Outcome analysis of surgery for disorders of the rotator cuff: a comparison of subjective and objective scoring tools.

A consecutive series of 372 patients who underwent surgery for disorders of the rotator cuff involving arthroscopic subacromial decompression and open or arthroscopic repairs of the cuff were prospectively investigated as to the comparability of subjective and objective assessment scores of shoulder function. Assessments were made before operation and at 3, 6, 12, 18 and 24 months after surgery using the Disabilities of the Arm, Shoulder, and Hand score, the Oxford shoulder score and the Constant-Murley score, which was used as a reference. All scores were standardised to a scale of 0 to 100 f

Reverse shoulder arthroplasty combined with a modified latissimus dorsi and teres major tendon transfer for shoulder pseudoparalysis associated with dropping arm.

Although a reverse shoulder arthroplasty (RSA) can restore active elevation in the cuff deficient shoulder, it cannot restore active external rotation when both the infraspinatus and teres minor muscles are absent or atrophied. We hypothesized that a latissimus dorsi and teres major (LD/TM) transfer with a concomitant RSA would restore shoulder function and activities of daily living (ADLs). We prospectively followed 11 consecutive patients (mean age, 70 years) with a combined loss of active elevation and external rotation (shoulder pseudoparalysis and dropping arm) who underwent this procedur

Patient outcome following rehabilitation for rotator cuff repair surgery: the impact of selected medical comorbidities.

STUDY DESIGN: Prospective, multicenter research design. OBJECTIVES: To assess functional and health status outcomes in patients following a physical therapy program after rotator cuff repair surgery, and to determine the impact of selected patient medical comorbidities on rehabilitation outcomes. BACKGROUND: While authors have studied the influence of multiple factors on patient outcomes after rotator cuff repair surgery, little research has been done on the impact of comorbidities, particularly as it relates to establishing an accurate patient prognosis. eighteen patients who had recently u

Role of the coracoacromial ligament as restraint after shoulder hemiarthroplasty.

Patients with an irreparable rotator cuff tear and glenohumeral degeneration often are treated with hemiarthroplasty. This procedure has proven effective as long as the coracoacromial ligament remained intact. The ligament reportedly acts as a restraint against anterosuperior dislocation. The purpose of the current study was to test the role of the coracoacromial ligament as an anterosuperior restraint after hemiarthroplasty in shoulders from cadavers with simulated irreparable rotator cuff tears. Six fresh-frozen shoulders were dissected to mimic a massive rotator cuff tear. After a hemiarthr

MR arthrography of the shoulder: comparison of low-field (0.2 T) vs high-field (1.5 T) imaging.

The objective of this study was to compare the image quality, sensitivity, specificity, and diagnostic accuracy of an open low-field MR system (0.2 T) with a standard high-field MR system (1.5 T) after arthrography of the shoulder. Thirty-eight patients either with suspected chronic instability (n = 12) or rotator cuff abnormalities (n = 26) were examined. Intra-articular injection of diluted Gd-DTPA was followed in randomized order either first by imaging on an open 0.2-T system or on a 1.5-T system. The image material was evaluated independently by two radiologists in a blinded fashion with

Internal impingement of the shoulder: An international survey of 261 orthopaedic surgeons.

BACKGROUND: Internal impingement of the shoulder (IIS) is a specific disorder of young overhead-throwing athletes that was first described in 1991. The many non-operative and surgical treatments suggested to date have produced mixed outcomes. The objective of this study was to compare the practices of surgeons in France versus other countries regarding the diagnosis and treatment of IIS. HYPOTHESIS: Diagnostic and therapeutic practices regarding IIS differ between surgeons in France and in other countries. MATERIAL AND METHODS: A 21-item questionnaire in French and English was emailed to the

Persistent Shoulder Pain After Vaccine Administration Is Associated with Common Incidental Pathology: A Systematic Review.

BACKGROUND: Claims of shoulder injury now account for half of all claims to the Vaccine Injury Compensation Program. Reports from databases of claims or potential adverse events note a relatively high mean age and high prevalences of rotator cuff tendinopathy and adhesive capsulitis-common shoulder problems that might be incidental to vaccination. Published case reports provide much more detail about individual patients than is available in databases. A review of published cases provides an opportunity for more detailed review of symptoms, diagnoses, pathology, treatment, and prognosis. Such a

Current concepts: scapular dyskinesis.

The scapula serves many roles in order for proper shoulder function to occur. These roles include providing synchronous scapular rotation during humeral motion, serving as a stable base for rotator cuff activation and functioning as a link in the kinetic chain. Each role is vital to proper arm function and can only occur when the anatomy around the shoulder is uncompromised. The presence of bony and soft tissue injury as well as muscle weakness and inflexibility can alter the roles of the scapula and alter scapular resting position and/or dynamic motion. This altered scapular position/movement

Effusion criteria and clinical importance of glenohumeral joint fluid: MR imaging evaluation.

