Diagnosis of Strongyloides stercoralis: Detection of parasite-derived DNA in urine.

Detecting infections of Strongyloides stercoralis is arduous and has low sensitivity. Clinically this is a major problem because chronic infections may disseminate in the host and lead to a life threatening condition. Epidemiologically, S. stercoralis is often missed in surveys as it is difficult to identify by standard stool examination procedures. We present, for the first time, evidence that the infection can be detected in filtered urine samples collected and processed in the field and subsequently assayed for the presence of parasite DNA. Urine specimens (∼40mL) were collected from 125 te

Strongyloides stercoralis, the hidden worm. Epidemiological and clinical characteristics of 70 cases diagnosed in the North Metropolitan Area of Barcelona, Spain, 2003-2012.

BACKGROUND: The nematode Strongyloides stercoralis has a very particular autoinfection life-cycle which leads to chronic infections remaining undetected for decades. However, hyperinfection can occur in patients receiving immunotherapy resulting in high mortality rates. The main objective of this study was to assess the results of a 10-year multicenter surveillance program performed in an area with dense immigration in Barcelona, Spain. METHODS: From January 2003 to December 2012, all individuals with Strongyloides stercoralis infection attending the four centers with diagnostic capability in

Strongyloides stercoralis hyperinfection syndrome after liver transplantation: case report and literature review.

Strongyloides stercoralis is an intestinal nematode that causes human infections and whose life cycle has special features, including autoinfection. Strongyloides infection may be asymptomatic for years, owing to a low parasite load. During immunosuppressive therapy, however, if cellular immunity is depressed, autoinfection can occur at a higher rate, resulting in hyperinfection syndrome. In this specific circumstance, it can become a fatal illness. We describe a case of hyperinfection syndrome in a liver transplant recipient and also review the literature.

Larva currens in a patient scheduled for renal transplant.

We present a case of larva currens in a patient scheduled for renal transplant. Larva currens is an eruption caused by Strongyloides stercoralis, characterized most often by a pathognomonic, migratory, rapidly extending, serpiginous, urticarial eruption. Infected patients who are immunocompromised are at risk for disseminated and often fatal infection. In disseminated disease, diffuse petechiae and purpura may be present, and periumbilical ecchymoses may resemble thumbprints. The dermatologist may be in a unique position to diagnose this condition and institute therapy. Although found endemica

Effect of temperature on the development of free-living stages of Strongyloides ratti.

In the genus Strongyloides, larval development external to the host is known to be markedly affected by a variety of environmental factors. This investigation focuses on the effect of temperature on Strongyloides ratti. Low temperature (15 degrees C) was shown to favor direct development, producing infective larvae, while high temperature (25 degrees C) favored indirect development, producing free-living females and males. Different courses of development were brought about by either a 16-h temperature stimulus at 15 degrees C or a 6-h temperature stimulus at 25 degrees C. Moreover, eggs were

Heritable transgenesis of Parastrongyloides trichosuri: a nematode parasite of mammals.

Germline transformation of a parasitic nematode of mammals has proven to be an elusive goal. We report here the heritable germline transformation of Parastrongyloides trichosuri, a nematode parasite whose natural hosts are Australian possums of the genus Trichosurus. This parasite can undergo multiple free-living life cycles and these replicative cycles can be maintained indefinitely in the laboratory. Transformation was achieved by microinjection of DNA into the ovary syncytium of either free-living or parasitic adult females. By selecting for the transgenic progeny of successive free-living

Strongyloides stercoralis infestation in a patient with severe ulcerative colitis.

Asymptomatic infestation with Strongyloides stercoralis, common in the tropics, may result in potentially fatal hyperinfection during treatment with immunosuppressive drugs used to treat patients with severe ulcerative colitis (UC). Hence, importance of early recognition and treatment of this nematode in patients with UC before starting immunosuppressive drugs can not be overemphasized. We report a 23-yrs old man with UC who presented with acute severe attack. Since his UC did not respond to intravenous hydrocortisone over 6 days, oral cyclosporine was started on 7th day after repeating stool

A dot-blot ELISA comparable to immunoblot for the specific diagnosis of human parastrongyliasis.

A dot-blot enzyme-linked immunosorbent assay (dot-blot ELISA) using an electroeluted 31-kDa glycoprotein from adult worms of Parastrongylus cantonensis as the specific antigen was evaluated for the immunological diagnosis of patients infected with P. cantonensis. The sensitivity and specificity for the detection of serum antibody to P. cantonensis in dot-blot ELISA were both 100%, as determined with serum samples of ten P. cantonensis-infected patients, 60 patients with other related parasitic infections, and 20 uninfected controls. The test was as sensitive and specific as the immunoblot test

Influence of rat strain on larval production by the parasitic nematode Strongyloides ratti.

The course of infection with Strongyloides ratti in a range of rat strains was assessed by monitoring the production of larvae. To our knowledge, this is the first such study of S. ratti using its natural host Rattus norvegicus. Host strain influenced the pattern of larval production. The results were qualitatively the same for 2 S. ratti lines of North American and Japanese origin.

An abattoir survey of gastrointestinal nematode infections in cattle in the central highlands of Kenya.

The gastrointestinal tracts of 672 crossbred cattle were obtained from various abattoirs in Kiambu District, Kenya from August 1992 to July 1993, and examined for the presence of gastrointestinal nematodes. Eight nematode species were found in 583 (86.8%) of the animals. The nematodes were, in order of prevalence: Haemonchus placei (67.0%), Cooperia pectinata (53.0%), Cooperia punctata (41.7%), Oesophagostomum radiatum (38.4%), Trichostronglyus axei (24.3%), Nematodirus helvetianus (19.6%), Trichuris globulosa (9.7%) and Strongyloides papillosus (3.6%). The intensity of the nematode infection

Sub-acute intestinal obstruction by Strongyloides stercolaris.

Strongyloides stercolaris infestation is rather rare in Iraq. Individuals with infection confined to the intestinal tract are often asymptomatic. Symptoms include abdominal pain, diarrhea, weight loss and other non-specific complaints. The diagnosis depends upon repeated examination of stool and duodenal aspirate. Two cases presenting as sub-acute intestinal obstruction and mimicking primary intestinal lymphoma (PIL) on presentation are presented. Differentiation between the two conditions regarding presenting features, barium studies and pathology are discussed.

Disseminated Strongyloides stercoralis in human immunodeficiency virus-infected patients. Treatment failure and a review of the literature.

We describe a North American human immunodeficiency virus (HIV)-positive patient with Strongyloides stercoralis infection of the gastrointestinal tract, who required repeated "standard" courses of thiabendazole. Pulmonary infection with numerous roundworms developed, as suspected by bronchoalveolar lavage, and while he was receiving therapy, dissemination occurred. On autopsy, S stercoralis was recovered in the gastrointestinal tract, liver, lung, and heart. After a literature review, we conclude that HIV-positive patients have a higher risk of dissemination and "standard" treatment failure. T
