HYPOTHESIS: The objective of this study was to present the authors' experience in the management of labyrinthine fistula caused by cholesteatoma.
METHODS: The clinical charts of 92 patients who underwent surgical procedures for cholesteatoma complicated by labyrinthine fistula between 1979 and 1975 were reviewed retrospectively. In this period, 1,205 patients were operated on for cholesteatoma. In each patient, the site and size of the fistula were evaluated during surgery and the hearing thresholds were compared before and after surgery.
RESULTS: The fistula involved the lateral semicircula

For the evaluation of functional inner ear lesions caused by an autoimmune-induced labyrinthitis, transiently evoked otoacoustic emissions (TEOAEs) were detected in guinea pigs before and after transfer of sensitized lymphocytes from animals suffering from a labyrinthitis induced by the foreign protein keyhole limpet hemocyanin (KLH). Initially TEOAEs were detectable from 47 of 62 ears (76%) in healthy guinea pigs. These animals then were used as recipients for sensitized lymphocytes from donors exclusively. Three months after cell transfer the incidence of TEOAEs was reduced to 48% in animals

We have treated 20 patients suffering from unilateral Morbus Meniere (2 x 12 mg gentamicin for 7 days). These patients were controlled for 24 months. In 95% we regarded a complete or substantial control of definitive spells ("American Committee on Hearing and Equilibrium guidelines for reporting treatment results in Meniere's disease", Otolaryngol. Head and Neck Surg. 10/19-85). Only 1 patient with pretherapeutic equal caloric response showed a post-therapeutic destroyed vestibular apparatus on the treated ear. In the other 7 patients (35%), who had a loss of vestibular function after therapy,

Problems in the examination of the inner ear using conventional CT are described. Intraoperative findings often differ from CT findings. A new development HR-CT is introduced. Findings of middle and inner ear malformations and otosclerosis are demonstrated. It is shown that HR-CT imaging using a new 1024 matrix, enables better spatial resolution. This new CT technique improves inner ear examination, especially in the preparation for cochlear implantation.

A retrospective series is presented of 32 cases of spontaneous labyrinthine window rupture proven at tympanotomy. There was no prior history of stapedectomy or other middle ear surgery. Seventy-eight per cent of cases involved the round window, in contrast to previous series on the subject. The pattern of hearing loss and vestibular symptoms varied widely and followed different antecedent factors, the commonest of which was head injury (46.9%). Including cases misdiagnosed initially, 92.9% of patients with vestibular symptoms experienced improvement following surgery, but only 20.7% of patient

Side effects of drugs on the vestibular system may occur when a drug is injected too fast during chronic application of drugs after discontinuation of drugs Dizziness due to ototoxic drugs is rare because of the symmetric influence on the vestibular system. The oculomotor system most often is found being reversible or irreversible damaged by chronic drug effects especially from barbiturates, psychopharmacologic drugs, anticonvulsives and some antiarrhythmics, whereas the vestibular organs and their pathways usually remain unaffected except from ototoxic drugs. An isolated oculomotor disfunctio

To find the incidence of complications of modified radical mastoidectomy and to evaluate different parameters that play a role in their causation, we conducted a retrospective study spanning a period of 12 years. Included were 210 patients who underwent primary modified radical mastoidectomies performed by one senior surgeon; of these patients, 163 fulfilled the inclusion criteria. The charts were evaluated for patients' age and sex, laterality, intraoperative pathology, and complications. The complications were grouped into nine categories for analysis of their etiology. The results in this s

OBJECTIVE: To present the surgical outcomes of complete removal of the matrix of labyrinthine fistulas in a large series of middle ear cholesteatomas.
PATIENTS AND METHODS: This is a retrospective study. We analyzed 38 of 778 patients who were operated on for cholesteatoma and were proved to have labyrinthine fistula from 1991 to 2007. For this study, a more aggressive strategy was adopted that compromised immediate total removal of the matrix, regardless of size. To evaluate the safety and efficacy of the procedure, pre and postoperative pure-tone audiometry were compared and the recurrence

Several cases illustrating potential pitfalls in neurotologic diagnosis from the viewpoint of the otologist are presented. The role of auditory brain stem response testing is specifically emphasized in cases with discordant audiologic and radiographic findings. Included are one case of a "false negative" ABR in a patient with an intracanalicular acoustic neuroma, a case of a "false positive" CT scan in a patient with Meniere's disease, and a case of a patient with normal hearing in whom an acoustic neuroma was discovered serendipitously.

Meniere's disease (MD) is a disorder of the inner ear characterized by episodes of spontaneous vertigo, fluctuating hearing loss, and tinnitus. Recent studies have demonstrated that IgE may play a role in the pathogenesis of MD. Patients with MD (n = 103), acoustic neuroma (n = 5), and healthy subjects (n = 72) were recruited into the study. Serum from the participants was analyzed for IgE and type 2-related cytokines. IgE and CD23 expression levels in vestibular end organs of patients, C57BL/6 mice, or mouse HEI-OC1 cells were analyzed. Finally, the role of CD23 in IgE transcytosis was assess

Superior semicircular canal syndrome (SSCS) includes vestibular and audiological symptoms which result from the introduction of a third mobile window into the osseous cochlea. Surgical repair is considered in cases of incapacitating symptoms. The present paper aims at comparing the different surgical approaches and modes of dehiscence repair, regarding their respective efficacy and potential pitfalls. A systematic literature review and meta-analysis of pooled data were performed. Study selection included prospective- and retrospective-controlled studies, prospective- and retrospective-cohort s

The aim of this study was to evaluate the clinical symptoms, the otoneurological examinations, the treatment and the clinical course of three patients suffering from Cogan's syndrome, a rare disease based on the clinical association of a non-syphilitic interstitial keratitis with a cochleo-vestibular deficit. This case series involved three patients with follow up. The clinical course of the three patients (aged 30, 48 and 49 years) with Cogan's syndrome during a follow-up period of 2 to 6 years is reported. All patients underwent complete otoneurological, ophthalmologic and rheumatologic exam

In 75 patients with glomus tumors in the head and neck region, 57 tumors arose from the jugular bulb region, 11 from the middle ear, and seven from the vagus nerve. Thirty-seven percent (28 patients) had cranial nerve paralysis, and 14.6% (11) had intracranial tumor extension. The jugular foramen syndrome was associated with a 50% (two of four patients) incidence, and hypoglossal nerve paralysis with a 75% (three of four) incidence of posterior fossa tumor invasion. Horner syndrome and labyrinthine destruction had a 50% (two of four) incidence of a middle cranial fossa tumor invasion. The inci
