Safety of Infliximab Following Live Virus Vaccination in Kawasaki Disease Patients.

No published data are available regarding infliximab use in Kawasaki disease patients with recent live virus vaccination. We investigated 38 acute Kawasaki disease patients who received infliximab within 90 days of receiving measles-mumps-rubella, varicella-zoster virus, or Rotavirus vaccines. There were no adverse events or serious infections, which suggests that infliximab use in acute Kawasaki disease patients with recent live viral vaccinations is safe.

Seroprevalence of Antibodies against Measles, Rubella and Varicella among Asylum Seekers Arriving in Lower Saxony, Germany, November 2014-October 2015.

The number of asylum seekers arriving in Germany has increased rapidly since 2014 and cases of vaccine-preventable diseases at reception centres were reported. Asylum seekers 12 years and older arriving in Lower Saxony were serologically screened for antibodies against measles, rubella and varicella between November 2014 and October 2015. We calculated the seroprevalence from the screening data by disease, country of origin and age group and compared them to literature-based herd immunity thresholds in order to identify immunisation gaps. In total, 23,647 specimens were included in our study.

Evaluation of a commercial glycoprotein enzyme-linked immunosorbent assay for measuring vaccine immunity to varicella.

PURPOSE: To evaluate a recently marketed commercial glycoprotein enzyme-linked immunosorbent assay (gpEIA) kit, the VaccZyme™ VZV gpEIA, for measuring the immunity of varicella-vaccinated children. MATERIALS AND METHODS: We investigated the accuracy and reproducibility of the VaccZyme™ VZV gpEIA kit for the detection of antibodies to VZV. We also examined the sensitivity, specificity, and correlation between antibody titers calculated with gpEIA versus fluorescent antibody to membrane antigen (FAMA) by using sera of 349 children, ranging from 1 to 6 years old. RESULTS: VaccZyme™ VZV gpEIA ga

Safety of vaccines used for routine immunization of U.S. children: a systematic review.

BACKGROUND: Concerns about vaccine safety have led some parents to decline recommended vaccination of their children, leading to the resurgence of diseases. Reassurance of vaccine safety remains critical for population health. This study systematically reviewed the literature on the safety of routine vaccines recommended for children in the United States. METHODS: Data sources included PubMed, Advisory Committee on Immunization Practices statements, package inserts, existing reviews, manufacturer information packets, and the 2011 Institute of Medicine consensus report on vaccine safety. We au

Disseminated, persistent, and fatal infection due to the vaccine strain of varicella-zoster virus in an adult following stem cell transplantation.

Live attenuated varicella vaccine is recommended for healthy individuals who are susceptible to varicella. Although the vaccine is safe, effective, and used worldwide, serious adverse events have been reported, mainly in immunocompromised patients who subsequently recovered. Here, we describe the fatality of an immunocompromised patient who received the varicella vaccine. His medical history provides a cautionary lens through which to view the decision of when vaccination is appropriate. A middle-aged man with non-Hodgkin lymphoma received chemotherapy and a stem cell transplant. He was vaccin

Projecting adverse event incidence rates using empirical Bayes methodology.

Although there is considerable interest in adverse events observed in clinical trials, projecting adverse event incidence rates in an extended period can be of interest when the trial duration is limited compared to clinical practice. A naïve method for making projections might involve modeling the observed rates into the future for each adverse event. However, such an approach overlooks the information that can be borrowed across all the adverse event data. We propose a method that weights each projection using a shrinkage factor; the adverse event-specific shrinkage is a probability, based o

[Infections transmitted from the mother to the fetus: diagnostic issues and management of pregnancy].

Some infections are considered as feared risks during pregnancy. These infections may lead to severe damage of the fetus or the newborn depending on the infectious agent and the term of pregnancy where the infection occurred. Antenatal screening (in France it concerns toxoplasmosis, rubella, syphilis and hepatitis B) play an important role in prevention and management of vertically transmissible infections. However, biological diagnosis is also essential when maternal/neo-natal clinical symptoms or abnormal ultrasound findings are observed. In this article we chose to focus on rubella, varicel

The further and future evolution of the New Zealand Immunisation Schedule.

