Use of Supinator Motor Branches in Targeted Muscle Reinnervation of the Superficial Radial Nerve.

Symptomatic neuromas of the superficial radial nerve (SRN) can cause debilitating pain. Traditional surgical management options have demonstrated inconsistent outcomes prompting a search for alternatives. Recent reports have emerged on the use of targeted muscle reinnervation (TMR) for neuromas of the SRN using donors that are well established in hand surgery, such as the brachioradialis (BR) or extensor capri radialis longus or brevis. Use of the brachioradialis or extensor capri radialis longus motor targets can require surgery at or above the level of the antecubital fossa, and denervation

A case report: New technique of proximal lengthening for treatment of injection induced rectus femoris muscle contracture.

UNLABELLED: Contracture of quadriceps femoris muscle is an uncommon condition in clinical practice. It was suggested that intramuscular injections into the thigh for children are sometimes the cause of this condition. CASE REPORT: The case presents a new surgical technique for the treatment of isolated rectus femoris muscle contracture. The patient is a 6- year-old boy complaining of external rotation of the left leg when he walks and inability to squat. Clinical examination showed positive Ely's test which indicated isolated rectus femoris muscle contracture. The patient underwent proximal l

Current advancements in diagnosing and managing cavovarus foot in paediatric patients.

The cavovarus deformity is a pathological condition characterised by an anomalous elevation of the longitudinal arch. This condition results … ating malalignment. Various surgical procedures have been recorded, including soft tissue releases, tendon transfers, osteotomies, and arthrodesis, which are selected based on the nature and extent of the deformity assessment findings, with the ultimate goal of reaching a foot that is both plantigrade and balanced. Due to a lack of research on this topic, the pre …

Hierarchical Serpentine-Helix Combination for 3D Stretchable Electronics.

3D stretchable electronics attract growing interest due to their new and more complex functionalities compared to 1D or 2D counterparts. Among all 3D configuration designs, a 3D helical structure is commonly used as it can be designed to achieve outstanding stretching ratios as well as highly robust mechanical performance. However, the stretching ratio that mainly focuses on the axis direction hinders its applications. Inspired by hierarchies in a tendon, a novel structural design of hierarchical 3D serpentine-helix combination is proposed. The structural design constructed by a sequence with

The Medial Sural Artery Perforator Flap: Lessons Learned from 200 Consecutive Cases.

BACKGROUND: The medial sural artery perforator flap is a versatile option in reconstructive microsurgery. However, most reports of this flap have been limited by sample size. This study reviews the experience of a single large-volume center using the medial sural artery perforator flap and its variants for a variety of reconstructive purposes. METHODS: A retrospective review of all patients who received the medial sural artery perforator flap reconstruction between March of 2006 and July of 2014 was performed. RESULTS: Two-hundred medial sural artery perforator flaps were used at three anato

Foot salvage using microsurgical free muscle flaps in severely crushed foot with soft tissue defects.

OBJECTIVE: Management of complex foot injuries including skin, tendons, vessels, bone, with soft tissue defects is considered as an orthopedic challenge. Microsurgical free muscle flaps provide the best solution in such cases. PATIENTS AND METHODS: Thirty two consecutive cases of complex ankle and foot injuries were treated by skin grafted muscle free flap in one stage procedure after radical debridement. Twenty four were males and 8 were females. Right foot was affected in 26 and left was involved in 6 patients. The average age was 22 years (range 6-33 years). The most common cause of injury

[A case of pure-sensory-type Guillain-Barré syndrome with galactocerebroside antibody].

A 67-year-old man noticed paresthesia in both legs in July 2011. Three days later, he was found on a street where he was unable to stand up. On admission, the deep sensation in both legs was severely disturbed, but their muscle strength remained normal. Cranial nerves and autonomic functions were normal. The deep tendon reflexes were diminished in both legs. Magnetic resonance imaging of the spine was normal. Motor nerve conduction studies revealed normal conduction velocity, amplitude, and F-wave latency. However, sensory nerve conduction studies revealed severe reduction of amplitude in the

Alternative lymphatic drainage routes from the lateral heel to the inguinal lymph nodes: anatomic study and clinical implications.

BACKGROUND: The route of lymphatic drainage from the heel to the inguinal lymph nodes is required to be accurately evaluated for clinical needs. METHODS: Seven lower limbs from four unembalmed human cadavers were studied. Under a surgical microscope, 6% hydrogen peroxide was used to detect the lymphatic vessel on the lateral side of the heel. The vessel was then injected with a radio-opaque lead oxide mixture. The vessel was traced, photographed and radiographed to demonstrate the lymphatic pathways from the lateral heel to the inguinal lymph nodes. The final results were transferred to the c

Management of severe crouch gait in children and adolescents with cerebral palsy.

