Asymmetric Cortical Vessel Sign Indicates Hemodynamic Deficits in Adult Patients with Moyamoya Disease.

BACKGOUND: Asymmetric cortical vessel sign (ACVS) on susceptibility-weighted imaging (SWI) indicates elevated concentration of deoxyhemoglobin and elevated oxygen extraction fraction in patients with cerebral ischemia. This study aimed to clarify whether ACVS is associated with impaired hemodynamics and hyperperfusion syndrome in patients with moyamoya disease (MMD). METHODS: Consecutive adult patients with MMD were enrolled. ACVS data on SWI and perfusion data using dynamic perfusion computed tomography were obtained and evaluated preoperatively and on postoperative days 2 and 180. RESULTS:

Decision Making in Surgery for Nonsaccular Posterior Inferior Cerebellar Artery Aneurysms With Special Reference to Intraoperative Assessment of Collateral Blood Flow and Neurophysiological Function.

BACKGROUND: Posterior inferior cerebellar artery (PICA) aneurysms represent a challenging pathology. PICA sacrifice is often necessary, due to the high proportion of nonsaccular aneurysms that can be found in this location. Several treatments are available, but the infrequency of these aneurysms and the increasing number of endovascular techniques have limited the development of a standardized algorithm for cases in which open surgery is indicated. OBJECTIVE: We present our series of nonsaccular PICA aneurysms, in the attempt to define an algorithm for their surgical management. METHODS: We

Interdisciplinary Prevention and Management of Wound-Related Complications in Extracranial-to-Intracranial Bypass Surgery.

Extracranial-to-intracranial (EC-IC) bypass surgery may be necessary in patients with moyamoya disease and other ischemic conditions. However, there is a potential risk of wound-related complications in some cases. In this study, we report our approach to the prevention of wound-related complications in EC-IC bypass. Technical considerations and pitfalls of surgery are also discussed. This study included 89 patients with ischemic-onset moyamoya disease and atherosclerotic disease who underwent 108 superficial temporal artery (STA)-to-middle cerebral artery bypass procedures. Our study emphasiz

Adjuvant revascularization of intracranial artery occlusion with angioplasty and/or stenting.

INTRODUCTION: In conjunction with intravenous and/or intra-arterial thrombolysis, adjuvant revascularization of intracranial artery occlusion by angioplasty vs. stenting remains controversial. We evaluated outcome in patients with intracranial occlusion after angioplasty and/or stenting. MATERIALS AND METHODS: Thirty-three patients who underwent angioplasty or stenting (17 stenting and 16 angioplasty) for intracranial arterial occlusion during the past 5 years were enrolled from prospective neurointerventional database. We compared recanalization rate [defined as thrombolysis in myocardial in

Ischemic stroke due to dissection of intracranial internal carotid artery: implications for early surgical treatment.

BACKGROUND: Intracranial ICA dissection is a rare pathology that frequently results in severe morbidity or mortality in young patients. This study examined the suitability of early surgical treatment. METHODS: We present and analyze 6 of our own cases and 39 other cases reported after 1980. The clinical courses were classified into 3 types: A, single attack and severe outcome; B, single attack and mild to moderate outcome; and C, recurrent attacks and various outcomes. RESULTS: Type A patients (n = 11) usually deteriorated quickly and severely so that early surgical treatment was not possibl

Enhanced brain angiogenesis in chronic cerebral hypoperfusion after administration of plasmid human vascular endothelial growth factor in combination with indirect vasoreconstructive surgery.

OBJECT: Vascular endothelial growth factor (VEGF) is a secreted mitogen associated with angiogenesis. The conceptual basis for therapeutic angiogenesis after plasmid human VEGF gene (phVEGF) transfer has been established in patients presenting with limb ischemia and myocardial infarction. The authors hypothesized that overexpression of VEGF using a gene transfer method combined with indirect vasoreconstruction might induce effective brain angiogenesis in chronic cerebral hypoperfusion, leading to prevention of ischemic attacks. METHODS: A chronic cerebral hypoperfusion model induced by perman

Pre-Therapeutic Vascular Anatomical Evaluation in Penetrating Cerebrovascular Injuries: Insights from Two Cases.

