Digital Cognitive Behavioral Treatment for Generalized Anxiety Disorder: A Randomized Clinical Trial.

IMPORTANCE: Generalized anxiety disorder (GAD) is a chronic and disabling condition. Although cognitive behavioral therapy (CBT) is a first-line treatment for GAD, access to this treatment is limited; digital CBT (DCBT) programs may have the potential to overcome barriers to effective treatment and expand therapist reach and impact. OBJECTIVE: To evaluate the effectiveness of smartphone-delivered DCBT compared with an active psychoeducation control condition for the treatment of GAD. DESIGN, SETTING, AND PARTICIPANTS: This single-blind, parallel-group, decentralized randomized clinical trial

Longitudinal modeling of health-related quality of life trajectories over 12 months following road trauma.

INTRODUCTION: This study identified the potential phases of health-related quality of life (HRQoL) change over a year following road trauma (RT) injury, as well as its predictors. METHODS: This inception cohort study recruited 1480 Canadian RT survivors from July 2018 to March 2020. HRQoL outcome was assessed with the 5-level version of the EuroQol (EQ-5D-5L) instrument at baseline (pre-injury) and at 2, 4, 6, and 12 months post-injury. Predictors of HRQoL included sociodemographic, psychological, medical, and trauma-related factors collected at baseline. We applied generalized additive mixed

Aversive Conditioning of Spatial Position Sharpens Neural Population-Level Tuning in Visual Cortex and Selectively Alters Alpha-Band Activity.

Processing capabilities for many low-level visual features are experientially malleable, aiding sighted organisms in adapting to dynamic environments. Explicit instructions to attend a specific visual field location influence retinotopic visuocortical activity, amplifying responses to stimuli appearing at cued spatial positions. It remains undetermined both how such prioritization affects surrounding nonprioritized locations, and if a given retinotopic spatial position can attain enhanced cortical representation through experience rather than instruction. The current report examined visuocorti

The SUMMIT ambulatory-ICU primary care model for medically and socially complex patients in an urban federally qualified health center: study design and rationale.

BACKGROUND: Medically complex urban patients experiencing homelessness comprise a disproportionate number of high-cost, high-need patients. There are few studies of interventions to improve care for these populations; their social complexity makes them difficult to study and requires clinical and research collaboration. We present a protocol for a trial of the streamlined unified meaningfully managed interdisciplinary team (SUMMIT) team, an ambulatory ICU (A-ICU) intervention to improve utilization and patient experience that uses control populations to address limitations of prior research.

[Abuse of alcohol and benzodiazepine during substitution therapy in heroin addicts: a review of the literature].

INTRODUCTION: In spite of its seriousness, dependence on alcohol and benzodiazepines during substitution treatment are poorly documented. Its frequency is nonetheless significant. According to studies, between one and two thirds of patients are affected. This consumption is under verbalized by patients and underestimated by carers. In one study, where the average diazepam doses were from 40 to 45 mg per day, 30% of the patients were taking 70 to 300 mg per day, two thirds having experimented with a fixed dose of 100mg. Benzodiazepines, especially diazepam and flunitrazepam, were studied versus

Digital Psychosocial Interventions Tailored for People in Opioid Use Disorder Treatment: Scoping Review.

BACKGROUND: A total of 60% of patients with opioid use disorder (OUD) leave treatment early. Psychosocial interventions can enhance treatment retention by addressing behavioral and mental health needs related to early treatment discontinuation, but intervention engagement is key. If well-designed, digital platforms can increase the engagement, reach, and accessibility of psychosocial interventions. Prior reviews of OUD treatment have predominantly focused on outcomes, such as reductions in substance use, without identifying the underlying behavior change principles that drive the effectiveness

Left Write Hook: trial protocol for a community-based type II hybrid effectiveness-implementation cluster randomised controlled trial of a boxing and writing-based intervention for adult survivors of child sexual abuse and gender-based violence in Australia.

INTRODUCTION: Sexual and gender-based violence can have long-term impacts on the physical and mental health of survivors, with demonstrated impairments to immune, endocrine and nervous systems, and increased risk of chronic conditions such as cardiovascular disease, depression and post-traumatic stress. Moreover, survivors commonly experience low self-efficacy and lack of perceived control over their lives. Creating space for survivors to feel empowered through a multidimensional approach to health promotion, considering both physical and psychological influences on health, is necessary to red

Integrating Wearable Sensor Data With an AI-Based, Protocol-Flexible Triage Platform to Accelerate Decision-Making During the Golden Hour of Combat Casualty Care.

