[Growth and deformity after in situ fixation of slipped capital femoral epiphysis].

BACKGROUND: For mild to moderate slipped capital femoral epiphysis (SCFE) in situ fixation is the current treatment standard. However, concerning the implant selection (screw versus k-wires) as well as the prophylactic stabilisation of the non-affected hip, controversies still exist. The aim of this study was to analyse femoral residual growth and femoral deformities after in situ fixation of SCFE either with k-wires or screws. PATIENTS AND METHODS: We conducted a retrospective analysis of the radiographs of adolescents treated for SCFE in our department between 01/2003 and 02/2011. To evalua

Elastic stable intramedullary nailing (ESIN) of pediatric metacarpal fractures: experiences with 66 cases.

INTRODUCTION: Elastic stable intramedullary nailing (ESIN) is the first-choice surgical technique for stabilizing various pediatric diaphyseal and selected metaphyseal fractures of the long bones. This technique has increasingly been applied in fractures of the small bones. Here, we report experiences with ESIN in displaced fractures of the metacarpals in children. PATIENTS AND METHODS: Retrospective data analysis of metacarpal fractures in children stabilized by ESIN in three pediatric trauma centers between 2003 and 2009. Indication for intervention was total displacement or axial deviation

[The hand in hematologic diseases].

Radiography of the hand often reflects the picture of generalized diseases, affecting both the muscolo-skeletal system and the others. Some of the most common hematologic disorders may be detected in roentgenograms of the hand, especially in the anemias, but also in plasma cell dyscrasias and proliferative malignant diseases (i.e., lymphomas and leukemias). On the basis of their experience, the authors have reviewed and discussed the radiographic "pattern" of the hand in several hematologic conditions (i.e., anemias; thalassemias; sickle-cell disease; lymphomas; multiple myeloma; etc.), and th

Use of patient-specific instrumentation for hip resurfacing arthroplasty: Measurement of accuracy using pre and postoperative CT-scan.

BACKGROUND: Hip resurfacing arthroplasty (HRA) is a valuable option for the treatment of primary osteoarthritis in young patients. However, the learning curve for femoral component implantation remains long. Few data are dedicated to patient-specific instrumentation (PSI) in this indication, and merely applied to limited study groups using X-rays as tool to assess accuracy of the method. Therefore, we conducted a prospective study assessing the results of PSI for HRA using pre- and postoperative CT scans based aiming to: 1) assess the accuracy of the planned axis in the axial and coronal plane

Periprosthetic tibial fracture in robotic-assisted versus conventional medial UKA: in vitro testing and finite element analysis of baseplate footprint.

BACKGROUND: Periprosthetic tibial fracture after unicompartmental knee arthroplasty (UKA) is uncommon but clinically important. Although robotic-assisted UKA may improve the precision of bone preparation and component positioning, whether these technical differences influence fracture resistance or fracture morphology remains unclear. This in vitro study examined whether robotic-assisted medial UKA alters axial load-to-failure compared with conventional UKA, whether fracture morphology differs between the two techniques, and whether implant base footprint influences fracture behavior in comple

The role of patient and surgeon characteristics on the treatment decision for displaced midshaft clavicle fractures in athletes: a global survey.

BACKGROUND: Diaphyseal clavicle fractures are common in athletes. While surgery can reduce the risk of nonunion and expedite return to sport, it carries risks and may lead to hardware removal. How patient and surgeon characteristics influence treatment decisions in athletes with displaced midshaft clavicle fractures has not been clearly defined. METHODS: Participants from the Science of Variation Group were invited to review randomized radiographs of displaced midshaft clavicle fractures paired with different patient scenarios that included age, sports level, dominant arm, type of sport, timi

Design and Validation of a Novel Orthopedic Device for Enhanced Fracture Stabilization and Reduction Efficiency: A Randomized Controlled Trial With Implications for Surgical Outcomes.

OBJECTIVE: Traumatic long bone fractures require precise reduction and stable fixation to achieve optimal outcomes during open reduction and internal fixation (ORIF). Conventional bone-holding instruments are often associated with prolonged operative time, increased blood loss, reliance on surgical assistance, and higher complication rates. This study aimed to evaluate the clinical effectiveness of a newly designed orthopedic repositioning device in reducing operative time, intraoperative blood loss, postoperative pain, and complications compared with conventional ORIF techniques. METHODS: Th

Do modular tapered long stems differ in stability and function? A prospective comparison of two stems in complex femoral revision THA.

BACKGROUND: Revision total hip arthroplasty (rTHA) in the setting of substantial proximal femoral bone loss is technically challenging. Modular tapered fluted stems provide predictable diaphyseal fixation while allowing independent adjustment of version, offset, and limb length. Among these, two commonly used systems-modular tapered fluted stem Type A (Revitan™) and Type B (LIMA Modulus™)-have limited direct comparative evidence. This study aimed to prospectively compare radiographic stem subsidence (primary outcome), as well as functional outcomes, complications, survivorship, and secondary o

Comparison of Infrapatellar and Suprapatellar Intramedullary Nails with New Clinical Score for Fixation of Tibial Shaft Fractures.

Objectives: Tibial shaft fractures (TSFs) represent the most common diaphyseal fractures in adults. The gold-standard treatment is intramedullary nailing. Recently, the suprapatellar technique has been increasingly adopted due to its ability to reduce complications associated with the infrapatellar approach. Currently, no clinical score for leg fractures comprehensively assesses the entire lower limb. Therefore, we reviewed the main lower-limb scores available in the literature and developed a new clinical evaluation tool for tibial shaft fractures. The aim of our study was to report our exper

Extra-articular coronal protrusion of volar locking plate and screw cutout in the treatment of distal radius fracture in coronal plane: Classification, clinical outcomes and how to prevent.

BACKGROUND: Complications related to implant placement can occur during the surgical treatment of displaced distal radius fractures (DRF) with a volar locking plate(VLP). The literature has often focused on implant placement in the sagittal plane, whereas the coronal plane has been neglected. The purpose of this study was to evaluate the effect of VLP protrusions in the coronal plane in the surgical treatment of DRF. MATERIAL AND METHOD: Between 2015 and 2022, 302 patients who underwent DRF surgery with VLP between 2015 and 2022 were included in the study. Patients were divided into group 1(a

Radiological and clinical evaluation of stemless anatomical shoulder arthoplasty Affinis Short™ (Mathys) with a minimum follow-up of 5 years.

INTRODUCTION: Stemless shoulder implants appeared on the market in 2003. The interest of this type of anatomic shoulder prosthesis is a better adaptation of the implant to the humeral anatomic characteristics and humeral bone preservation. These implants enabled an easier restoration of the rotation centre of the humerus, eliminating the problem of diaphyseal humeral offset, but few publications at long term are related. Therefore we did a retrospective investigation aiming to analyse signs of humeral and glenoid loosening and to assess the clinical results obtained using anatomic stemless sho

Correlation of Histopathological Profiles With Clinical and Radiological Risk Factors in Long-Bone Nonunion: A Prospective Cohort Study.

Background Fracture nonunion is a significant complication of orthopedic trauma, affecting a notable proportion of long bone fractures and contributing to prolonged disability and increased healthcare costs. Although radiological classification into hypertrophic, atrophic, and oligotrophic types is traditionally used to estimate biological activity at the fracture site, imaging alone may not accurately predict healing potential. Histopathological evaluation offers direct insight into tissue viability, cellular activity, and infection, potentially enhancing prognostication and guiding treatment
