Ankle fractures involving the fibula proximal to the distal tibiofibular syndesmosis.

Thirty-two cases of ankle fractures associated with fibular fractures above the distal tibiofibular syndesmosis were studied. All were treated with open reduction and internal fixation. The average follow-up was 25 months. The results of the postoperative evaluation were rated, based on subjective clinical criteria, as good, fair, and poor. According to the Lauge-Hansen classification, there were 17 (53%) cases of supination-external rotation injury (2 stage 2 and 15 stage 4), 9 (28%) cases of stage 3 pronation-abduction injury, and 6 (19%) cases of pronation-external rotation injury (3 stage

Functional Outcome of Radius and Ulnar Shaft Fractures Treated With Intramedullary Kirschner's Wire Fixation in Children.

INTRODUCTION: Fractures are a common occurrence in childhood, with approximately one-third of boys and girls sustaining at least one fracture before the age of 17. Both-bone forearm fractures, particularly those involving the radius and ulna, are more common in the non-dominant hand and in boys and usually involve the distal portions of both bones. If not properly treated, these injuries can have a significant impact on limb function. OBJECTIVE: This study aimed to determine the functional outcomes of radius and ulnar shaft fractures in children treated with intramedullary Kirschner's wire fi

Flexible Intramedullary Nail Placement in Pediatric Humerus Fractures.

BACKGROUND: Flexible intramedullary nailing is an effective method of stabilization in pediatric patients with a humeral shaft fracture when surgery is indicated1-3. Although these fractures are most often treated nonoperatively, operative indications include open fractures, bilateral injuries, compartment syndrome, pathologic fractures, neurovascular compromise, unacceptable alignment after attempted nonoperative treatment, and ipsilateral upper-extremity injuries4. The current literature on flexible intramedullary nailing of the pediatric humeral shaft lacks concise descriptions of available

Transverse subtrochanteric shortening osteotomy with double tension-band fixation during THA for Crowe III-IV developmental dysplasia: 12-year outcomes.

BACKGROUND: When performing total hip arthroplasty in patients with severe developmental dysplasia, shortening the femur facilitates reduction while also preventing sciatic or crural nerve injury and excessive length of the operated limb. No consensus exists about the optimal type of implant and best internal fixation procedure, two parameters that directly govern the risk of the most common intraoperative and postoperative complications (diaphyseal fractures, dislocation, non-union). To minimise these risks, we developed a technique combining a transverse subtrochanteric shortening osteotomy,

[Supracondylar femoral extension osteotomy for knee flexion contracture correction in poliomyelitic conditions].

PURPOSE OF THE STUDY: Knee flexion contracture due to quadriceps paralysis is a major handicap in poliomyelitis patients. The patient has to stabilize the knee with the ipsilateral hand to achieve weight bearing and the deformed knee precludes use of orthopedic devices. Extension can be achieved with supracondylar femoral osteotomy if the knee flexion contracture is less than 30 degrees. We assessed functional and anatomic outcome. MATERIAL AND METHODS: We reviewed the files of 87 patients who had undergone 93 supracondylar femoral osteotomies for knee flexion contracture (6 bilateral cases);

Vascular injuries in compound fractures of the leg with initially adequate circulation.

We carried out a randomised, controlled trial in 157 patients who had isolated severe Gustilo type-IIIa and type-IIIb fractures of the metaphysis or diaphysis of the leg to determine the prevalence of vascular injuries and the role of vascular repair. All patients had stable vital signs and clinically adequate circulation in their legs before operation. In a control group of 64 patients we performed conventional surgery with systematic debridement and primary stabilisation of the fractures. In the trial group of 93 patients the major vessels and nerves adjoining the compound fracture were rout

External fixation of tibial pilon fractures and fracture healing.

Distal tibial fractures are rare and difficult to treat because the bones are subcutaneous. External fixation is commonly used, but the method often results in delayed union. The aim of the present study was to find out the factors that affect fracture union in tibial pilon fractures. For this purpose, prospective data collection of tibial pilon fractures was carried out in 1998-2004, resulting in 159 fractures, of which 83 were treated with external fixation. Additionally, 23 open tibial fractures with significant > 3 cm bone defect that were treated with a staged method in 2000-2004 were ret

Are we underestimating pathological fracture risk in malignant bone lesions of the proximal humerus?

OBJECTIVE: This study is aimed at evaluating the distribution of metastatic bone disease (MBD), with a particular focus on the humerus, and its association with pathological fractures. Factors for contributing to the underestimation of fracture risk were assessed, including their impact on surgical management. MATERIALS AND METHODS: We retrospectively reviewed patient records of patients undergoing surgical treatment for MBD at our institution between 2005 and 2023. The analysis included factors such as medical history, tumour type, metastatic status, surgical method, lesion location, and ima

Predicting Pathological Fractures at Metastatic Humeral Lesions.

UNLABELLED: The humerus is the second most common site of metastatic disease involving long bones, yet it is still unclear which patients are at high risk for a fracture and may require prophylactic surgical fixation. The goal of this study was to assess the validity of the Mirels score to predict fractures of metastatic lesions in the humerus. METHODS: We performed a retrospective electronic chart review of patients with humeral metastases at our institution (2005 to 2021), with 188 patients meeting the inclusion criteria. Sixty-one of the patients developed a fracture during follow-up. The

Intramedullary Nailing Versus Modular Megaprosthesis in Pathological Humeral Fractures: A 10-Year Retrospective Comparative Study.

BACKGROUND: Pathological fractures of the humerus secondary to metastatic disease represent a significant cause of pain, disability, and reduced quality of life in oncologic patients. Surgical management aims to restore stability, reduce pain, and allow early mobilization. However, the optimal strategy between intramedullary nailing and modular megaprosthesis remains debated, particularly in relation to functional outcomes and long-term results. METHODS: A retrospective observational study was conducted on 48 patients treated for pathological or impending humeral fractures between January 201

Biomechanical testing of the LCP--how can stability in locked internal fixators be controlled?

New plating techniques, such as non-contact plates, have been introduced in acknowledgment of the importance of biological factors in internal fixation. Knowledge of the fixation stability provided by these new plates is very limited and clarification is still necessary to determine how the mechanical stability, e.g. fracture motion, and the risk of implant failure can best be controlled. The results of a study based on in vitro experiments with composite bone cylinders and finite element analysis using the Locking Compression Plate (LCP) for diaphyseal fractures are presented and recommendati

Importance of Whole-Body Scintigraphy or Positron Emission Tomography for Early Detection of Femoral Lesions in Breast Cancer Patients Treated with Bisphosphonates.

BackgroundWe hypothesized that most of the atypical femoral fractures (AFFs) associated with bisphosphonate treatment for breast cancer (BC) could be found before the fracture event in another radiological examination already performed by breast surgeons, rather than on simple radiographs (SRs).MethodsWe thoroughly inspected the clinical charts of BC patients treated at our institute between 2008 and 2017. In total, 228 patients were categorized into three groups based on SRs: complete AFF on at least one side (group 1); incomplete fracture on at least one side, but not any complete fracture (
