Mechanistic-data synergy for predicting fracture load in canine bone with appendicular osteosarcoma.

Canine appendicular osteosarcoma substantially increases the risk of pathological fractures. Limb-sparing approaches such as Stereotactic Body Radiation Therapy (SBRT) can extend and improve the quality of life in large-breed dogs; however, recent clinical studies have reported high post-SBRT fracture rates and significant complications following prophylactic stabilization. While patient-specific Finite Element Analysis (FEA) can estimate pre- and post-stabilization fracture-initiation thresholds, its computational burden limits its clinical applicability. This study presents Machine Learning

Antegrade ESIN technique via the Kocher interval reduces radiation exposure and accelerates recovery in pediatric DRDMJ fractures: A comparative study with cadaveric validation.

BACKGROUND: Distal radius diaphyseal-metaphyseal junction (DRDMJ) fractures in children often require surgical intervention due to the unique anatomical characteristics and high failure rate of nonoperative treatment. However, the choice of internal fixation remains a challenge for pediatric orthopedic surgeons. Traditional fixation methods, including plate and screw fixation, crossed Kirschner wires (K-wires), and external fixators, have drawbacks such as extensive local trauma and the risk of physeal injury. This study evaluates the clinical efficacy of antegrade elastic stable intramedullar

Safety and performance of contemporary press-fit titanium osseointegration implants in lower extremity amputation : a five-year follow-up study.

AIMS: Treatment using bone-anchored prostheses (BAPs) with press-fit femoral osseointegration implants (OIs) has been used in patients with lower limb amputations for more than two decades. The development of these OIs has enabled treatment in patients with a short transfemoral and a transtibial amputation. The aim of this follow-up study was to report the safety of this treatment at five years postoperatively, the prosthesis wearing time (PWT), and health-related quality of life (HRQoL). METHODS: All consecutive patients undergoing treatment between March 2015 and June 2018 using one of thre

Foetal achondroplasia: Prenatal diagnosis, outcome and perspectives.

BACKGROUND: Achondroplasia, due to a specific pathogenic variant in FGFR3, is the most common viable skeletal dysplasia and the diagnosis is mostly done in the prenatal period. Since 2021, the use of Vosoritide, a specific treatment for achondroplasia, validated in phase 3 placebo-controlled trials, has been recommended to significantly increase the height of children and infants. In the light of these new therapeutic prospects, a complete understanding of the pathophysiology of skeletal damages occurring from foetal life is required. OBJECTIVES: To describe foetal imaging and the antenatal a

Computed tomography-based structural rigidity analysis can assess tumor- and treatment-induced changes in rat bones with metastatic lesions.

BACKGROUND: Breast cancer (BrCa) is a predominant malignancy, with metastasis occurring in one in eight patients, nearly half of which target the bone, leading to serious complications such as pain, fractures, and compromised mobility. Structural rigidity, crucial for bone strength, becomes compromised with osteolytic lesions, highlighting the vulnerability and increased fracture risk in affected areas. Historically, two-dimensional radiographs have been employed to predict these fracture risks; however, their limitations in capturing the three-dimensional structural and material changes in bo

Anterolateral congenital tibial bowing: case report.

BACKGROUND: The treatment of congenital curvatures (bowing) of the tibia still represents a challenging problem for all pediatric orthopedic surgeons because of its unpredictable course, especially if pseudoarthrosis occurs after a pathologic fracture of the tibia. CASE PRESENTATION: We describe the case of a child affected by an isolated curvature of his left leg. The congenital malformation was discovered at birth and no other pathological clinical finding was present. The first x-ray showed the presence of a congenital curvature of the tibia of the antero-lateral type. He was born in anoth

What Are the Applications and Limitations of Artificial Intelligence for Fracture Detection and Classification in Orthopaedic Trauma Imaging? A Systematic Review.

