3D mapping of cerebrospinal fluid local volume changes in patients with hydrocephalus treated by surgery: preliminary study.

OBJECTIVE: To develop automated deformation modelling for the assessment of cerebrospinal fluid (CSF) local volume changes in patients with hydrocephalus treated by surgery. METHODS: Ventricular and subarachnoid CSF volume changes were mapped by calculating the Jacobian determinant of the deformation fields obtained after non-linear registration of pre- and postoperative images. A total of 31 consecutive patients, 15 with communicating hydrocephalus (CH) and 16 with non-communicating hydrocephalus (NCH), were investigated before and after surgery using a 3D SPACE (sampling perfection with app

Thoracolumbar intradural extramedullary bronchiogenic cyst.

Intradural extramedullary bronchiogenic cysts are rare findings. All five reported cases were located cervically or upper thoracically. To our knowledge, we describe the first case of an intraspinal bronchiogenic cyst in a thoracolumbar location. We present the case of a 41-year-old patient with a known spina bifida occulta who suffered from a continuous, sharp, and therapy-refractory pain in the left leg. Magnetic resonance imaging of the thoracic and lumbar vertebra revealed an intradural extramedullar mass at T12 to L1 level. After laminectomy T-12 through L-1/L-2 and longitudinal opening o

Attenuation of cerebral vasospasm after subarachnoid hemorrhage in mice overexpressing extracellular superoxide dismutase.

BACKGROUND AND PURPOSE: Subarachnoid hemorrhage (SAH) increases production of vascular extracellular superoxide anion (*O2-). We examined whether overexpression of murine extracellular superoxide dismutase (EC-SOD) alters SAH-induced cerebral vasospasm, oxidative stress, and neurological outcome. METHODS: Mice exhibiting a 2-fold increase in vascular EC-SOD and wild-type (WT) littermates were subjected to sham surgery or SAH by perforation of the right anterior cerebral artery. Neurological deficits were scored 72 hours later. Middle cerebral artery (MCA) diameter was measured or immunohistoc

Leptomeningeal disease in pre-leukemic syndrome: cytogenetic abnormality versus cellular morphology.

Refractory anemia with excess blasts in transformation (RAEB-T) is a pre-leukemic syndrome that can progress to acute myelogenous leukemia (AML). Although leptomeningeal disease in AML is rare and it is commonly associated with specific cytogenetic abnormalities, it has not been documented in RAEB-T. We report a woman who experienced seizures and had subarachnoid enhancement on MRI suggestive of leukemic infiltrates in the leptomeninges. Her bone marrow aspirate revealed 6.6% blasts and the presence of Auer rods, findings consistent with a diagnosis of RAEB-T. Subsequent cytogenetic analysis d

Construction and evaluation of a diagnostic prediction model for bacterial meningitis based on clinical and laboratory data.

Bacterial meningitis refers to the rapid inflammation of the meninges caused by bacteria or their byproducts, impacting the pia mater, arachnoid mater, and the subarachnoid space. This condition is a serious infectious illness affecting the central nervous system, if not diagnosed and treated promptly, it may result in severe neurological complications or even fatalities, making prompt and precise diagnosis essential for better outcomes. The objective of this research was to develop and assess a diagnostic prediction model for bacterial meningitis utilizing clinical and laboratory information.…

Microsurgical repair of lumbar Segmental Myelocystocele and spinal cord Untethering: 2-Dimensional operative video.

Lumbar (non-terminal) myelocystocele is a rare form of closed spinal dysraphism which is characterized by posterior bony defect [1-3], a herniated segment of the spinal cord associated with cystic dilatation of the central canal [1-3], and surrounded by cyst filled with CSF in the subarachnoid space i.e. cyst-within-a-cyst. [1-4] The mass is covered by intact skin and variable amounts of subcutaneous fat which is often attached to neural tissue. [1-3] Surgery is advocated to untether the cord and reconstruct the neural tube which will prevent further neurological deterioration. [1-3] In this v…

Glial scarring limits recovery following decompressive surgery in rats with syringomyelia.

Syringomyelia is a neurological disease that is difficult to cure, and treatments often have limited effectiveness. In this study, a rat model of syringomyelia induced by epidural compression was used to investigate the factors that limit the prognosis of syringomyelia. After we treated syringomyelia rats with surgical decompression alone, MRI revealed that the syringomyelia rats did not show the expected therapeutic effect. Through cerebrospinal fluid (CSF) tracing experiments, we found that the CSF flow in the subarachnoid space (SAS) of rats was restored after decompression. This shows that…

Endoscopic Ventriculocysternostomy, Magendie Foraminoplasty, and Plexusectomy With Craniovertebral Shunt Placement in a Pediatric Patient With Hydrocephalus and VACTERL Association: A Novel Treatment Option.

Endoscopic third ventriculocysternostomy (ETV) is a minimally invasive neurosurgical technique with good results in the treatment of obstructive hydrocephalus. The VACTERL (vertebrae, anorectal, cardiovascular, tracheal, esophageal, renal, limb defects) association, or VATER syndrome, is defined as congenital malformations, mostly derived from the mesoderm, affecting specific areas. It is diagnosed by the presence of at least three of the seven characteristic malformations that describe it. The association of this pathology and obstructive hydrocephalus in pediatric age is not common, making m

Insights on pathophysiology of hydrocephalus rats induced by kaolin injection.

Hydrocephalus can affect brain function and motor ability. Current treatments mostly involve invasive surgeries, with a high risk of postoperative infections and failure. A successful animal model plays a significant role in developing new treatments for hydrocephalus. Hydrocephalus was induced in Sprague-Dawley rats by injecting 25% kaolin into the subarachnoid space at the cerebral convexities with different volumes of 30, 60 and 90 μL. Magnetic resonance imaging (MRI) was performed 1 month and 4 months after kaolin injection. The behavioral performance was assessed weekly, lasting for 7 wee…

Rare Combination of Abducens Nerve Palsy and Optic Neuritis on the Same Side: Case Report and Review of 8 Patients in Literature.

The concurrent development of abducens nerve palsy and optic neuritis on the same side is rare. Here we presented an 82-year-old man who developed the combination of abducens nerve palsy and optic neuritis on the left side 2 months after the sixth inoculation of COVID-19 mRNA vaccine. In past history at 45 years old, he experienced subarachnoid hemorrhage and underwent surgery for the clipping of intracranial aneurysm. The patient had no systemic symptoms, such as general fatigue, fever, arthralgia, and skin rashes. Physical and neurological examinations were also unremarkable. Since the aneur
