears to anchor the

Ampicillin-aminoglycoside combinations as initial treatment for neonatal septicaemia or meningitis. A retrospective evaluation of 12 years' experience.

In a retrospective study covering the years 1975-1986, 341 episodes of invasive infections in 338 newborn infants were evaluated. Of the 365 pathogens isolated from blood and/or cerebrospinal fluid, 91% were sensitive to either ampicillin or aminoglycosides or both. Ampicillin resistance was mainly found in very low and low birthweight infants with late-onset infections, in which aerobic Gram-negative rods were common pathogens. In contrast, aminoglycoside resistance was common in early-onset infections, due to the dominance of group B streptococcal infections. The ampicillin-aminoglycoside co

Restriction endonuclease digest patterns of chromosomal DNA from group B beta-haemolytic streptococci.

Scanning densitometry and computer-assisted numerical analysis were used to examine restriction endonuclease digest patterns (RDPs) of chromosomal DNA from 26 infecting strains and 44 vaginal isolates of group B beta-haemolytic streptococci (GBS). At the 95% similarity level, HindIII RDPs of serotype Ia and III strains clustered into four and three RDP types, respectively. Nine of 10 strains from neonates with early-onset septicaemia belonged to two particular RDP types (Ia-3 and III-3). In contrast, serotype III GBS strains from meningitis cases were not characterised by particular RDP types.

An obstetric approach to the prevention of early-onset group B beta-hemolytic streptococcal sepsis.

Group B beta-hemolytic streptococcus is one of the most common causes of neonatal sepsis. Despite its relationship to neonatal morbidity and mortality, no consensus exists for an approach to its prevention. Several characteristics of the organism such as high maternal carriage rates, the intermittent nature of this carriage, and the failure of antibiotics to permanently eliminate carriage have limited the success of proposed intervention protocols. In this clinical opinion we review characteristics of the organism and previously suggested intervention protocols. Then, based on this review and

Intraamniotic infection due to group G streptococcus: treatment and antibody response.

Penicillin treatment and antibody response were studied using a rhesus monkey model for intraamniotic infection with type III group B streptococci (T3GBS). Acute and convalescent phase sera from mothers and their offspring were tested with a radioactive antigen-binding assay to determine the concentration of antibody to the capsular T3GBS antigen. The frequency of placentitis was significantly lower in penicillin-treated animals (3 of 8) than in controls (10 of 10; P less than .01). The penicillin group also had a significantly lower neonatal mortality (1 of 9) than controls (6 of 10; P less t

The natural history of group B streptococcal colonization in the pregnant woman and her offspring. I. Colonization studies.

A longitudinal study of colonization by group B streptococcus (GBS) was conducted in 93 pregnant women and 92 of their infants. Positive cultures were obtained from vaginal swab specimens on at least one occasion during pregnancy from 20.4% of the women. Three types of carriage were observed: chronic, transient, and intermittent. At the time of delivery, 12.9% of the mothers were colonized by GBS. GBS colonization occurred in 41.7% of the infants of the mothers colonized intrapartum. Only 1.2% of infants intrapartum culture-negative mothers were colonized. Infants colonized at birth were cultu

Selective maternal culturing to identify group B streptococal infection.

During a 5-year period vaginal cultures were obtained from all women with an obstetric history of premature onset of labor or premature rupture of membranes. With these indications, 1,213 (12.7%) of all parturient patients were cultured and 10.2% of those cultured were colonized with group B beta-hemolytic streptococci. Maternal colonization did not correlate with ABO blood group, although a significantly higher percentage of Rh negative women were colonized (p < 0.01). During this 5-year period, 20 infants had documented early-onset infection (sepsis or meningitis) with group B beta-hemolytic

Group B streptococci in the female genital tract.

Vaginal carriage rates of group B streptococci among 250 women attending a clinic for sexually transmitted diseases, 123 attending family planning clinics, and 110 in labour wages were 36.0%, 17-1% and 6.4% respectively. The presence of group B streptococci was not associated with a vaginal discharge or the use of oral contraceptives in the non-pregnant women, or with the isolation of Neisseria gonorrhoeae or Trichomonas vaginalis from the women attending the clinic for sexually transmitted diseases. Serotyping showed a predominance of types II and III in non-pregnant women and an overall inci

A comparison of early-onset group B streptococcal neonatal infection and the respiratory-distress syndrome of the newborn.

In attempting to differentiate early-onset Group B streptococcal infection from hyaline-membrane disease we found features of severe Group B infection to be rupture of the membranes for more than 12 hours before delivery (four or eight versus one of nine), gram-positive cocci in the gastric aspirate (four or four versus none of one), apnea and shock in the first 24 hours of life (seven of eight versus none of nine), and the generation of lower peak inspiratory pressures on avolume-cycled respirator (mean of 36.5 +/- 2.8 versus 63.9 +/- 6.2 cm of water; P = 0.005). Group B streptococci in venereal disease clinic patients.

