Specific fixation beyond the watershed line for distal radius fractures: A comparative study.

PURPOSE: Various fixation methods are available for intra-articular distal radius fractures, depending on column involvement. Some rely solely on ulnar hook plates, while others use specific "extra distal" plates that address both columns. To date, no comparative studies have evaluated these techniques. This study compared the fixation by ulnar hook plate combined with one or two percutaneous screws (Group 1) in the radial styloid with extra distal plates fixing both columns (Group 2). Functional outcomes were hypothesized to be similar, whereas complication rates were expected to differ. MET

Clinical Outcomes Following Posterior Malleolus Fixation in Trimalleolar Ankle Fractures: A Prospective Study.

INTRODUCTION: Posterior malleolus (PM) fractures are frequently caused by pronation or supination injuries with an external rotation component to the ankle. Historically, fixation of the PM was not considered essential if it involved less than 25% of the tibial articular surface. However, studies have now shown that trimalleolar fractures fare worse than bimalleolar fractures. This study primarily aims to evaluate the clinical outcomes of trimalleolar fractures, focusing on posterior malleolus fixation regardless of the fragment size. MATERIALS AND METHODS: A prospective observational study w

Preclinical study analysis of massive magnesium alloy graft for calcaneal fractures.

The highly comminuted calcaneal fractures represent a challenge for surgeons and require bone grafts for a good clinical outcome. Postoperative results are generally associated with increased morbidity and long periods of inactivity. The biomedical community promotes the use of artificial materials for grafts in order to achieve improved results. In an era when cosmetic concerns as well as the satisfaction of patients are mandatory and the use of autologous bone grafts is not without complications, an artificial replacement appears to be a favorable option. Synthetic bone grafts are known to f

Do demographic and perioperative parameters really affect the final outcomes of pediatric femur shaft fractures managed by elastic nails? A prospective study.

OBJECTIVE: Pediatric femoral shaft fractures are more commonly treated with intramedullary titanium elastic nail system (TENS). Adhering to the principles, most studies had supported excellent results with this instrumentation and attributed the variation in age, weight, immobilization protocols, technical factors like fracture pattern, reduction and complications as reasons to poor outcomes in their individual studies. Hence, we wanted to identify the potential demographic and perioperative parameters that could affect the final outcomes in this cohort. METHODS: A prospective (level III) stu

Colon ascendens stent peritonitis (CASP)--a standardized model for polymicrobial abdominal sepsis.

Road traffic accidents (RTA) are the major cause of maxillofacial injuries (MFIs) in developing countries (Akama et al. 2007). Road traffic accidents were reported to be the 9th most common cause of death and morbidity in the world and are expected to rise to 3rd position by 2020 (Peden et al. 2002). Maxillofacial injuries remain a serious clinical problem because of the involvement of complex anatomic region. Facial fractures occur most commonly in males in the third decade of life (Motamedi et al. 2014). The goal of treatment in facial fractures is to achieve anatomic reduction and restore f

[Proximal ulna fractures].

Proximal ulna fractures are common injuries in the upper extremity. The severity of injury ranges from isolated olecranon fractures to complex elbow fracture dislocations. Relevant anatomic features of the proximal ulna for fracture management are presented in the first part of this article. Furthermore, standard and recent techniques of surgical treatment are described in detail for olecranon and Monteggia fractures as well as for fractures of the coronoid process. Functional outcome and complications of fracture treatment are discussed.

Relevancy of radiographic features in elbow fractures.

PURPOSE: To correlate radiographic and clinical findings of elbow fractures. MATERIAL AND METHODS: A retrospective study was made of 110 consecutive adult elbow fractures of various types involving the humerus, the ulna, and the radius. RESULTS: Twenty-seven fractures with radiographically conspicuous distortions healed. Of these only 15 had clinical sequelae. Five had clinical sequelae without showing conspicuous radiographic distortions after healing. CONCLUSION: For radiographic evaluation and classification purposes, it would seem to be important to note the status of the capitellum, th

What to Expect? Use of Supplemental Fixation With a Concomitant Dorsal Spanning Plate for Complex Intraarticular Distal Radius Fractures.

BACKGROUND: The dorsal spanning plate (DSP) is a versatile implant suitable for bridging severely comminuted intraarticular distal radius fractures [AO (Arbeitsgemeinschaft für Osteosynthesefragen) 23-C.1-C.3]. It may be used alone or with supplemental fixation such as a volar locking plate (VLP) or fragment-specific fixation (FSF). Outcomes following DSP fixation with additional implants have not been specifically evaluated. METHODS: We retrospectively reviewed consecutive patients who underwent internal fixation of a distal radius fracture by a single surgeon from 2017 to 2021. Patients wer

Orbital deformity after craniofacial fracture repair: avoidance and treatment.