BACKGROUND: Artificial-intelligence algorithms derive rules and patterns from large amounts of data to calculate the probabilities of various outcomes using new sets of similar data. In medicine, artificial intelligence (AI) has been applied primarily to image-recognition diagnostic tasks and evaluating the probabilities of particular outcomes after treatment. However, the performance and limitations of AI in the automated detection and classification of fractures has not been examined comprehensively. QUESTION/PURPOSES: In this systematic review, we asked (1) What is the proportion of correc

[Complications of first ray osteotomies: a consecutive series of 475 feet with first metatarsal Scarf osteotomy and first phalanx osteotomy].

PURPOSE OF THE STUDY: Available studies on Scarf osteotomies of the first metatarsal (M1) and first phalange (P1) shortening and varus displacement have reported good results, but have have not focused on complications. We reviewed a consecutive series of 475 feet operated on over a five year period. Our goal was to determine the incidence of complications and to compare our data with reports in the literature. We wanted to know if association with Weil osteotomy on the lateral metatarsals affects the rate of complications. MATERIAL AND METHODS: We used the following inclusion criteria: hallu

Clinical outcome and quantitative evaluation of periprosthetic bone-remodeling of an uncemented femoral component with taper design. A prospective study.

The rationale of uncemented taper stems is based on fixation in the proximal portion of the femoral diaphysis, a self-locking principle, and a low modulus of elasticity. The aim of this study was to evaluate a collarless femoral component designed to be a three-dimensional taper for press-fit insertion with regard to clinical outcome, efficacy of fixation, incidence of osteolysis, and periprosthetic bone-remodeling. The first 48 consecutive patients (50 hips) with osteoarthritis who had had primary total hip arthroplasty using the Cerafit Multicone stem and the Cerafit Triradius-M press-fit cu

Effects of recombinant human growth hormone and insulin-like growth factor-I, with or without 17 beta-estradiol, on bone and mineral homeostasis of aged ovariectomized rats.

This study aimed to evaluate whether recombinant human growth hormone (rhGH) or insulin-like growth factor-I (rhIGF-I) can reverse or prevent further bone loss in aged osteopenic ovariectomized (OVX) rats and to compare their effects with those of 17 beta-estradiol (E2). Twelve-month-old rats were OVX, remained untreated for 8 weeks, and subsequently received daily subcutaneous (SC) injections of rhGH (75 micrograms/day), rhIGF-I (250 micrograms/day), E2 (1.5 micrograms/day), and their respective combinations during 8 weeks, and were then compared with sham-operated, pretreatment OVX, and sali

SECEC Grammont Award 2025: the resting arm position influences impingement-free range of motion and the distalization and lateralization shoulder angle in reverse shoulder arthroplasty using Imascap preoperative planning software.

BACKGROUND: Preoperative planning software enables virtual implantation of reverse total shoulder arthroplasty (rTSA) and assessment of impingement-free range of motion (ROM). Recent studies have shown that scapular resting position varies significantly among individuals and impacts impingement-free ROM. The resting position of the arm also varies between individuals, yet its impact on simulated outcomes and whether this parameter should be integrated into planning programs remains unclear. The objective of this study was to assess the effect of the resting arm position in the coronal plane on

The over-designed limb: Skeletal robustness and locomotor adaptation in terrestrial and freshwater turtles.

The morphology and internal structure of tetrapod limb bones reflect complex interactions between functional demands, mechanical loading, and environmental conditions. In most tetrapods, limb bone microanatomy shows a clear ecological signal, with aquatic taxa typically displaying increased bone compactness and terrestrial taxa exhibiting more open medullary cavities. Testudines represent a notable exception to this pattern, as both terrestrial and aquatic turtles possess unusually compact long bones, suggesting that factors beyond habitat alone shape their skeletal design. Here, we present th

The "umbrella" technique: reducing hoop stress during suprapatellar nailing in complex proximal tibial fractures.

BACKGROUND: The surgical management of complex proximal tibia shaft fractures that extend into the joint remains a difficult challenge. The existing body of literature outlines a variety of reduction and fixation strategies, ranging from traditional double-plate constructs, with or without the use of minifragment plates, to fragment-specific plates combined with intramedullary nails, nail-plate combinations, and circular frames. In patients with severely compromised soft tissues, conventional internal fixation techniques can elevate the risk of complications. Nail-plate combinations have emerg

Changes in Management Based on X-rays after Pinning of Supracondylar Humerus Fractures: Rare, but More Common in Complex Injuries.

