Prospective randomized comparison of double-bundle versus single-bundle anterior cruciate ligament reconstruction.

PURPOSE: Biomechanical studies show increased anterior and rotational stability with double-bundle (DB) compared to single-bundle (SB) anterior cruciate ligament (ACL) reconstruction. The aim of this study was to evaluate the clinical results of four-tunnel DB ACL reconstruction. METHODS: Seventy patients undergoing arthroscopic hamstring ACL reconstruction were prospectively randomized to DB (n = 35) or SB (n = 35) groups. Each bundle fixation was by means of a femoral EndoButton CL and a tibial biodegradable interference screw. Demographic data were comparable between groups, and the averag

Chronic lateral ankle instability: assessment of subjective outcomes following delayed primary repair and a new secondary reconstruction.

Chronic lateral ankle instability is a condition commonly encountered by the podiatric physician. Chronic instability usually occurs after injuries to the lateral collateral ligamentous complex. The purpose of this article is to report subjective results of a retrospective study comparing delayed primary ligamentous repair and a new secondary ligamentous reconstruction. Our patient population includes 23 patients who responded to a detailed questionnaire. Three patients had bilateral ligamentous repair for a total of 26 ankles. Sixteen ankles underwent delayed primary ligamentous repair, while

Arthroscopic treatment of basal joint arthritis by partial trapeziectomy with ligament reconstruction: Short-term results from a prospective study of 20 patients.

Partial trapeziectomy for basal joint arthritis is an alternative to total trapeziectomy that preserves the height of the thumb column. Using arthroscopy reduces the incidence of periarticular lesions and the risks of complications. The purpose of this prospective single-center study was to evaluate the results of arthroscopic partial trapeziectomy combined with suspension and interposition ligament reconstruction using half of the abductor pollicis longus tendon. Twenty patients (18 women, 2 men) with a mean age of 55 years (43-65 years) were operated using this technique between November 201

Combined synovectomy and extensor carpi radialis longus transfer to realign and stabilise the rheumatoid wrist.

BACKGROUND: In patients with rheumatoid arthritis, the surgical treatment of wrist lesions relies on synovectomy combined with stabilisation and realignment of the carpal bones. The objective of this study was to evaluate the outcomes and define the indications of extensor carpi radialis longus (ECRL) transfer to the extensor carpi ulnaris (ECU) as described by Clayton and Ferlic. HYPOTHESIS: ECRL-to-ECU transfer combined with synovectomy can prevent the development and/or progression of rheumatoid deformities at the wrist. MATERIAL AND METHODS: A retrospective observational study was perfor

Reconstructive procedures for extremities with peripheral nerve defects.

Surgeons who undertake reconstruction of extremities with peripheral nerve loss should be experienced enough to select the appropriate procedures for the individual patient. To gain adequate strength for a specific function, a new muscle-tendon unit usually is added to the power-train rather than redistribute muscles that have related function. Precise sensibility required precise motion, which is very difficult to achieve following peripheral nerve loss. The anticipated result should be a balanced simplification of motor and sensory function.

Reconstruction of the Upper Extremity High-Voltage Electrical Injury: A Pediatric Burn Hospital's 13-Year Experience.

High-voltage electrical injury is a rare yet destructive class of burn injury that persists as a serious public health issue. High-voltage exposure is commonly associated with complex wounds to the upper extremities, which can be a significant challenge for burn and plastic surgeons to reconstruct. This intensive and multistage reconstructive process is especially difficult in the growing child. Maximizing upper extremity function is a top priority, as it can have a significant impact on a patient's quality of life. Therefore, the purpose of this retrospective review was to describe lessons le

Resilience, self-efficacy, social support, and quality of life in patients with skin defects of the lower extremity after flap transplantation.

BACKGROUND: Lower extremity trauma is one of the most common emergency presentations. It may cause skin and muscle tissue defects, and in severe cases, it may also be accompanied by bone, tendon, and neurovascular exposure. Pedicled flap transfer is the most important treatment for this type of lower extremity trauma. The purpose of the study is to investigate the relevant factors affecting resilience and provide a theoretical basis of improving resilience in patients after skin flap transplantation. METHODS: In this study, the convenience sampling method was used to conduct a questionnaire s

Surgical Offloading Techniques Should be Used More Often and Earlier in Treating Forefoot Diabetic Ulcers: An Evidence-Based Review.

Conservative treatment is the basis for diabetic foot ulcer (DFU) management, whereas surgical treatment is usually reserved for patients with failed, recurrent, or nonresponsive infected wounds. However, many reports demonstrated good to excellent results following surgery. Evidence synthesis on surgical offloading techniques and clear guidelines regarding the timing of surgery are lacking. The present study aimed to investigate the evidence behind surgical offloading techniques and propose a cutoff time for surgical indication following failed conservative treatment of neuropathic diabetic f

The Role of Extraocular Muscle Pulleys in Incomitant Non-Paralytic Strabismus.

The rectus extraocular muscles (EOMs) and inferior oblique muscle have paths through the orbit constrained by connective tissue pulleys. These pulleys shift position during contraction and relaxation of the EOMs, dynamically changing the biomechanics of force transfer from the tendon onto the globe. The paths of the EOMs are tightly conserved in normal patients and disorders in the location and/or stability of the pulleys can create patterns of incomitant strabismus that may mimic oblique muscle dysfunction and cranial nerve paresis. Developmental disorders of pulley location can occur in conj