Introduction Rapid, accurate triage within the first "golden hour" on the battlefield is critical to survival, yet existing wearable sensors and manual protocols operate in isolation, delaying care and creating inconsistent priority ratings across military and civilian systems. We therefore designed a vendor-agnostic, protocol-flexible platform that ingests live biometric streams from military-grade wearables, applies an artificial intelligence (AI)-enhanced hybrid decision engine, and returns colour-coded casualty categories under any standard (e.g., MARCH (Massive hemorrhage, Airway, Respira

Group Cognitive Behavioral Therapy With Virtual Reality Exposure Versus In-Vivo Exposure for Social Anxiety Disorder and Agoraphobia: Underpowered Results From the SoREAL Pragmatic Randomized Clinical Trial.

BACKGROUND: Social anxiety disorder (SAD) and agoraphobia are common, impairing conditions often treated with cognitive behavioral therapy (CBT) conducted in groups. In CBT, exposure therapy is a core element. However, in-vivo exposure therapy is logistically challenging and aversive for both patient and therapist, especially in a group context, often leading to exposure being skipped altogether in clinical practice. Virtual reality exposure (VRE), in which phobic stimuli are presented through immersive virtual reality technology, has shown promise as a flexible alternative to in-vivo exposure

Multimodal subspace independent vector analysis effectively captures the latent relationships between brain structure and function.

A key challenge in neuroscience is to understand the structural and functional relationships of the brain from high-dimensional, multimodal neuroimaging data. While conventional multivariate approaches often simplify statistical assumptions and estimate one-dimensional independent sources shared across modalities, the relationships between true latent sources are likely more complex - statistical dependence may exist within and between modalities, and span one or more dimensions. Here we present Multimodal Subspace Independent Vector Analysis (MSIVA), a methodology to capture both joint and un

Seeking safety intervention for comorbid post-traumatic stress and substance use disorder: A meta-analysis.

PROBLEM STATEMENT: Seeking Safety (SS) is a widely implemented cognitive-behavioral therapy for comorbid post-traumatic stress disorder (PTSD) and substance use disorder (SUD). It is a present-focused coping skills model that is highly flexible, with varied methods of delivery, to maximize acceptability and client access. The purpose of this meta-analysis is to examine the effect of SS on comorbid PTSD and SUD across randomized control trials (RCTs). In addition, ours is the first meta-analysis to examine the dose-response of SS by comparing delivery of all 25 SS topics versus fewer. METHODS

Clinical stance on response initiation in autistic adults: co-creating an integrative approach based on theory and lived experiences to act from language, via motor movement to wellbeing.

Getting 'stuck', literally and figuratively, is a common experience for autistic people. Literally 'stuck' means exhibiting limited response initiation due to immobility with tense muscles and inability to move. Figuratively 'stuck' means loneliness, passivity or captivity in activities that do not offer long-term satisfaction. To further conceptualize this complex phenomenon of limited response initiation in autistic individuals, we performed qualitative interviews and focus groups with autistic people and their family members, followed by brainstorm sessions and a Delphi study with input fro

Efficacy of an Internet-based intervention with self-applied exposure therapy in virtual reality for people with panic disorder: study protocol for a randomized controlled trial.

BACKGROUND: Due to several treatment barriers, many individuals with panic disorder do not receive evidence-based treatment. One promising option to narrow this treatment gap is Internet-based psychotherapy, which has been shown particularly effective in guided formats. Still, there remains room for improvement to make these digital therapies more accessible, cost-efficient, and aligned with best practices for in-person interventions (e.g., exposure). The smartphone app "Invirto - Treatment for Anxiety" offers digitally guided, evidence-based treatment of panic disorders including virtual real

Safety, Tolerability and Efficacy of Eslicarbazepine Acetate as Adjunctive Therapy in Patients Aged ≥ 65 Years with Focal Seizures.

BACKGROUND: The incidence of epilepsy is high within the first few years of life, stabilizes over the second through fifth decades, and then rises again. Treatment of elderly patients with antiepileptic drugs (AEDs) is complicated by increased sensitivity to drug effects, altered pharmacokinetics and an increased risk for drug interactions due to polytherapy. On the other hand, the safety and efficacy data of AEDs attained during clinical development programmes are relatively limited for this age group. OBJECTIVE: The aim of this study was to evaluate the safety, tolerability and efficacy of

[Behavioral mechanisms of spatial competition between red wood ants (Formica aquilonia) and ground beetles (Carabidae)].

Behavioral aspects of spatial competition between red wood ants (Formica aquilonia) and six mass species of Carabidae were studied in field and laboratory experiments. We showed that red wood ants essentially influence spatial distribution of ground beetles on their common territories. Transplantation experiments suggest that in newly established ants' settlements stronger forms of interrelations arise than in old stable colony. To examine the ability of beetles to avoid collisions with ants we used two experimental techniques. In laboratory, we tested carabids ability to avoid a clash in a Y-
