Sleep and bipolar disorder: Associations with delivery and neonatal outcomes.

INTRODUCTION: This study aims to examine the relationship between sleep duration and sleep quality during pregnancy and the delivery and neonatal outcomes in a prospective cohort of pregnant women with bipolar disorder (BD). METHODS: Data were from the Sleepreg-Bipolar Disorder (Sleepreg-BD) study and included 65 pregnant Dutch women. Quality and duration of sleep were measured using actigraphy and a sleepdiary, respectively. The Altman Self-Rating Mania Scale (ASRM) and the Quick Inventory of Depressive Symptomatology (QIDS) were used to measure the severity of manic and depressive symptoms,

Septic shock during pregnancy.

A multiinstitutional review of 10 pregnancies complicated by septic shock was undertaken to identify the clinical characteristics and hemodynamic alterations associated with this condition. Prolonged rupture of membranes with the subsequent development of chorioamnionitis or postpartum endometritis were risk factors that commonly preceded the diagnosis of septic shock. The majority of septic shock cases occurred during the puerperium. There were two maternal deaths in this selected series. Associated complications included pulmonary edema, adult respiratory distress syndrome, disseminated intr

Exploring the relationship between breastfeeding and psychological well-being after traumatic childbirth: a secondary analysis of the PERCEIVE study data.

BACKGROUND: The relation between maternal mental health and breastfeeding is complex, with breastfeeding often associated with improved well-being. OBJECTIVE: This study investigates associations between breastfeeding outcomes and psychological well-being in women with traumatic birth experience (TBE). METHODS: This exploratory secondary analysis utilized data from the PERCEIVE randomized controlled trial, which assessed the effectiveness of early eye movement desensitization and reprocessing (EMDR) therapy on posttraumatic stress disorder (PTSD) symptoms after TBE. Women with self-reported

The Role of Oxytocin in Early-Life-Stress-Related Neuropsychiatric Disorders.

Early-life stress during critical periods of brain development can have long-term effects on physical and mental health. Oxytocin is a critical social regulator and anti-inflammatory hormone that modulates stress-related functions and social behaviors and alleviates diseases. Oxytocin-related neural systems show high plasticity in early postpartum and adolescent periods. Early-life stress can influence the oxytocin system long term by altering the expression and signaling of oxytocin receptors. Deficits in social behavior, emotional control, and stress responses may result, thus increasing the

Selective Serotonin Reuptake Inhibitors (SSRIs) in Pregnancy: An Updated Review on Risks to Mother, Fetus, and Child.

PURPOSE OF REVIEW: To provide an updated summary and appraisal of work from 2019 to 2022 examining risks of selective serotonin reuptake inhibitor (SSRI) use in pregnancy. RECENT FINDINGS: Perinatal SSRI exposure does not increase risk of major malformations or gestational diabetes after accounting for underlying maternal illness. SSRIs are associated with small increase in risk of pre-eclampsia, postpartum hemorrhage, preterm delivery, persistent pulmonary hypertension of the newborn, and neonatal intensive care unit admissions, though absolute risk of these outcomes is low. While data sugge

Comparison of the ACE-Q and CTES: A study of childhood trauma and perinatal depression.

Postpartum depression (PPD) is a major public health issue, significantly affecting the health and well-being of women and children. Research indicates a strong link between adverse childhood experiences (ACEs) and increased PPD rates. While the Adverse Childhood Experiences Questionnaire (ACE-Q) is widely used for ACE screening, the Childhood Traumatic Events Scale (CTES) may better capture trauma items not covered by the ACE-Q, along with the timing and severity of ACEs. However, the psychometric properties of the CTES in perinatal populations have not been previously assessed. We present a

Lack of partner impacts newborn health through maternal depression: A pilot study of low-income immigrant Latina women.

INTRODUCTION: Latina women have a high burden of depression and other mental health issues, particularly in the perinatal period. Suboptimal maternal mental health can have adverse developmental and physiological impacts on child growth. The present study examines the impact of unplanned pregnancy and pregnancy relationship status on prenatal maternal depression in a sample of low-income Latina women. We hypothesized that the association between these prenatal stressors and newborn health would be mediated through prenatal depression. METHOD: The present study included a sample 201 Latina mot

Occlusive hydrocephalus caused by a fourth ventricle arachnoid cyst.

The case history of a woman with occlusive hydrocephalus caused by a fourth ventricle cyst is presented. She had slowly progressive complaints and symptoms--concentration and memory disturbances, low-tempered mood, then slight dizziness, loss of appetite and progressive headache--transitorily misinterpreted for signs of depression. She had been treated by psychotherapy and antidepressants for months. Since she did not improve she was referred to a psychiatric hospital. The rapidly progressing neurological syndrome with worsening headache, gait disturbance and vomiting was finally identified an

Risk of postnatal depression, miscarriage, and preterm birth in bulimia nervosa: retrospective controlled study.

OBJECTIVE: Bulimia nervosa is common and treatable. An association between bulimia and obstetric complications has been suggested, but sample size and absence of control have limited previous studies. Our aim was to determine if active bulimia nervosa affects obstetric outcome. METHODS: This was a retrospective case-control comparison of obstetric complications in primigravidae previously treated for bulimia in a specialist eating disorder service. A cohort of 122 women with active bulimia during pregnancy was contrasted against 82 with quiescent bulimia, using structured interviews comprisin

Olanzapine excretion in human breast milk: estimation of infant exposure.