BACKGROUND AND OBJECTIVES: To achieve the optimal preoperative appearance following craniofacial fracture repair, the surgeon must be facile in the most sophisticated reconstructive techniques and able to determine their application. The purpose of this article is to describe the common deformities following such repairs, outline a strategy to avoid them, and review the surgical techniques to correct them. METHODS AND MATERIALS: The deformities are categorized by the anatomic zones of the orbit, i.e., zygomatic, frontal, and nasoethmoidal, affected by low-, middle-, and high-energy impact. Th

Visualizing access in pilon fractures: A comparative study of eight approaches.

BACKGROUND: Pilon fractures, constituting 3-10 % of all tibia fractures, present significant challenges due to their complex nature and frequent involvement of soft tissue damage. Effective management necessitates careful preoperative planning and strategic surgical approach selection. This study aims to provide a comprehensive cadaveric analysis comparing eight distinct surgical approaches to tibial pilon fractures, focusing on the consistently exposed segment and the extent of the exposed bone area for each approach. METHODS: Sixteen foot and ankle specimens, obtained through our university

Comparison of tibial fracture plate length, placement, and fibular integrity effects on plate integrity through finite element analysis.

Minimally invasive plate osteosynthesis is the most commonly used minimally invasive surgery technique for tibial fractures, possibly involving single or dual plate methods. Herein, we performed a finite element analysis to investigate plate strength according to the plate type, length, and presence of a fibula by constructing a three-dimensional tibia model. A thickness of 20 mm was cut 50 mm distal from the lateral plateau, and the ligaments were created. Plates were modeled with lengths of 150, 200, and 250 mm and mounted to the tibia. Screws were arranged to avoid overlapping in the dual p

Comparative study on anterior pelvic plating and pubic ramus screw fixation for straddle fracture: a matched-pair outcome analysis.

PURPOSE: Straddle fractures involving both the superior and inferior rami often require surgical fixation due to instability. This study compared the clinical and radiological outcomes of pubic ramus screw fixation (PRSF) and anterior pelvic plating (APP) for the treatment of these fractures to identify the superior method. METHODS: A retrospective analysis was conducted on 70 patients (37 males, 33 females; average age 47.6 years) treated surgically for straddle fractures at two Level 1 trauma centres between May 2017 and August 2022. The patients were divided into two groups, where 26 under

Rupture of the Tibialis Posterior Tendon With Associated Bimalleolar Ankle Fracture.

The acute traumatic rupture of the tibialis posterior tendon in association with closed ankle fractures is rare and often under-recognised. If recognised early, outcomes can be excellent. There are 28 known cases in the literature, and we report two further cases associated with bimalleolar ankle fracture dislocation. A 49-year-old presented with valgus deformity at the ankle joint and global tenderness following a work injury as a mechanic. A plain radiograph showed a displaced oblique comminuted fracture of the lateral malleolus with valgus angulation at a syndesmosis, with significant talar

Pseudoaneurysm of the Posterior Tibial Artery following Removal of Tibial Plate- A Rare Case.

INTRODUCTION: Fractures of the distal humerus are a rare entity accounting for approximately 2% of the humeral fractures. Non union of the distal humerus is further rare and poses a major challenge. These fractures usually respond to open reduction and bone grafting but some may fail multiple surgical attempts at union and hence are labelled as "resistant" non union. We report a case of resistant non union of the distal humerus which was managed by total elbow arthroplasty. CASE PRESENTATION: A 49-year-old male presented to the out patient department with history of a compound comminuted frac

Long-term investigation of nonsurgical treatment for thoracolumbar and lumbar burst fractures: an outcome analysis in sight of spinopelvic balance.

The nonsurgical treatment of thoracolumbar (TLB) and lumbar burst (LB) fractures remains to be of interest, though it is not costly and avoids surgical risks. However, a subset of distinct burst fracture patterns tend to go with a suboptimal radiographic and clinical long-term outcome. Detailed fracture pattern and treatment-related results in terms of validated outcome measures are still lacking. In addition, there are controversial data on the impact of local posttraumatic kyphosis that is associated, in particular, with nonsurgical treatment. The assessment of global spinal balance followin

A novel stacked double-plate fixation with quadrilateral surface buttress screws for osteoporotic acetabular fractures in elderly patients: a prospective pilot case series study evaluating clinical and radiological outcomes.

BACKGROUND: Acetabular fractures in the elderly present unique challenges due to osteoporotic bone, comminution, and frequent involvement of the quadrilateral surface (QLS) and anterior column. Conventional fixation often fails in this population, leading to hardware complications and secondary displacement. This prospective pilot study evaluates the clinical and radiological outcomes of a novel stacked double-plate technique combined with QLS buttress screws in geriatric patients. METHODS: Twelve consecutive patients aged ≥ 70 years with displaced acetabular fractures involving the anterior
