Assessment and treatment of abstinence in the infant of the drug-dependent mother.

To accomplish the assessment and treatment of abstinence in the infant of the drug dependent mother, a scoring system has been devised for use as a clinical and research tool. The score monitors the passively addicted infant in a more comprehensive and precise way than has previously been described, and permits us to be more objective in our clinical estimates of the abstinence syndrome. Further, this scoring system has been used by relating it to the dosage schedule of phenobarbital or paregoric as part of an ongoing research project designed to test the comparative usefulness of recommended

Opioid treatment for opioid withdrawal in newborn infants.

BACKGROUND: Neonatal abstinence syndrome (NAS) due to opioid withdrawal may result in disruption of the mother-infant relationship, sleep-wa … ent (seizure) was reported in infants exposed to buprenorphine. Morphine versus diluted tincture of opium (DTO): a single study (33 infants) reported no difference in days hospitalisation, days treatment or weight gain (low certainty evidence). Opioid versus clonidine: a single study (31 infants) reported no infant with treatment failure in either group …

Co-existence of HIV-1 subtypes B' and E infections among Thai injecting drug users.

Subtypes B' and E are the two major subtypes of HIV-1 among injecting drug users (IDU) in Thailand. However, there are not many reports on subtype distribution during the early epidemic. Random blood specimens collected during 1994-2000 from 3,286 IDU at the Thanyarak Hospital were tested for HIV antibody and subtyped by using peptide binding enzyme immunoassay. The prevalence rate of HIV infection was 36.8%. All HIV-seropositive IDU were ascertained for "year of first HIV seropositivity" from their medical records. Of 1,512 HIV-seropositive samples, 1,408 (93.1%) were typeable. During 1987-19

[Immunomodulatory role of opioids in the central nervous system].

Endogenous and exogenous opioids can influence and modulate neuronal and glial cell function via an opioid receptor mediated mechanism, leading to either protection or damage of the brain. Opiates such as morphine have been postulated to promote the progression of HIV-1 and the development of secondary opportunistic infections. Kappa opioid receptor ligands, on the other hand, may play a neuroprotective role. More studies are needed to delineate how opioids exert their effects on glial cells as well as neurons with the goal of developing new therapeutic approaches for neurodegenerative disease

Comparative trial of rectal indomethacin and cryoanalgesia for control of early postthoracotomy pain.

A randomised double blind trial was carried out over the first two days after thoracotomy to compare the analgesic effects of rectal indomethacin 100 mg administered eight hourly, cryoanalgesia, and a combination of both of these with the effects of conventional intramuscular opiate analgesia. Pain scores were significantly reduced with both rectal indomethacin alone and cryoanalgesia alone; these treatments had an additive effect when used in combination. Pain on movement was significantly increased, and indomethacin was more effective in reducing this than cryoanalgesia. Groups receiving eit

Search for an endogenous ligand for the opiate receptor.

It was investigated whether there is an endogenous factor in the brain which binds to the opiate receptor in neural tissue. Extracts from rat brain were processed in different ways; fractions were assayed for ability to inhibit the receptor binding of dihydromorphine. There was no evidence for high-molecular weight substances or lipid soluble substances with such ability. On the other hand, processing of an acid water extract of brain except the cerebellum (which was negative) yielded an active fraction with receptor blocking activity. This fraction was heat stable, polyionic and had an appare