BACKGROUND: Crouch gait in cerebral palsy is associated with spasticity and contracture of the hamstrings and weakness of the extensors of the hip and knee and ankle plantar flexors. Different treatment options have been described in the literature to deal with this difficult problem. We devised a different protocol of treatment aimed at correction of the flexion deformity of the knee, weakening of the hamstrings, and augmenting the power of the knee and hip extension, which we used on 17 children with severe crouch. METHODS: This surgery, performed in 2 stages, entailed shortening of the fem

Fourth-degree burns to the lower extremity with exposed tendon and bone: a ten-year experience.

Fourth-degree extremity burns involve muscle, tendon, and bone, often leading to amputation or significant functional impairment. We report our 10-year experience (1995-2004) at an urban burn center with fourth-degree burns to the lower extremity to characterize treatments and outcomes. Twenty-one patients (40 limbs), mean age of 45 years, were treated for fourth-degree lower-extremity burns with the average extremity burn size of 24% TBSA (range, 2-36%) and a mean fourth-degree burn size of 9% TBSA (range, 2-18%). A mean of eight operations were required for limb salvage. Six free-tissue tran

Short Expandable-Wing Suture Anchor for Osteoporotic and Small Bone Fixation: Validation in a 3D-Printed Coracoclavicular Reconstruction Model.

Suture anchors are widely used for tendon and ligament repair, but their fixation strength is compromised in osteoporotic bone and limited bone volume such as the coracoid process. Existing designs are prone to penetration and insufficient cortical engagement under such conditions. In this study, we developed a novel short expandable-wing (SEW) suture anchor (Ti6Al4V) designed to enhance pull-out resistance through a deployable wing mechanism that locks directly against the cortical bone. Finite element analysis based on CT-derived bone material properties demonstrated reduced intra-bone displ

Talus osteoid osteoma misdiagnosed as ankle synovitis: A case report in rehabilitation therapy.

RATIONALE: Osteoid osteoma, accounting for approximately 10% of benign bone tumors, is predominantly found in long bones and rarely in the foot bones, such as the talus. Its nonspecific symptoms, such as nocturnal pain and swelling, often lead to misdiagnosis, especially when it mimics conditions like ankle synovitis. PATIENT CONCERNS: A 27-year-old male presented with persistent pain and swelling in his left ankle following an injury at the gym. Initial treatments, including arthroscopic debridement, failed to resolve his symptoms. DIAGNOSES: Comprehensive evaluation using magnetic resonanc

The influence of risk factors on clinical outcomes following anatomical medial patellofemoral ligament (MPFL) reconstruction using the gracilis tendon.

PURPOSE: Patellofemoral instability is influenced by ligamentous, boney and neuromuscular factors. The most important variables are trochlea geometry, medial patellofemoral ligament (MPFL), patella height, tibial tuberosity-trochlea groove distance (TT-TG) and the extensor muscles. Treatment is complicated by these multifactorial conditions. This prospective study examined the influence of risk factors on clinical results and athletic activities where treatment was confined to ligamentous procedures only. METHODS: Fifty patients with chronic patellofemoral instability were treated with MPFL r

Negative pressure wound therapy for the treatment of infected wounds with exposed knee joint after patellar fracture.

Treatment of soft tissue defects with exposed bones and joints, resulting from trauma, infection, and surgical complications, represents a major challenge. The introduction of negative pressure wound therapy has changed many wound management practices. Negative pressure wound therapy has recently been used in the orthopedic field for management of traumatic or open wounds with exposed bone, nerve, tendon, and orthopedic implants. This article describes a case of a patient with a large soft tissue defect and exposed knee joint, in which negative pressure wound therapy markedly improved wound he

Management of large infected tibial defects with radical debridement and staged double-rib composite free transfer.

Seven patients with tibial fractures complicated by large infected tibial defects were treated with a two-stage protocol. In the first stage, antibiotic-impregnated polymethylmethacrylate (PMMA) bead chains were used to obliterate the debrided osseous defect, and a meshed porcine skin was used for temporary wound coverage. In the second stage, the bead chains were removed, and the defects were reconstructed with a microvascularized double-rib and serratus anterior muscle composite free transfer. The interval between the first and second operations was 2 to 4 weeks. The bone defects ranged from