Newer antipsychotic drugs offer significant clinical advantages for the treatment of psychosis. In particular for the treatment of postpartum disorders newer agents may be suited due to their favourable side-effect profiles. Of concern is the passage of the drugs into breast milk and what potential risks this poses for an infant who is breastfed. The excretion of olanzapine into the breast milk of five lactating women with postpartum psychosis was examined in this study. Nine pairs of plasma and breast-milk samples were collected and the concentration of olanzapine determined by high-performan

Impact of pregnancy on bulimia nervosa.

BACKGROUND: Bulimia nervosa affects women at a peak age of reproductive functioning, but few studies have examined the impact of pregnancy on bulimia. AIM: To examine the impact of pregnancy on symptoms of bulimia nervosa and associated psychopathology. METHOD: Women actively suffering from bulimia nervosa during pregnancy (n = 94) were interviewed using the eating disorder examination (12th edn) and structured clinical interview for DSM-III-R, with additional structured questions. Behaviours were recorded at conception, each trimester and postnatally. Relative risks were calculated for prog

Post-Partum Depression, Personality, and Cognitive-Emotional Factors: A Longitudinal Study on Spanish Pregnant Women.

In this study, our purpose was to examine whether personality and cognitive factors could be related to post-partum depression (PPD), mediated by anxiety, in Spanish women. Women were evaluated for personality and cognitive factors after the first trimester, for anxiety in the third trimester, and for PPD 4 months after childbirth. A structural equation model revealed that personality and cognitive factors were associated with anxiety and PPD as predictors. Neuroticism and extroversion proved to be the most relevant factors. Conscientiousness was associated with pregnancy anxiety. Pregnancy an

Factors associated with post-natal anxiety among primiparous mothers in Nepal.

BACKGROUND: In Nepal, new mothers are fatigued by childcare responsibilities because of lack of knowledge and preparation for their new role. Knowledge deficit and adherence to traditional health practices place infants at risk. Some cultural practices for the care of newborns may cause tetanus, hypothermia or other life-threatening illnesses. Maternal education, previous experience of newborn care, economic status, relationship problems with husband or mothers-in-law and dissatisfaction with social relations are the variables of interest for this study. AIMS: To examine the association betwe

Personality factors and vulnerability to post-traumatic stress responses after childbirth.

OBJECTIVES: To investigate whether levels of perfectionism, organization, and intolerance of uncertainty predispose women to more negative birth experiences and post-partum post-traumatic stress symptoms (PTSS). Birth experience was also examined as a potential moderator of the relationship between levels of the personality traits and post-natal PTSS. DESIGN: Prospective survey. METHOD: First-time expectant mothers (N = 10,000) were contacted via Emma's Diary during the perinatal period. At 32-42 weeks' gestation, participants completed measures examining the three personality traits and pre

The relationship between under-five child death and maternal mental distress in Gamo Gofa Zone, Southern Ethiopia: a community based comparative cross-sectional study.

BACKGROUND: Knowledge of the association between child death and maternal mental distress may help to understand the indirect impact of reduction of under-five mortality on maternal mental wellbeing. This will further have a positive impact on the development of the nation. Depression is associated substantially with reduced quality of life and functional capacity of women. Although studies in the country assessed the magnitude of Common Mental Disorders (CMD) among postpartum mothers, those assessing the association between child death and maternal mental distress are lacking. Therefore, this

Antenatal maternal depressive mood and parental-fetal attachment at the end of pregnancy.

The present study investigates if mothers and fathers have similar ways of thinking and feeling about their babies during late pregnancy and how aspects of parental-fetal attachment are related to maternal depressive mood. Two hundred and ninety-eight Swedish-speaking women at 30-32 weeks of gestation and partners (n = 274) participated in the study. Socio-demographic background data were collected. Prenatal attachment was assessed with the maternal/paternal-fetal attachment scale (MFA/PFA), and depressive symptoms were assessed by the Edinburgh postnatal depression scale (EPDS). MFA and PFA s

Perinatal obsessive-compulsive symptoms and maternal-infant bonding: A longitudinal examination of depression and relationship quality.

BACKGROUND: Perinatal obsessive–compulsive (OC) symptoms are common and distressing, yet their impact on early parenting outcomes such as maternal-infant bonding remains poorly understood. Although prior research links maternal depression and anxiety to bonding difficulties, less is known about how perinatal OC symptoms relate to maternal-infant bonding over time, or the extent to which maternal-partner relationship quality is associated with these outcomes, and whether depressive symptoms explain observed associations. METHODS: Data were drawn from a longitudinal study of 256 pregnant women r

Modified electroconvulsive therapy for perinatal depression: scoping review.

BACKGROUND: Modified electroconvulsive therapy (mECT), the administration of ECT under general anesthesia with muscular relaxation, is indicated for perinatal depression complicated by high severity, psychosis, catatonia, or resistance to conventional therapeutics; however, knowledge gaps remain regarding its effectiveness and safety in depressed patients and its fetal/neonatal risk profile. MATERIALS AND METHODS: We conducted a scoping review of the literature describing the effectiveness and safety (maternal, fetal, and neonatal) of mECT for perinatal depression. Effects of skin-to-skin contact on mental health outcomes in the parents of full-term newborns: A systematic review and meta-analysis of randomized clinical trials.

BACKGROUND: The postpartum period can be emotionally challenging for parents, with documented levels of anxiety, stress, and depression. While the benefits of skin-to-skin contact (SSC) are well-known, its impact on the mental health of parents of full-term newborns has been less studied. OBJECTIVE: We aimed to assess the effects of SSC in mental health outcomes in the parents of full-term newborns. METHODS: In this systematic review, a comprehensive search was conducted to identify randomized clinical trials (RCTs) comparing SSC versus no SSC and assessing mental health outcomes. Meta-analy