Penetrating cerebrovascular injuries (PCVIs) are often life-threatening, with trauma to the carotid or vertebral arteries potentially causing severe neurological deficits. This study aimed to retrospectively examine two cases of severe penetrating cerebrovascular injury with unstable hemodynamics that resulted in markedly different neurological outcomes. The first patient was struck by a high-speed metal rod, sustained injuries to the right common carotid and right vertebral arteries and was admitted to our hospital 10 h after the trauma. The second patient, who was stabbed, sustained an injur

Biomimetic dynamic nanomedicine SrHCF: Lattice self-reconstruction based on Fe2+ response to achieve synergistic "ferrous chelation-antioxidation-calcium antagonism-inflammation inhibition" quadruple treatment of CIRI.

Revascularization has revolutionized the treatment of ischemic stroke (IS), but its efficacy is limited by cerebral ischemia-reperfusion injury (CIRI). CIRI involves the dynamic interaction of multidimensional pathological mechanisms such as ferroptosis, calcium overload, oxidative stress, and subsequent inflammatory cytokine storm, which make many emerging single-target therapeutic strategies limited in alleviating CIRI. Strategies that simultaneously and synergistically target multiple key pathological factors in CIRI remain highly anticipated but extremely challenging. This study constructe

Endovascular recanalization of symptomatic long-segment nonacute vertebral artery occlusion: a case report.

BACKGROUND: Vertebral artery occlusive disease is a leading cause of posterior circulation ischemic stroke. Endovascular recanalization has been described as a viable treatment option for individuals with symptomatic vertebral artery occlusion who fail to respond to appropriate medicinal therapy. However, endovascular recanalization of symptomatic long-segment nonacute vertebral artery blockage is associated with significant surgical risk and the possibility of perioperative complications. CASE PRESENTATION: Despite rigorous pharmacologic therapy, a patient with a protracted segmental left ve

Pathological Findings of Donor Vessels in Bypass Surgery.

(1) Background Cerebral revascularization is necessary to treat intracranial arterial stenosis caused by moyamoya disease, atherosclerosis, or large complex aneurysms. Although various donor vascular harvesting methods have been reported safe, there are no reports on the histological evaluation of donor vessels for each disease, despite the variety of diseases wherein vascular anastomosis is required. (2) Methods Pathological findings of the superficial temporal artery (STA), radial artery (RA), occipital artery (OA), and saphenous vein (SV) harvested at the institution were analyzed. Patients…

Regional computed tomography perfusion deficits in patients with hypoglycemia: two case reports.

BACKGROUND: Hypoglycemia in patients with diabetes mellitus, particularly type 1 can mimic acute ischemic stroke by causing focal neurological deficits. In acute ischemic stroke, the interpretation of emergency imaging including computed tomography with angiography and perfusion is crucial to guide revascularizing therapy including intravenous thrombolysis. However, different metabolic abnormalities and stroke mimics can cause focal hypoperfusion. METHODS: We describe two type 1 diabetes patients presenting with acute focal neurological deficits and hypoglycemia, who underwent multimodal comp

Treatment of intracranial vertebral artery dissecting aneurysms involving the posterior inferior cerebellar artery origin.

BACKGROUND: Vertebral artery dissecting aneurysm (VADA) involving the origin of the posterior inferior cerebellar artery (PICA) is a complex disease entity in which the dual goals of preventing future rebleeding and maintaining perfusion of the lateral medulla must be considered. We present an illustrative case and review the literature surrounding treatment strategies. CASE DESCRIPTION: We report a patient presenting with extensive subarachnoid hemorrhage due to rupture of an intracranial VADA involving the PICA origin. After consideration of the patient's cerebral vasculature and robustness

Should We Treat a Patient's Symptoms or Angiography Image in TIA?: Two Case Reports.

OBJECTIVES: Data on vascular status in the first hours after onset of transient ischemic attack (TIA) and its clinical significance are missing. Also, it is not known whether arterial occlusion, if present in TIA, should be the target for revascularization. We present 2 patients to demonstrate that TIA can be due to acute major intracranial arterial occlusion and to show how such arterial occlusion may affect the outcome. CASE REPORTS: Two patients (54 and 63 years old) were diagnosed with TIA and at the same time had occlusion of the middle cerebral artery. No recanalization therapy was perf