PURPOSE: To investigate the clinical importance of glenohumeral joint (GHJ) fluid. MATERIALS AND METHODS: The amount of GHJ fluid in 17 volunteers and 208 consecutive patients was graded at magnetic resonance imaging with T2-weighted fat-suppressed coronal oblique images by two blinded observers. Thorough historical data and physical examination results were available for 108 patients. Presence and grade of GHJ fluid were correlated with age, sex, presence of osteophytes activity scale, supraspinatus tenderness, clinical impingement, prior subacromial injections, rotator cuff tears (RCTs), jo

Long-term results after SLAP repair: a 5-year follow-up study of 107 patients with comparison of patients aged over and under 40 years.

PURPOSE: The aims of this prospective cohort study were to assess the long-term results after isolated superior labral repair and to determine whether the results were associated with age. METHODS: One hundred seven patients underwent repair of isolated SLAP tears. There were 36 women and 71 men with a mean age of 43.8 years (range, 20 to 68 years). Mean follow-up was 5.3 years (range, 4 to 8 years). Of the patients, 62 (57.9%) were aged 40 years or older. Follow-up examinations were performed by an independent examiner; 102 patients (95.3%) had a 5-year follow-up. RESULTS: The Rowe score im

Outcomes after arthroscopic revision rotator cuff repair.

BACKGROUND: Although a number of reports have documented outcomes after open revision rotator cuff repair, there are few studies reporting results after arthroscopic revision. HYPOTHESIS: Arthroscopic repair of failed rotator cuff results in significant improvement in shoulder functional outcome and pain relief. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Multiple variables including demographic data, the number of prior ipsilateral shoulder surgeries, and tear size were recorded from chart review. An independent examiner then measured shoulder strength, range of motion, and s

Transfer of pectoralis major for the treatment of irreparable tears of subscapularis: does it work?

Transfer of pectoralis major has evolved as the most favoured option for the management of the difficult problem of irreparable tears of subscapularis. We describe our experience with this technique in 30 patients divided into three groups. Group I comprised 11 patients with a failed procedure for instability of the shoulder, group II included eight with a failed shoulder replacement and group III, 11 with a massive tear of the rotator cuff. All underwent transfer of the sternal head of pectoralis major to restore the function of subscapularis. At the latest follow-up pain had improved in seve

Intratendinous strain fields of the supraspinatus tendon: effect of a surgically created articular-surface rotator cuff tear.

Articular-surface partial-thickness rotator cuff tears play a significant role in shoulder pathology, but the role of the articular-surface tissue is poorly understood. This investigation assessed the effect of an articular-surface partial-thickness rotator cuff tear on intratendinous strain fields. A magnetic resonance imaging-based technique quantified intratendinous strains in healthy cadaveric shoulders at 15 degrees, 30 degrees, 45 degrees, and 60 degrees of glenohumeral abduction. A second set of magnetic resonance images was acquired after an articular-surface partial-thickness tear was

Results of open repair of the rotator cuff--a long-term review of 79 shoulders.

In 72 patients with 79 tears of the rotator cuff that had been completely repaired by open surgery, the outcome was evaluated on the basis of history, Constant functional score and radiography. At a mean follow-up period of 6.75 years, the Constant score was 71.5 points on average, showing a high correlation with the patients' subjective satisfaction, but the score was not a reliable indicator of recurrence. The larger the cuff tear, the poorer the result was. However, deterioration was not related to the duration of follow-up or the patient's age. The presence of acromioclavicular arthrosis a

Arthroscopic subacromial decompression for chronic impingement. Two- to five-year results.

Subacromial decompression was performed arthroscopically on 65 patients who were evaluated two to five years after the procedure. None had full thickness rotator cuff tears. Patients with partial thickness cuff tears were included in this study in order to allow comparison of arthroscopic acromioplasty with open acromioplasty for stage II impingement. On the UCLA shoulder rating scale, 89% of the cases in this study achieved a satisfactory result. These results compare favourably with those reported following open acromioplasty. The arthroscopic procedure is technically demanding. When properl

Study of bone-tendon interface healing in an animal model using a synthetic scaffold and PRP.

PURPOSE: Biological matrices have been used to reinforce large rotator cuff tear repairs. However, rapid resorption and initial immune reactions presented challenges in clinical practice. This study evaluates whether a resorbable synthetic matrix (scaffold), used alone or with platelet-rich plasma (PRP), impacts repair processes at microscopic, ultrasound, and biomechanical levels in a rabbit model of induced tendon-bone interface injury. METHODS: An experimental study was performed on 24 rabbits. Two experimental groups (n = 12 each) and a control group (n = 24) were defined. In the first gr
