Outcomes of ulna shortening osteotomy: a cohort analysis of 106 patients.

BACKGROUND: Ulna shortening osteotomy (USO) for ulnar impaction syndrome (UIS) aims to improve pain and function by unloading the ulnar carpus. Previous studies often lack validated patient-reported outcomes or have small sample sizes. The primary objective of this study was to investigate patient-reported pain and hand function at 12 months after USO for UIS. Secondary objectives were to investigate the active range of motion, grip strength, complications, and whether outcomes differed based on etiology. MATERIALS AND METHODS: We report on 106 patients with UIS who received USO between 2012

Access to orthopaedic care for children with medicaid versus private insurance: results of a national survey.

BACKGROUND: It has been documented that children insured by Medicaid in California have significantly less access to orthopedic care than children with private insurance. Low Medicaid physician reimbursement rates have been hypothesized to be a major factor. The first objective of this study was to examine whether children insured by Medicaid have limited access to orthopedic care in a national sample. The second objective was to determine if state variations in Medicaid physician reimbursement rates correlate with access to orthopedic care. METHODS: Two-hundred fifty orthopedic surgeon's off

Burnei's technique in the treatment of radial head displacement; innovative surgery. Study on two cases.

INTRODUCTION: Dislocation of the radial head, congenital or traumatic, anteriorly, posteriorly or laterally displaced, requires surgery to reseat and stabilize the head of the radius within the joint, in order to restore elbow flexion and, as much as possible, pronation-supination. SCOPE: This article is meant to present the technique of proximal radial-ulnar ligament plasty using the extensor carpi radialis longus (ECRL) tendon, as well as other techniques for the stabilization of a dislocated radial head. The ECRL tendon technique, quadrate and annular ligament reconstruction variant was fi

Epiphysiodesis for bilateral irregular closure of the distal radial physis in a gymnast.

Wrist pain is a common complaint in gymnasts. Repetitive stress on the distal radial physis may lead to either gradual slipping of the epiphysis or growth disturbances. In some cases growth disturbances of the distal radial physis lead to triangulation of the distal radius and secondary ulnar overgrowth, and eventually a Madelung-like deformity. The present case report is the first to describe the outcome of epiphysiodesis of the distal radial and ulnar growth plates in a skeletally immature gymnast as a surgical treatment to prevent Madelung's deformity.

Relationship Between Ulnar Variance, Cortical Bone Density, and Load to Failure in the Distal Radius at the Typical Site of Fracture Initiation.

PURPOSE: Increased ulnar variance has been shown to lead to diminished load borne by the distal radius. The purpose of this study was to determine the correlations among ulnar variance, bone mineral density, and load to failure at the distal radius. METHODS: Posteroanterior radiographs and computed tomographic scans were taken of 12 cadaveric forearms in neutral rotation. Ulnar variance was measured for each wrist by the method of perpendiculars. Measurements of cortical, trabecular, and combined bone density were made at the distal radius. We performed linear regression analysis and correlat

A comparative study of fragment-specific versus volar plate fixation of distal radius fractures.

BACKGROUND: There are many plating systems available for treating distal radius fractures, and deciding which to use can be difficult. This prospective cohort study compared outcomes of two commonly used fixation systems: fragment-specific fixation and a fixed-angle volar locking plate system. METHODS: Consecutive distal radius fractures were prospectively evaluated in a fragment-specific fixation cohort and a volar locking plate system cohort. Radiographic, functional, and patient-rated outcomes were collected immediately postoperatively and at 6 and 12 months postoperatively. Complications

Ulnar nonunion after osteoclasis for rotational deformities of the forearm.

PURPOSE: Osteoclasis, a minimally invasive technique to rotate the radius and ulna, is used commonly to correct forearm rotational deformities in children. The purpose of this investigation was to evaluate objectively osteotomy healing in patients treated with osteoclasis, with specific attention given to the risk for nonunion. METHODS: We identified 69 extremities in 65 children treated with osteoclasis and performed retrospective chart and radiographic reviews to evaluate the time to union of the radius and ulna and factors influencing healing. RESULTS: The average rotational correction wa

Hyperkalemic periodic paralysis episode during halothane anesthesia in a horse.