The isolation rates of group B streptococci for 457 men and 300 women attending a venereal disease clinic were 16-4 and 20-6% respectively. The frequencies were significantly increased by using a selective medium and by taking specimens from the female urethra as well as the cervix. In contrast to gonorrhoea the presence of group B streptococci could not be related to promiscuity. In addition, the isolation of group B streptococci was found to be independent of a concomitant gonococcal infection. This study indicates that group B streptococci have little relation to the clinical signs and symp

Persistent reduction of Bifidobacterium longum in the infant gut microbiome in the first year of age following intrapartum penicillin prophylaxis for maternal GBS colonization.

INTRODUCTION: Group B Streptococcus is a significant cause of early-onset disease in term newborns, with a global incidence of 0.41/1000 live births. Intrapartum antibiotic prophylaxis (IAP) has reduced EOD incidence by over 80%, but concerns exist about its impact on the neonatal gut microbiome and potential long-term health effects. METHODS: This single center study examines the effects of IAP on the fecal infant microbiome in the first year of age and on the T cell phenotype in the first days after birth among 22 infants receiving IAP with penicillin due to maternal GBS colonization and 26

Pharmacokinetics of Intrapartum Benzylpenicillin: Insights Into Candidate Regimens to Prevent Early Onset Neonatal Group B Streptococcus Disease.

Early onset neonatal Group B Streptococcus (GBS) infection accounts for significant global morbidity and mortality. Intrapartum prophylaxis with benzylpenicillin is advised for women at high risk of having a baby affected by early onset GBS disease. Pregnancy-related physiological changes can alter pharmacokinetics. To estimate the intrapartum pharmacokinetics of penicillin, women (n = 12) at risk of GBS disease were enrolled. A fixed regimen of intravenous benzylpenicillin 3 g at onset of labor and 1.5 g every 4 h until delivery was used. Benzylpenicillin concentrations in plasma and umbilica

Genotypic and phenotypic characteristics of group B streptococcal isolates from neonates in a tertiary hospital in Qingyang, Gansu Province, China.

Group B Streptococcus (GBS) causes severe neonatal infections, leading to high mortality rates. This study aimed to investigate the genetic characteristics of GBS isolates in a hospital setting. Nineteen isolates were identified through a review of preservation and cultivation processes since January 2022, followed by antibiotic susceptibility testing. Whole-genome sequencing (WGS) was used to investigate genetic relationships. Among 19 isolates, serotype V was the most prevalent (n = 7), followed by Ib (n = 6) and III (n = 6). Genomic sequence analysis revealed that 19 isolates belonged to fo

[WHO recommendation on screening of pregnant women for intrapartum antibiotic prophylaxis for the prevention of early onset Group B streptococcus disease in newborns].

Group B Streptococcus (GBS) colonisation affects around 13% of pregnant women and increases the risk of peripartum infection in newborns, preterm delivery and premature rupture of membranes. GBS infections are a major cause of neonatal sepsis. Early onset GBS (EOGBS) infections in the first seven days of life are particularly dangerous for newborns. Prevention is achieved by intrapartum antibiotic prophylaxis following a positive GBS screening during pregnancy. Screening strategies in pregnant women include universal GBS screening using a rectovaginal swab, assessment of maternal risk factors

Sexual activity, vaginal symptoms, maternal perineal hygiene behavior, and constipation on ano-vaginal colonization of group B streptococcus in near term pregnancy.

BACKGROUND: Maternal Group B Streptococcus (GBS) colonization is influenced by many factors but results are inconsistent. Consideration of antenatal risk factors may help inform decision making on GBS microbiological culture screening where universal screening is not standard of care. We sought to identify independent predictors of GBS colonization at 34-37 weeks gestation incorporating vaginal symptoms, perineal hygiene measures, sexual activity, and a potential novel factor, constipation. METHODS: In this prospective cross-sectional study, 573 women at 34-37 weeks gestation had an ano-vagin

Alternative antimicrobials for prophylaxis of the Group B Streptococcus maternal-fetal disease.

INTRODUCTION: GBS colonization is an important risk factor for maternal and neonatal infection morbidity and mortality. Intrapartum antibiotics may prevent vertical transmission of GBS from colonized mothers to their babies. The objective of this study was to evaluate the effectiveness of cefazolin prophylactic regimen for GBS disease, comparing it to the established penicillin-based protocols, given the opportunity provided by the temporary unavailability of first-choice antibiotics in Brazil. METHODOLOGY: A retrospective analysis was conducted at the Hospital Femina Obstetrics Service betwe
