Evidence for disturbed cortical signal processing and altered serotonergic neurotransmission in generalized anxiety disorder.

BACKGROUND: Current pathophysiological concepts of generalized anxiety disorder (GAD) assume a disturbed exteroceptive sensory system. Furthermore, central serotonergic neurotransmission has been shown to play an important role in anxiety disorder. Cortical signal processing as measured by auditory evoked potentials (AEPs) may reflect the integrity of the exteroceptive sensory system. Because a special aspect of AEP, the loudness dependence of the N1/P2-component (LD), has been related to central serotonergic activity, the LD may be useful for investigating serotonergic dysfunctions in GAD. Changes in waiting-list patients over time: data on some commonly-used measures. Beware!

When samples of psychiatric patients are assessed on 2 occasions on state and trait measures, without any formal intervention having yet taken place, a mean change in scores towards less psychopathology is often observed. This re-test effect, which is a potential threat to the validity of longitudinal studies, has not been given serious attention by researchers using repeated administrations. The present study addressed this issue by examining re-test effects in 2 independent clinical samples with the Symptom Check List-90-Revised and other widely-used state and trait measures. Quality of life in patients with panic disorder.

In this study we assessed the quality of life of patients with panic disorder, with particular attention to the influence of anxiety and depression comorbidity on quality of life. Findings were compared with established general population norms as well as norms for patients with chronic medical conditions and major depression. The Medical Outcomes Study Short-Form Health Survey (SF-36) was administered to panic disorder patients entering clinical trials or treatment in an outpatient anxiety disorders program. Subjects were 73 consecutive patients with a primary diagnosis of panic disorder with

Patterns of change during cognitive behavioral treatment for panic disorder.

The present investigation examined patterns of change in basic features of panic and anxiety during cognitive behavioral treatment for panic disorder. Data were collected in the context of a study comparing the efficacy of cognitive therapy (CT) and relaxation training, both administered without exposure-based treatments of any kind. Thirty-six panic disorder patients completed 10 weeks of either CT or relaxation training. Weekly measures of panic frequency, state and trait anxiety, and associated fears were obtained. Data were examined using both multivariate techniques (which conceptualize c

Reliability of DSM-III-R anxiety disorder categories. Using the Anxiety Disorders Interview Schedule-Revised (ADIS-R).

A large reliability study of DSM-III-R anxiety disorders is reported in which outpatients (n = 267) received two independent structured interviews (Anxiety Disorders Interview Schedule-Revised). It is the only reliability study to date in which the final DSM-III-R criteria are used throughout the study. Reliability was assessed for each diagnosis when it was assigned as a principal diagnosis and when it was assigned as either a principal or an additional diagnosis. Excellent reliability was obtained for current principal diagnoses of simple phobia, social phobia, and obsessive-compulsive disor

Sodium D-lactate infusion of panic disorder patients.

Nineteen patients with panic disorder received sodium D,L-lactate (racemic) and pure sodium D-lactate infusions in a pilot study. Sodium D-lactate, which is less metabolically active than L-lactate, produced panic attacks in half the patients. D-Lactate also produced hypocapnia and alkalosis, indicating hyperventilation. These findings suggest that metabolism of lactate is not necessary for the induction of panic in susceptible patients. D-Lactate appears similar to other agents that cause panic in its capacity to stimulate respiration in the preliminary study.

Relationships Between Somatic Symptoms and Panic-Agoraphobic Spectrum Among Frequent Attenders of the General Practice in Italy.

Frequent attenders (FAs) of general practitioners (GPs) often complain of nonspecific physical symptoms that are difficult to define according to typical medical syndromes criteria but could be acknowledged as atypical manifestations of mental disorders. We investigated the possible correlation between somatic symptoms and panic-agoraphobic spectrum symptoms in a sample of 75 FAs of GPs in Italy, with particular attention to the impact on functional impairment. Assessments included the Patient Health Questionnaire, Panic-Agoraphobic Spectrum-Self-Report (PAS-SR) lifetime version, Global Assess

The structure and dimensionality of the Inventory of Depressive Symptomatology Self Report (IDS-SR) in patients with depressive disorders and healthy controls.

BACKGROUND: The Inventory of Depressive Symptomatology Self Report (IDS-SR) is a widely used but heterogeneous measure of depression severity. Insight in its factor structure and dimensionality could help to develop more homogeneous IDS-SR subscales. However previous factoranalytical studies have found mixed results. Therefore, the present study tested which factor structure underlies the IDS-SR and, in addition, if the factors can be used as unidimensional subscales. METHODS: Confirmatory factor analysis (CFA) was done to identify the best-fitting factor structure. The study sample consisted

Quality of life and treatment outcome in panic disorder: cognitive behavior group therapy effects in patients refractory to medication treatment.

BACKGROUND: Improvements in quality of life (QoL) as well as symptomatic relief are important outcomes for the treatment of panic disorder (PD). The aim of this study is to assess the impact of brief cognitive behavior group therapy (CBGT) for panic disorder on QoL and to identify the clinical features associated with these changes. METHODS: Thirty-six patients with PD refractory to pharmacological treatment took part in a treatment protocol consisting of 12 sessions of CBGT. To evaluate the changes in QoL, the WHOQOL-bref was administered before and after treatment. RESULTS: Thirty-two pati

Implicit and explicit memory processes in panic patients as reflected in behavioral and electrophysiological measures.