A 7-month-old Quarter Horse filly was admitted for surgical repair of a right olecranon fracture. Anesthesia was achieved with xylazine hydrochloride, guaifenesin, ketamine hydrochloride, and halothane. Two and a half hours after induction of anesthesia, myotonia, muscle fasciculations, and sweating, concurrent with high serum potassium concentration and associated electrocardiographic changes consistent with hyperkalemic periodic paralysis, were observed. Treatment included intermittent positive-pressure ventilation, changing intravenous administration of fluids from lactated Ringer's solutio

Job site analysis facilitates work reintegration.

The Americans with Disabilities Act (ADA) offers challenges and opportunities in rehabilitative care for returning clients to previous employment after medical illness or injury. The process of work reintegration begins with assessing the client's rehabilitation potential to return to work. When clinical reasoning indicates a good rehabilitation potential, a job site analysis (JSA) is conducted to determine the essential functions of the client's work. Upon completion of the JSA, a variety of strategies can be used for successfully returning the client to work. The three case reports in this a

The influence of anatomic factors in elbow joint dislocation.

Twenty-seven patients with elbow joint dislocation were reviewed and treated between 1979 and 1983. For statistical reasons a sample of 27 elbows of healthy adults matched on age and sex were also roentgenographed. A computer-aided analysis of the carrying angle of the arm and the bow angle of the ulnar trochlear notch was made. The stability of the joint was tested clinically. The carrying angle showed no pathologic deviation. Values of the bow angle were at the lower end of the range given in the literature. Referring to these results, only normal anatomic constitutional factors predisposing

Chronic anterior monteggia lesions in children: report of 4 cases treated with closed reduction by ulnar osteotomy and external fixation.

BACKGROUND: Chronic, subacute, and chronic Monteggia fracture dislocations of the elbow are challenging problems. The literature generally recommends complex open reconstructive procedures. We present an alternative, minimally invasive, percutaneous method with external fixation. METHODS: This was a retrospective review of 4 cases presenting with chronic Monteggia lesions with anterior dislocation of the radial head. RESULTS: Reduction of the radiocapitellar joint was successfully achieved in all patients after ulnar osteotomy and gradual correction using the Ilizarov external fixation. Open

Can clinical parameters predict fractures in acute pediatric wrist injuries?

OBJECTIVE: Fractures around the wrist are common in pediatric patients presenting to the emergency department (ED). This pilot study was aimed at identifying clinical variables that are most likely to be associated with a fracture. METHODS: This was a prospective blinded case series of patients 3-18 years of age presenting with an acute (<3 days) wrist injury, without obvious deformity. A team of five investigators blinded to the eventual radiographic findings evaluated patients. Physical examination variables included range of motion (ROM), site of maximal tenderness, and functional deficit.

Changes in length of the radioulnar ligament and distal oblique bundle after Colles' fracture.

The purpose of this study was to investigate changes in length of the radioulnar ligament and distal oblique bundle (DOB) within the distal interosseous membrane after Colles' fracture and correlate the magnitude of the changes in length with clinical features. This study investigated 10 patients with malunion of a Colles' fracture. In three-dimensional computed tomography, the paths of the four limbs of the radioulnar ligament (superficial and deep, dorsal, and palmar limbs) and DOB were modelled and each path length was computed. Differences in length between the affected and contralateral u

Unravelling a hidden pathology of a vertebral fracture in a teenage girl.

BACKGROUND: Although the skeleton remains a common target of primary hyperparathyroidism, the classic bone disease "osteitis fibrosa cystica" is currently rare due to early diagnosis. This case represents severe classic bone manifestations of primary hyperparathyroidism due to delayed diagnosis and delayed medical attention. CASE PRESENTATION: A 19-year-old young female was symptomatically managed for chronic back pain and nonspecific bone pain in the small joints of both hands over 2 months by a general practitioner. The patient had delayed seeking for treatment for 3 months. Later, she was

Assessment of the degradation rates and effectiveness of different coated Mg-Zn-Ca alloy scaffolds for in vivo repair of critical-size bone defects.

Surgical repair of bone defects remains challenging, and the search for alternative procedures is ongoing. Devices made of Mg for bone repair have received much attention owing to their good biocompatibility and mechanical properties. We developed a new type of scaffold made of a Mg-Zn-Ca alloy with a shape that mimics cortical bone and can be filled with morselized bone. We evaluated its durability and efficacy in a rabbit ulna-defect model. Three types of scaffold-surface coating were evaluated: group A, no coating; group B, a 10-μm microarc oxidation coating; group C, a hydrothermal duplex
