[Status of cellular immunity in viral, serous and meningococcal meningitis].

The paper is concerned with the immunological status of 62 patients with meningococcal and viral serous meningitides. A noticeable inhibition of cellular immunity, more pronounced in the CSF was detected. A correlation was shown to exist between the level of humoral factors that block T lymphocyte receptors in the CSF and the disease gravity.

Computed tomography in diabetes insipidus.

Thirteen patients aged 1 to 59 years hospitalized for diabetes insipidus (DI) were investigated by computed tomography (CT). Apart from one craniopharyngioma, CT revealed in 8 out of 12 cases several modifications in the hypothalamic-pituitary region. In this group, CT demonstrated a small, round, isodense mass in the chiasmatic cistern that enhanced after contrast medium injection. Coronal and sagittal reconstructions helped pinpoint the lesion in the infundibulum and pituitary stalk, and, in one case, in the supraoptic nuclei. These findings, and the noninvasiveness of CT, make this techniqu

Accidental subarachnoid injection of gallamine. A case report.

Accidental subarachnoid injection of gallamine and its management is reported. Methods of identification of the drug in the c.s.f. are described. Early treatment should be initiated, since the drug was still identifiable in the c.s.f. after 90 min. Removal of the c.s.f. and sedation may have prevented a fatal outcome in our patient.

Increased cerebral blood flow but no reversal or prevention of vasospasm in response to L-arginine infusion after subarachnoid hemorrhage.

OBJECT: The reduction in the level of nitric oxide (NO) is a purported mechanism of delayed vasospasm after subarachnoid hemorrhage (SAH). Evidence in support of a causative role for NO includes the disappearance of nitric oxide synthase (NOS) from the adventitia of vessels in spasm, the destruction of NO by hemoglobin released from the clot into the subarachnoid space, and reversal of vasospasm by intracarotid NO. The authors sought to establish whether administration of L-arginine, the substrate of the NO-producing enzyme NOS, would reverse and/or prevent vasospasm in a primate model of SAH.…

Simultaneous activation of border-associated immune cells and glial cells at the CNS-meningeal interface after subarachnoid haemorrhage in rats.

Border-associated macrophages (BAM) and mast cells are resident immune cells at the peripheral CNS borders, strategically located close to the brain surface, potentially influencing the homeostasis of the underlying parenchyma. Subarachnoid haemorrhage (SAH), when blood enters between the meningeal layers that cover the brain, is associated with neuroinflammation, which has been shown to play a critical role in subsequent brain damage; however, the impact of the activation of border-associated immune cells on the pathomechanism of the disease has not been investigated. Our aim was to examine i

Cranial Nerve III Palsy as the First Sign of Carcinomatous Meningitis From Non-Hodgkin's Lymphoma.

Carcinomatous meningitis (CM) is characterized by the multifocal dissemination of malignant cells into the cerebrospinal fluid (CSF), pia mater, and subarachnoid space. Involvement can occur in the advanced stage of malignancy, causing multifocal involvement and a wide array of symptoms. Diagnosis requires suspicions and a multimodal approach that includes imaging, lumbar puncture, and diagnostic laboratory evaluation. This case represents a female with a history of non-Hodgkin's lymphoma (NHL) and venous thromboembolism on chronic anticoagulation who presented due to acute encephalopathy, hal…

Clipping of Kissing Distal Anterior Cerebral Artery Aneurysms: The Importance of Microsurgical Techniques to Enlarge the Surgical Corridor and Prevent a Premature Aneurysm Rupture.

Intracranial kissing aneurysms, arising either from the same artery or from 2 adjacent arteries at similar locations, are rare.1,2 The internal carotid artery is most frequently involved; kissing aneurysms rarely affect the distal anterior cerebral artery (DACA). By dint of the close proximity of the aneurysm fundus, these aneurysms can pose unique operative challenges.3,4 A highly fragile aneurysm dome with a high intraoperative rupture rate is a unique management challenge in DACA aneurysms.5 The stakes are higher when there is an aneurysm rupture in the setting of kissing DACA aneurysms req

A Case of Superficial Siderosis with Elevated Anti-Ro/SSA Antibody.

Superficial siderosis (SS) of the central nervous system is a rare disorder that is caused by chronic or recurrent hemorrhage in the subarachnoid space via a dural defect at the spinal level. The most common clinical features of SS include slow-progressive sensorineural deafness, cerebellar symptoms, and pyramidal tract signs. Considering that SS can present with broad clinical manifestations, for precise diagnosis, this disease must be understood. Anti-Ro/SSA antibodies are commonly detected in patients with Sjögren's syndrome and are utilized as markers for autoimmune diseases. In this repor…

Impact of Shunt Placement on CSF Dynamics.

BACKGROUND: CSF dynamics are disturbed in chronic hydrocephalus (NPH). We hypothesise that these alterations reflect a disturbance of intrac … cular CSF at the level of the aqueduct of Sylvius (SVAQU) and CSF at the level of the high cervical subarachnoid spaces (SVCERV). We calculated the ratio between SVAQU and SVCERV, called CSFRATIO, which reflects the participation of intraventricular pulsatility in overall intracranial CSF pulsatility. RESULTS: SVAQU significantly (p = 0.003) decreased from 240 ± 11 …

Systematic Review: Evaluating the Efficacy of Intrathecal Fluorescein for Localizing Cerebrospinal Fluid Rhinorrhea.

OBJECTIVES: A defect in the skull base can result in leakage of cerebrospinal fluid (CSF) out of the subarachnoid space into the sinonasal cavities, termed CSF rhinorrhea. Patients presenting with CSF rhinorrhea often require surgical repair, typically performed endoscopically. Successful surgical intervention is highly reliant on accurate identification of the leak site. Identification can be enhanced by the administration of intrathecal fluorescein (IF) via a lumbar drain before surgery. The objective of this systematic review is to better characterize the benefits and limitations of IF admi…

Preoperative Third Ventricle Floor Bowing is Associated with Increased Surgical Success Rate in Patients Undergoing Endoscopic Third Ventriculostomy - A Systematic Review and Meta-analysis.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is a procedure that involves devising an opening in the third ventricle floor, allowing cerebrospinal fluid to flow into the prepontine cistern and the subarachnoid space. Third ventricular floor bowing (TVFB) serves as an indicator of intraventricular obstruction in hydrocephalus and existence of pressure gradient across third ventricular floor, which is the prerequisite of a successful ETV. OBJECTIVE: In this systematic review and meta-analysis, we aimed to synthesize the latest evidence on the TVFB as a marker for surgical success in patie…
