Total obstetric brachial plexus palsy: results and strategy of microsurgical reconstruction.

From 2000 to 2006, 35 infants with total obstetric brachial plexus palsy underwent brachial plexus exploration and reconstruction. The mean … tervention is recommended. Intercostal neurotization is preferred for restoration of elbow flexion. Tendon transfer may be required to improve external rotation in selected cases. Apparently, intact C8 and T1 roots should be left alone if the patient has partial hand recovery, no Horner syndrome, and was operated early (3- or 4-months old). Apparently, intact non …

Autosomal recessive cerebellar ataxias.

Autosomal recessive cerebellar ataxias (ARCA) are a heterogeneous group of rare neurological disorders involving both central and peripheral nervous system, and in some case other systems and organs, and characterized by degeneration or abnormal development of cerebellum and spinal cord, autosomal recessive inheritance and, in most cases, early onset occurring before the age of 20 years. This group encompasses a large number of rare diseases, the most frequent in Caucasian population being Friedreich ataxia (estimated prevalence 2-4/100,000), ataxia-telangiectasia (1-2.5/100,000) and early ons

Long term results of late reconstruction for anterior or anteromedial instability of the knee.

Forty-one patients who had had surgical repair for anterior or anteromedial instability of the knee were examined on average 3.7 years after the operation. Of these patients, 17 had been injured in sports activities, 11 in work accidents, 5 in traffic accidents and 8 during other activities. Twenty-six patients underwent intra-articular reconstruction of the anterior cruciate ligament where the Jones procedure was used, 7 with the HeyGroves procedure, and 2 by the Eriksson procedure. The remaining 6 patients were treated by transfer of the gracilis, semitendinosus tendon or fascia lata. During

What Is the Probability of Radial Nerve Recovery After Surgical Repair of Humerus Fractures Accounting for Time Since Injury?

BACKGROUND: Radial nerve injury after operative treatment of humeral shaft fracture usually improves, but a subgroup of patients does not re … se probabilities can aid in the decision whether and when nerve reconstruction, nerve transfers, or tendon transfers may be beneficial. Because the probability of recovery remains relatively high for at least 7 months after injury, early surgery is unlikely to be beneficial in patients with radial nerve motor injury after surgical fixation of humerus fractures. Hip Power and "Stiff Knee" Gait: A Tool for Identifying Appropriate Candidates for Rectus Transfer.

BACKGROUND: Rectus femoris transfer (RFT) is performed in children with cerebral palsy to treat stiff-knee gait. However, the results are variable in part because there is no gold standard to identify appropriate candidates. Hip flexion kinematics and kinetics are important factors in the determination of peak knee flexion during gait. This study aimed to evaluate whether a kinetic preoperative analysis of hip power can be a predictor of RFT outcomes and determine its usability for identifying appropriate candidates for the procedure. METHODS: Sixteen children with spastic cerebral palsy (20

The treatment of recurrent posterolateral instability of the elbow.

This study describes the surgical technique used for reconstruction and reinforcement of the lateral collateral ligament complex in patients with posterolateral instability of the elbow and the results. A triceps tendon graft from the ipsilateral elbow which was inserted through bone tunnels and fixed with bone anchors augmented the reconstruction. The operation was performed on 18 consecutive patients with instability after an acute traumatic dislocation. The mean follow-up was 44 months (14 to 88). There were no recurrent dislocations. The elbow was stable in 14 patients; three had some mino

[Effects of a proprioceptive training program on sensorimotor capacities of the lower extremity in patients with anterior cruciate ligament instability].

Sensomotory function after ACL-reconstruction was tested prior and after a proprioceptive rehabilitation program with the Kinesthetic Ability Trainer (KAT 2000) which documented the static balance index (SBI). A control group consisting of healthy subjects was also tested two times with a time interval of six weeks. The results showed an adaptation effect to the testing device in the control group with an 17.12% increase of the SBI. There were no differences between the dominant and non dominant extremity. The active sensomotory function in the healthy knee of the operated patients showed no d

Transfer of latissimus dorsi for irreparable rotator-cuff tears.

We treated 12 shoulders in ten patients with irreparable rotator-cuff tears by transfer of the latissimus dorsi. There were nine men and one woman. Their average age was 64.0 years and the average follow-up was 35.6 months (26 to 42). The results were excellent in four shoulders, good in four, fair in one, and poor in three. Active forward flexion improved from a preoperative average of 99 degrees to a postoperative average of 135 degrees. Osteoarthritic changes appeared in five shoulders and proximal migration of the humeral head progressed in six. EMG revealed that nine of the 12 transferred

The mid-term outcomes on reverse total shoulder arthroplasty with isolated latissimus dorsi transfer in patients with isolated external rotation lag without pseudoparalysis: an average follow-up of 5 years.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is a well-established replacement strategy to treat irreparable massive rotator cuff tears and cuff tear arthropathy. In patients with forward (FF) deficits combined with a loss of external rotation (ER), RSA with latissimus dorsi transfer (LDT), which was initially used for brachial plexopathies, has been addressed to restore both functional deficits. The aim of the study is to assess mid-term clinical and radiological outcomes of concomitant RSA with LDT in patients with significant preoperative external rotation lag greater than 30 degr

[Postoperative results in VI nerve palsy].

This study analyze the surgical results obtained in a lot of patients with sixth nerve paralysis operated with different techniques. The results were evaluated using the following parameters: deviation in primary position, abduction deficit, existence of induced vertical deviation and presence of anterior segment ischemia. Ortotropia was achieved in higher percentage in full tendon transposition technique, closely followed by recession-resection technique; the weakest results were recorded in partial tendon transposition technique. Similar results were obtained in abduction improvement. The in

Three-ligament tenodesis for the treatment of scapholunate dissociation: indications and surgical technique.

Different surgical techniques have been proposed to treat traumatic scapholunate instability. Deciding which treatment is best for each individual case is not easy. In this article we report an algorithm of treatment based on a number of prognostic factors that may help in this matter. We also report on the promising results obtained using a new technique, the 3-ligament tenodesis, for the treatment of nonrepairable complete scapholunate ligament rupture, causing a reducible carpal malalignment without secondary osteoarthritis. This technique incorporates features from 3 previously described t