MeNZB was introduced to control meningococcal disease in New Zealand in 2004 and routine use ceased in 2008. In that year, two new vaccines were added to the New Zealand Childhood Immunisation Schedule, pneumococcal and human papilloma virus, and two more, varicella and rotavirus, have been recommended but not funded. By comparison, in the 16 years prior to 2006 only one new vaccine was introduced, Haemophilus influenzae type B. Coverage is improving and is now around 90%, making timeliness an important target and supplementary strategies for controlling pertussis of greater importance. A pers

Spectrum of purpura fulminans: report of three classical prototypes and review of management strategies.

Purpura fulminans is a rare syndrome of intravascular thrombosis and hemorrhagic infarction of the skin that is rapidly progressive and is accompanied by vascular collapse and disseminated intravascular coagulation. It usually occurs in children, but this syndrome has also been noted in adults. The three forms of this disease are classified by the triggering mechanisms. We describe three classical cases of purpura fulminans of the three classical prototypes treated at our center and their varied clinical outcomes. We also describe a case of acute infectious purpura fulminans secondary to syste

Probiotics and the immunological response to infant vaccinations: a prospective, placebo controlled pilot study.

BACKGROUND: Probiotics have been shown to be immunomodulatory and can affect antibody responses following vaccination. Several immunisations are associated with suboptimal seroconversion rates leaving a substantial part of the population exposed to infection. OBJECTIVES: To evaluate the influence of probiotic supplementation on the immune response of infants following mumps, measles, rubella and varicella vaccination. METHODS: A randomised, placebo-controlled, double blinded prospective trial was performed in a cohort of healthy infants. Study subjects were randomly assigned to receive probi

Varicella as a trigger of atypical haemolytic uraemic syndrome associated with complement dysfunction: two cases.

We report two cases of children who presented with haemolytic uraemic syndrome following varicella infection. One of them had a membrane cofactor protein mutation, and the other had anti-factor H antibodies. These observations show that infectious agents such as varicella-zoster virus may be the trigger of haemolytic uraemic syndrome in patients with complement dysregulation.

Consequences of varicella in pregnancy: a report of four cases.

UNLABELLED: Four infants are reported with varicella-Zoster virus (VZV) infection, whose mothers had varicella during the second-third trimester of pregnancy. Two newborns had neonatal varicella. One of them, whose mother contracted varicella 5 days before delivery, had a severe and complicated form of the disease. The infants who had herpes zoster did not have specific VZV-IgG antibodies at the onset of the disease. CONCLUSION: These cases showed that varicella during the second-third trimester of pregnancy may have serious consequence for infants.

Screening emergency department patients for under-immunized adolescents.

Among 200 adolescents presenting to an emergency department in a city with high vaccination coverage, 11 (6%) were seronegative to rubella or varicella. Seronegative adolescents were older and more likely to be immigrants. ED screening for under-immunized adolescents may have a low yield in settings with high vaccine coverage.

Varicella-associated invasive group A streptococcal disease in Alberta, Canada--2000-2002.

We determined the incidence of invasive group A streptococcal (GAS) disease among children < or =14 years of age in Alberta, Canada, with a focus on infections associated with varicella. During 2000-2002, there were 71 cases of invasive GAS disease among patients in this age group. Fourteen cases occurred in children with varicella (19.7%), for an annual incidence of 0.73 varicella-associated cases per 100,000 children. Necrotizing fasciitis was not noted in patients with invasive GAS disease and varicella.

Fetal varicella-herpes zoster syndrome in early pregnancy: ultrasonographic and morphological correlation.

We report a case of an intrauterine fetal infection by the varicella-herpes zoster virus following maternal varicella at 17 weeks' amenorrhea. Prenatal diagnosis of fetal infection was confirmed by serology and fetal damage by ultrasonography. Autopsy of the fetus showed multiorgan manifestation with disseminated foci of necrosis and microcalcifications, encephalitis and unilateral segmental skin scarring with an underlying hypoplastic fixed lower limb. The placenta showed a multifocal chronic villitis with multinucleated giant cells. The lesions included segmental anomalies and multiorgan dam

The congenital varicella syndrome.

Maternal varicella during the first two trimesters of pregnancy may cause the congenital varicella syndrome (CVS). After infection in the first 20 weeks' gestation, the incidence is estimated to be about 2%. To date, nearly 100 infants born with signs of CVS have been reported in the literature, more than three quarters of them during the last two decades. The characteristic symptoms consist of skin lesions in dermatomal distribution (76%), neurologic defects (60%), eye diseases (51%), and skeletal anomalies (49%). About 30% of infants born with these lesions died in the first months of life.
