Management of comminuted but continuous mandible defects after gunshot injuries.

INTRODUCTION: Firearm injuries continue as a major public health problem, contributing significant morbidity, mortality, and expense to our society. There are four main steps in the management of patients with gunshot wounds to the face: securing an airway, controlling haemorrhage, identifying other injuries and definitive repair of the traumatic facial deformities. The objective of this study was to determine late outcome of two treatment options by open reduction and internal fixation versus closed reduction and maxillomandibular fixation (MMF) in the treatment of gunshot injuries of the man

Failure of the interosseous membrane to heal with immobilization, pinning of the distal radioulnar joint, and bipolar radial head replacement in a case of Essex-Lopresti injury: case report.

The authors report a case of a patient with complex radial head and ulnar shaft fractures with an associated tear of the interosseous membrane, which failed to heal despite open reduction internal fixation of the ulna, a bipolar radial head replacement, and cross pinning of the distal radioulnar joint. This case demonstrates that failure of the interosseous membrane can occur even in a properly replaced bipolar radial head with adequate immobilization of the forearm and pinning of the distal radioulnar joint.

Polytrauma Patients managed by Damage Control Orthopedics or Early Total Care - Assessment of Biomarkers.

INTRODUCTION: Post-operative complications related to hardware, particularly symptoms caused by it, such as implant impingement, wire breakage, and skin infection are frequently reported following surgical management of patellar fractures with stainless steel wires. As a potential solution, some researchers have advocated for the use of non-absorbable sutures for fracture fixation. This study aims to investigate the clinical and radiological outcomes of patients treated with fiber taping as an alternative technique. MATERIALS AND METHODS: We conducted a retrospective evaluation of 20 patients

Giant Morel-Lavallée lesion complicated with open pelvic fracture: a rare case report.

BACKGROUND: Morel-Lavallée lesion (MLL) is a rare closed degloving injury from high-energy shear/crush trauma, often linked to pelvic/acetabular fractures. Due to limited clinical awareness, it is highly prone to misdiagnosis, potentially causing severe complications like infection and skin necrosis. Existing literature rarely documents giant MLL with open pelvic fractures. This article reports such a case in an obese patient after heavy-object impact, aiming to enhance understanding and improve management of complex injuries. CASE PRESENTATION: A 27-year-old male sustained lumbosacral giant

Destructive disasters, trauma, crush syndrome, and beyond.

Orthopedic injuries, especially fractures of long bones as well as multiple fractures and comminuted fractures, are very common after destructive disasters (e.g., earthquakes, wars, and hurricanes). Another frequent problem is traumatic rhabdomyolysis, which may result in crush syndrome, the second most frequent cause of death after direct traumatic impact following earthquakes. To improve outcomes, interventions should be initiated even before extrication of the victims, which include maintenance of airway patency and spine stabilization, stopping traumatic bleeding by any means, and initiati

Osteosynthesis of pediatric femoral shaft fractures with flexible intramedullary nailing-experience from developing world.

BACKGROUND: Flexible intramedullary nailing (FIN) has become the standard treatment for pediatric femoral fractures in the 6-14 years age group. The other treatment options include traction plus spica casting and external fixation. In spite of excellent results described, there is disunity in the orthopedic fraternity regarding its usefulness and nail material. The aim of our study is to determine the outcome of pediatric femur fractures treated with Stainless steel FIN. METHODOLOGY: Between 2014 to 2018, 45 children within the age group of 6-12 years with femoral shaft fractures were include

Use of a Multivector Mandibular Distractor for Treatment of Pediatric Proximal Interphalangeal Joint Pilon Fractures: A Case-Based Review.

BACKGROUND: The difficulties in surgical treatment of pilon fractures of the finger include fragment reconstitution and posthealing stiffness. In adults, external fixation with traction and early active range of motion (AROM)/passive range of motion (PROM) during healing is considered necessary for avoiding joint stiffness and attaining realignment. The authors present a unique approach to pediatric pilon fractures that uses open reduction and multivector external fixation with delayed AROM/PROM. Initial immobilization and significant traction allowed for joint realignment and prevented noncom

A novel approach with modified suture bridge fixation technique for posterior cruciate ligament tibial avulsion fracture in adult.

BACKGROUND: Posterior cruciate ligament (PCL) tibial avulsion fractures are relatively rare injuries that often result in chronic pain, limited range of motion, knee instability, and osteoarthritis. Most cases require surgical intervention to restore the fragment's normal anatomy, the ligament's tension, and the knee joint's stability. In this study, we propose a novel approach utilizing a modified suture bridge fixation technique to treat PCL tibial avulsion fractures and explore the clinical outcome and applicability. METHODS: We retrospectively reviewed and collected the clinical data from

The Utility of Sulfur Colloid Imaging to Differentiate Charcot's Neuroarthropathy from Osteomyelitis: A Case Study.

Charcot's neuroarthropathy and osteomyelitis can have similar initial presentations. The ability to differentiate between the two pathologic conditions is essential, as each requires different treatment. We present a case of a 53-year-old woman with pain, swelling, and warmth in her left first metatarsophalangeal joint and first tarsometatarsal joint. Radiographs showed comminuted fractures at the base of the first metatarsal. Osteomyelitis was suspected by the primary team based on physical findings and a history of previous first metatarsophalangeal joint arthrodesis. A triphasic bone scan a

Surgical management of a patient with severe and complex maxillofacial injuries caused by a bear mauling in the forest: A case report from Thailand.

A 60-year-old Thai male sustained severe and complex maxillofacial injuries, along with superficial neck and abdominal injuries, from being mauled by a black bear in Thailand. The accident caused massive injury to the face, including the right eye and periorbital soft tissue, right cheek, total nose, total upper lip, partial lower lip, and underlying bone structures. A computed tomography scan showed comminuted pan-facial fractures. Lifesaving procedures were performed at nearby hospitals, and then he was referred to our center one month after the injury. Our initial multi-stage surgical manag

[Influence of the method of measurement of muscle force on results of isolated resections of the head of the radius in the adult].

A statistical study of a group of 50 patients determined the mean difference in grip strength between the dominant and non-dominant hands. For every position of the wrist a mean non-dominant/dominant ratio was calculated. This ratio, 87% in the neutral position, was then used to assess grip strength in relation to the dominance of the injured limb in 15 patients who had undergone 16 radial head excisions, a mean time of 74.4 months earlier. Using this method as a correction factor, results vary considerably according to the functional scoring system used. Loss of grip strength was more frequen

[Homologous transplantation of spongiosa. Evaluation following 2 1/2 years].

Sufficient stability in comminuted fractures of the lower limb, in correcting bone axis or in hip surgery can often be achieved only by adjunctive transplantation of cancellous bone. The homologous cancellous bones is cut to chips out of extracted femoral heads and frozen in a bone bank at -30 degrees C. The homologous material is available whether or not the patient himself has enough autologous spongiosa. Establishing a bone bank in a small hospital can be done with small effort and works well, as experience shows. Immunological phenomena do not cause any problems although they are responsib