Purpose: Clinical practice, including utilization of postoperative x-rays, may vary after pinning of supracondylar humerus fractures in children. The purposes of this study were to: (1) evaluate the frequency at which postoperative radiography resulted in management changes and (2) determine risk factors for such changes, as previous studies may be underpowered for such analyses. Methods: This retrospective cohort study included skeletally immature patients that underwent pinning of a supracondylar humerus fracture at four tertiary children's hospitals. Skeletally mature patients or those requ

Is there a femoral morphology at risk of a cervical or pertrochanteric fracture? Description and validation of the "Neck shaft ratio" and the "Intertrochanteric distal ratio" after analyzing a continuous cohort of 126 bilateral fractures of the proximal femur.

BACKGROUND: Proximal femur fractures constitute a public health concern given their high frequency and the aging population. The frequency of a contralateral fracture occurring can reach up to 15% of cases. Certain historical or demographic factors constitute risk factors for refracture, but the type of fracture, either of the femoral neck (FN) or pertrochanteric (PT), cannot be predicted. The objectives of this retrospective study were: firstly, to analyze several anatomical markers in order to determine whether they predispose to a certain type of fracture in the event of contralateral refra

Does the stemless reverse arthroplasty compare to a conventional stemmed implant? Clinical and radiographic evaluation at 2 years' minimum follow-up.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a well-recognized treatment for many shoulder conditions, including rotator cuff arthropathy, primary glenohumeral joint arthritis, and rheumatoid arthritis, and can be used in both trauma and revision settings. Over the past 10 years, its popularity in New Zealand has been increasing, with a 6%-7% annual growth rate during this period. Stemless RSA designs have the following proposed advantages: They can preserve humeral bone stock, they can limit periprosthetic fractures, and they can be indicated in patients with abnormal diaphyseal humeral

Gradual Transverse Transport of Naturally Tibialized Fibula Using Hexapod Frames to Treat Neglected Type V Tibial Segmental Defects.

OBJECTIVE: Although several reconstructive methods have been developed to manage large segmental tibial bone defects including bone transport (distraction osteogenesis), contralateral fibular graft, allograft, tibiofibular synostosis, Masquelet technique, and 3D printed scaffold, neglected large tibial defects in adults remain challenging problems. This study describes gradual transverse transport of naturally tibialized fibula using hexapod frames in management of adult patients with neglected large tibial defects. METHODS: We retrospectively reviewed four cases of transverse transport of na

Long-term results (after a mean 11.3years, and up to 22years, of follow-up) of the Legacy Constrained Condylar Knee (LCCK™) in primary total knee arthroplasty.

INTRODUCTION: The use of a sliding prosthesis in total knee arthroplasty (TKA) with increased stress is an intermediate solution in primary surgery, between posteriorly stabilized prostheses and hinged prostheses, in cases of ligamentous laxity and/or loss of bone substance. Favorable results have been reported in the medium term but to our knowledge, this type of prosthesis has not been evaluated in Europe beyond 10 years of follow-up. We therefore conducted a retrospective study in order to carry out: 1) the study of the survival of the Legacy Constrained Condylar Knee (LCCK™) prosthesis in

Cessation of ambulation results in a dramatic loss of trabecular bone density in boys with Duchenne muscular dystrophy (DMD).

Glucocorticoids are currently used to improve muscle strength and prolong ambulation in boys with DMD although the effect on bone health is still unclear. The aim of this study was to compare bone strength in healthy children and boys with DMD and investigate the interaction between diminished muscle function, loss of ambulation and high dose oral steroids, over a two year time frame. Fifty children were studied, 14 healthy boys (HB), 13 boys with DMD who remained ambulant (DMD-RA) and 23 boys with DMD who lost ambulation (DMD-LA). All boys with DMD had taken oral glucocorticoids.