Implicit and explicit memory processes for panic-relevant and neutral word stimuli were examined in 16 panic patients and 16 healthy participants matched for sex, age, and education, using behavioral and electrophysiological measures. In the study phase, panic-relevant and neutral words were presented and the level of processing was varied by requiring either shallow (orthographic) or deeper (syntactic) processing. Implicit memory was tested with a lexical decision task, explicit memory with a recognition task. Panic patients and healthy participants did not differ in behavioral (response time

Review of the long-term effectiveness of cognitive behavioral therapy compared to medications in panic disorder.

Panic disorder is a recurrent and disabling illness. It is believed that Cognitive Behavioral Therapy (CBT) has a long-term protective effect for this disorder. This would offer CBT considerable advantage over medication management of panic disorder, as patients often relapse when they are tapered off their medications. This is a review of the literature about the long-term effectiveness of CBT. We searched for follow-up studies of panic disorder using CBT. Of the 78 citations produced in the initial search, most had major methodological flaws, including ignoring losses to follow-up, not accou

["Phobic postural vertigo". A further differentiation of psychogenic vertigo conditions seems necessary].

Due to the results of an interdisciplinary study on patients with vertigo as the chief symptom and on the background of psychodynamic theories concerning anxiety disorders the term of phobic postural vertigo (Brandt & Dieterich 1986) is discussed. It becomes obvious that phobic postural vertigo is a generalizing term which encompasses different forms of psychogenic vertigo. The authors plead for a more differentiated diagnosis and subgroup oriented classification of vertigo caused by psychiatric disorders.

Effects of intravenous diazepam pretreatment on lactate-induced panic.

The psychological and physiological effects of acute low-potency benzodiazepine administration on lactate-induced panic were examined in 10 patients with panic disorder (PD). The patients, who had panicked during a standard sodium-lactate infusion, underwent a repeat infusion modified by pretreatment with intravenous diazepam (5 mg). Acute Panic Inventory (API) scores preceding the second lactate infusion, which were associated with diazepam pretreatment, were significantly reduced in compared with those measured before the first lactate infusion. However, the second visit "fear of doom" item

Pharmacotherapy of the panic disorders.

The evidence supporting the effective pharmacotherapy of the panic disorders is reviewed. The medications discussed include the tricyclic antidepressants, the monoamine oxidase inhibitors, and the benzodiazepines. Clinical guidelines are developed for each group and cover such issues as side effects, dosing, length of treatment, and relapse after discontinuation of medications. Although this paper primarily reviews the pharmacologic treatment of panic disorders, nonpharmacologic behavioral treatments are briefly discussed, especially as they relate to the pharmacologic treatment of these disor

Changes in the drug treatment of anxiety disorders.

One of the major changes in the clinical treatment of anxiety has been the substitution of benzodiazepines for less safe sedative-hypnotic drugs. A second major change has been the finding that antidepressants suppress panic attacks. The evidence for this is reviewed in this paper. It appears that panic anxiety is qualitatively different from other kinds of anxiety; this has important implications for diagnosis and treatment. Other clinical issues presented in this symposium are a review of benzodiazepine and new, non-benzodiazepine anxiolytic drugs, the treatment of separation anxiety, and th

Panic and generalized anxiety disorders. Developmental antecedents and precipitants.

Seventeen subjects with panic disorder (PD) and 16 subjects with generalized anxiety disorder (GAD) were interviewed to obtain their developmental and psychiatric histories. The groups reported a similar incidence of early separation, separation disorder in childhood, and separation causing exacerbation of symptoms. The groups differed significantly in that those with PD had a higher incidence of a grossly disturbed childhood environmental and major depressive episodes. The results of this preliminary study support the validity of the DSM-III distinction between PD and GAD.

Subthreshold anxiety better defined by symptom self-report than by diagnostic interview.

BACKGROUND: There is no consensus on how to define subthreshold anxiety. Based on functional impairment, we aim to evaluate the use of a diagnostic instrument and an anxiety severity questionnaire to derive an empirical cut-off for defining clinically relevant, subthreshold anxiety. METHODS: Our sample consisted of 1788 subjects without full-syndromal anxiety disorders from the Netherlands Study of Depression and Anxiety (NESDA). We used ANCOVA to compare the Composite International Diagnostic Interview (CIDI) and the Beck Anxiety Inventory (BAI) in their association with functional impairmen

A randomized controlled clinical trial of psychoanalytic psychotherapy for panic disorder.

OBJECTIVE: The purpose of this study was to determine the efficacy of panic-focused psychodynamic psychotherapy relative to applied relaxation training, a credible psychotherapy comparison condition. Despite the widespread clinical use of psychodynamic psychotherapies, randomized controlled clinical trials evaluating such psychotherapies for axis I disorders have lagged. To the authors' knowledge, this is the first efficacy randomized controlled clinical trial of panic-focused psychodynamic psychotherapy, a manualized psychoanalytical psychotherapy for patients with DSM-IV panic disorder.
