Renal embolization as an alternative to surgical nephrectomy in children.

PURPOSE: Complete renal embolization may be an alternative to surgical nephrectomy. The indications for renal embolization do not differ from those for surgical nephrectomy, but the less invasive nature of the technique is a major advantage. Few case reports are available in the pediatric age group. Our experience showed that complete renal embolization was feasible in pediatric patients with results comparable to those obtained in adults. MATERIALS AND METHODS: Twelve pediatric patients underwent 14 renal embolizations. The indications for embolization were as follows: (1) severe hypertensio

Open kyphoplasty for management of metastatic and severe osteoporotic spinal fracture. Technical note.

OBJECT: Elderly patients in poor general health frequently suffer vertebral body (VB) fractures due to osteoporosis or vertebral metastatic lesions. Kyphoplasty and vertebroplasty have become the standard treatment for these types of fractures. In certain conditions that cause local kyphosis, such as spinal cord compression due to a metastatic epidural tumor or the shortening of the spinal canal secondary to vertebral compression, the surgical treatment should provide decompression and stabilization during a short intervention. In this study the authors evaluated a surgical technique that freq

The management and welfare of some common ovine obstetrical problems in the United Kingdom.

This review approaches sheep welfare during the periparturient period from a veterinary practitioner viewpoint. At the most fundamental husbandry level, the general hygiene standards adopted on many sheep farms in the United Kingdom (UK) during the lambing period are unacceptable and require urgent improvement. Concerns also arise from obstetrical conditions attended by farmers which cause pain that can be identified using clinical criteria such as altered demeanour and behaviour of the ewe, and the presence of inflammation of the reproductive tract. Reduced production is evidenced by poor mil

Maximum recommended doses of local anesthetics: a multifactorial concept.

The current recommendations regarding maximum doses of local anesthetics presented in textbooks, or by the responsible pharmaceutical companies, are not evidence based (ie, determined by randomized and controlled studies). Rather, decisions on recommending certain maximum local anesthetic doses have been made in part by extrapolations from animal experiments, clinical experiences from the use of various doses and measurement of blood concentrations, case reports of local anesthetic toxicity, and pharmacokinetic results. The common occurrence of central nervous system toxicity symptoms when lar

[A lateral fluid percussion model for the experimental severe brain injury and a morphological study in the rats].

Few morphological studies have been reported on fluid-percussion experimental models using mechanically induced severe brain injury have been reported. This study was initiated to evaluate microscopic and immunohistochemical findings in severe brain injury models using rats. The experimental rats and the methods used were the same as described for a fluid-percussion model. Fluid-percussion models of traumatic brain injury were produce by rapidly injecting fluid volume into the epidural space of the temporal lobe. We used 5 rats which sustained various degrees of injury by high (7.0 atm), mediu

Lidocaine constricts or dilates rat arterioles in a dose-dependent manner.

The microvascular effects of varying concentrations of lidocaine were evaluated with the use of videomicroscopy in an in vivo rat cremaster muscle preparation. Animals were anesthetized with chloralose and urethane and breathed room air spontaneously. Mean arterial pressure and heart rate were measured via a carotid artery cannula. The cremaster muscle was suffused with a balanced electrolyte solution and pH, temperature, PO2, PCO2, and osmolarity were controlled. Internal diameters of fourth-order arterioles in the cremaster muscle were measured with an electronic vernier system. In one group

Infections Following Interventional Spine Procedures: A Systematic Review.

BACKGROUND: Interventional spine procedures, such as discography, epidural steroid injections (ESIs), facet joint procedures, and intradiscal therapies, are commonly used to treat pain and improve function in patients with spine conditions. Although infections are known to occur following these procedures, there is a lack of comprehensive studies on this topic in recent years. OBJECTIVES: To assess and characterize infections following interventional spine procedures. STUDY DESIGN: Systematic review. METHODS: Studies that were published from January 2010 to January 2020 and provided informa

Dysethesia due to irritation of the dorsal root ganglion following lumbar transforaminal endoscopy: Analysis of frequency and contributing factors.

BACKGROUND: New onset of acute dysethetic leg pain due to irritation of the dorsal root ganglion (DRG) following uneventful recovery from an expertly executed lumbar transforaminal endoscopic decompression is a common problem. Its incidence and relation to any risk factors that could be mitigated preoperatively are not well understood. METHODS: We performed a multicenter frequency analysis of DRG irritation dysesthesia in 451 patients who underwent lumbar transforaminal endoscopic decompression for herniated disc and foraminal stenosis. The 451 patients consisted of 250 men and 201 women with

From Protrusion to Extrusion: Reassessing the Selective Role of Intradiscal Interventions.

CONTEXT: Extruded lumbar disc herniations are traditionally managed surgically, whereas contained protrusions are more commonly considered suitable for percutaneous intradiscal procedures. However, extrusion itself may not be a universal contraindication to all needle-based intradiscal interventions, particularly when the fragment is non-sequestered and remains continuous with the parent disc. This narrative review evaluates the evidence, safety considerations, and clinical positioning of intradiscal procedures in extruded, non-sequestered lumbar disc herniations. EVIDENCE ACQUISITION: A stru

Prevalence of interventional spine procedures performed by pain management physicians: blurred boundaries.

OBJECTIVE: Interventional pain management physicians (IPMPs) and spine surgeons are allies in treating spinal disorders. In addition to medical management, IPMPs can perform procedures such as epidural injections, rhizotomies, or kyphoplasties to bring relief to patients. However, some IPMPs perform procedures traditionally performed by spine surgeons, such as minimally invasive spine surgery, neural decompression, and spinal fusion. The line between spine procedures performed by spine surgeons and interventional pain management procedures performed by IPMPs has blurred. This study aimed to de

The Impact of Lumbar Partial Microdiscectomy for Vertebral Ring Apophyseal Fractures in Adolescents on PROMIS Pain, Physical Function, and Mental Health Domains.

BACKGROUND: Lumbar partial microdiscectomy (LPM) in adolescents is an infrequently performed procedure, reserved for pain and neurological symptoms unresponsive to nonsurgical management. Most studies have focused on the interventional impact on pain outcomes creating a paucity of data on physical function and mental health outcomes. The study hypothesis is LPMs in adolescents will provide improvements in measured PROMIS domains (mental health, physical function, and pain) at 2 years postoperatively. METHODS: This study is a retrospective analysis of patients under 21 years of age who underwe

Understanding quality of life and treatment history of patients with Bertolotti syndrome compared with lumbosacral radiculopathy.

OBJECTIVE: Bertolotti syndrome is a rare spinal condition that causes low-back pain due to a lumbosacral transitional vertebra (LSTV), which is a pseudoarticulation between the fifth lumbar transverse process and the sacral ala. Bertolotti syndrome patients are rarely studied, particularly with regard to their quality of life. This study aimed to examine the quality of life and prior treatments in patients with Bertolotti syndrome at first presentation to the authors' center in comparison with those with lumbosacral radiculopathy. METHODS: This study was a retrospective cohort analysis of pat

CT-guided cyanoacrylate injection for spinal cerebrospinal fluid leaks in spontaneous intracranial hypotension: outcomes from an expanded cohort.

BACKGROUND: Definitive minimally invasive treatment of spontaneous intracranial hypotension (SIH) is challenging due to heterogeneous mechanisms and locations of spinal cerebrospinal fluid (CSF) leaks. We evaluated CT-guided percutaneous cyanoacrylate injection for spinal CSF leaks. METHODS: We retrospectively reviewed consecutive SIH patients with CT myelography-confirmed leaks treated at our institution (2016-2025). Under local anesthesia, leak-type-tailored approaches were used: transforaminal or interlaminar for type 1 (dural tears) and posterior foraminal for types 2 (lateral leaks) and

Fentanyl Versus Dexmedetomidine for the Prevention of Emergence Agitation in Children After Sevoflurane Anaesthesia: A Comparative Clinical Study.

Introduction Sevoflurane is widely used in pediatric anesthesia due to its rapid onset and offset of action, smooth induction, and less hepatotoxicity. However, it is associated with emergence agitation, which can be frightening and harm the patient or the caregiver. While a definite preventive measure of emergence agitation is in search, the use of some drugs is associated with a lesser incidence of emergence related to sevoflurane.  Aims and objective This study aimed to compare the efficacy of fentanyl with dexmedetomidine in preventing emergence agitation in children undergoing surgery wit

Central opioid receptors mediate morphine-induced itch and chronic itch via disinhibition.

Opioids such as morphine are mainstay treatments for clinical pain conditions. Itch is a common side effect of opioids, particularly as a result of epidural or intrathecal administration. Recent progress has advanced our understanding of itch circuits in the spinal cord. However, the mechanisms underlying opioid-induced itch are not fully understood, although an interaction between µ-opioid receptor (MOR) and gastrin-releasing peptide receptor (GRPR) in spinal GRPR-expressing neurons has been implicated. In this study we investigated the cellular mechanisms of intrathecal opioid-induced itch b

Transversus Abdominis Plane Block Improves Perioperative Outcome After Esophagectomy Versus Epidural.

BACKGROUND: Pain control is challenging during esophagectomy. An epidural is commonly used, but the sympathetic blockade can have unintended consequences such as hypotension or delayed return of bowel function. A transversus abdominis plane (TAP) block has the potential to control upper abdominal pain without these adverse consequences. We aimed to compare bilateral TAP blocks with patient-controlled analgesia (PCA) for immediate management after esophagectomy with a cohort using a thoracic (T5 to T8) epidural. METHODS: This was a retrospective review of patients undergoing esophagectomy betw
