Internal fixation of osteopenic acetabular fractures involving the quadrilateral plate.

BACKGROUND: Older patients tend to have acetabular fractures with medial displacement patterns and associated comminution, particularly of the quadrilateral surface. Our goal was to investigate the appropriateness of open reduction and internal fixation using an infra-pectineal buttress plate for osteopenic acetabular fractures. MATERIALS AND METHOD: We conducted a retrospective review involving twenty one consecutive patients over the course of 4 years with an acetabular fracture in an academic level 1 trauma centre. We performed the modified Stoppa approach with buttress plating of the quad

[Distal intraarticular humerus fractures in elderly patients. Treatment with combined percutaneous screw fixation and an external fixator].

BACKGROUND: Intra-articular distal humerus fractures are relatively uncommon. Due to osteoporosis in elderly patients, stable fixation still remains a problem despite new implants providing angular stability. MATERIAL AND METHODS: The treatment of choice is open reduction and plate fixation requiring an extended approach with olecranon osteotomy often combined with additional postoperative immobilisation in a long arm cast. We describe a technique using closed reduction and percutaneous osteosynthesis with cannulated screws combined with external fixation. Morbidity due to surgical exposure a

Humeral shaft reconstruction surgery following military combat gunshot injury using a titanium spinal mesh cage - A case report.

INTRODUCTION: High-velocity gunshot injuries to the upper extremity are rare but devastating, particularly in military personnel, and are associated with high risks of infection, instability, and limb loss. Reconstruction of extensive segmental bone defects in such cases is a complex surgical challenge. CASE PRESENTATION: We report a challenging case of a 32-year-old Indian Air Force soldier who sustained a Grade 3B open right humeral shaft fracture from a gunshot wound during military combat. Initial management included wound debridement, skin grafting, and external fixation at military hosp

Diagnostic imaging findings in lame Warmblood horses with bone injuries of the medial proximal phalanx glenoid cavity.

This retrospective study aims to describe baseline and follow-up imaging findings in subchondral and trabecular bone damage occurring outside of the sagittal groove in the proximal phalanx (P1) glenoid in a case series of lame Warmblood horses. Thirteen lame horses (16 forelimbs) with standing magnetic resonance imaging (sMRI) evidence of nonsagittal groove glenoid subchondral and trabecular bone injury of P1 as the main lesion were included. All injuries were located at the medial aspect of the P1 glenoid. At sMRI, changes included subchondral bone plate thickening and trabecular sclerosis, b

Clavicle nonunion and plate breakage after locking compression plate fixation of displaced midshaft clavicular fractures.

Open reduction and plate fixation have been widely used for the treatment of displaced midshaft clavicular fractures (DMCF). The nonunion rate after plate fixation of DMCF has been reported to be between 0.1 and 15% and the construct failure rate is approximately 5%. Few studies have discussed the risk factors of construct failure. The aim of the present study was to identify possible risk factors of construct failure in plate fixation of DMCF and discuss the subsequent treatment strategies. Six patients who experienced plate breakage and clavicle nonunion between 2015 and 2017 were evaluated.

Comparison of radiographic appearance and bone scintigraphy in fracture nonunions.

Many surgeons assess the biological activity of fracture nonunions by the presence or absence of callus on radiographs. However, the assessment of biological activity by radiographic appearance alone is controversial. Bone scintigraphy reflects blood flow and new bone formation; therefore, it is useful in assessing such biological activity in nonunion cases. This retrospective study compared radiographs with Tc-99m bone scintigraphy in 48 patients with uninfected nonunions. Positive uptake was observed in all cases. The uptake patterns were classified into 4 types: type 1, intense, uniform upt

Primary treatment of complex proximal humerus fractures using Humelock cementless reversible shoulder arthroplasty in the elderly.

BACKGROUND: Proximal humerus fractures are quite common, constituting 5% of all fractures. Plate osteosynthesis of comminuted fractures in the elderly with osteoporotic bones is prone to complications, including loss of reduction, intraarticular protrusion of screws, avascular necrosis and non-union. Hemiarthroplasty may be preferred to achieve a stable fixation, which permits early shoulder motion. Prerequisites for the successful functional outcome of this surgical technique are to have an intact rotator cuff, which is often torn, and achieve proper soft tissue balance, which is technically

Circular frame management of distal tibial fractures; 15 years of practice in a tertiary referral unit.

INTRODUCTION: Distal tibial fractures, although relatively uncommon, are a concern to orthopaedic surgeons owing to their high energy nature and the associated soft tissue damage, metaphyseal comminution and articular involvement. Presently, there is no robust evidence to guide operative management, although circular frame fixation is one technique that can address and obviate some of these problems. METHODS: A retrospective review was undertaken to identify patients who had underwent circular frame fixation (CFF) of a distal tibia fracture in a UK major trauma centre from Jan 2010 - Dec 2023

Effects of trochlear fragmentation on functional outcome in coronal shear fractures: a retrospective comparative study.

BACKGROUND: Coronal shear fractures of the distal humerus are rare injuries, and fragmentation of the capitellum and trochlea with posterior comminution is challenging for surgeons. We retrospectively evaluated the functional outcomes of patients with coronal shear fractures managed with open reduction and internal fixation, focusing on the number of trochlea fragments in Dubberley type 3B fractures. MATERIALS AND METHODS: The functional outcomes of 25 patients, including 8 patients with type 3B fractures, with a mean age (and standard deviation) of 57 ± 20 years, were evaluated at a mean fol

Use of non-vascularized autologous fibula strut graft in the treatment of segmental bone loss.

BACKGROUND: Fractures resulting in segmental bone loss challenge the orthopedic surgeon. Orthopedic surgeons in developed countries have the option of choosing vascularized bone transfers, bone transport, allogenic bone grafts, bone graft substitutes and several other means to treat such conditions. In developing countries where such facilities or expertise may not be readily available, the surgeon has to rely on other techniques of treatment. Non-vascularized fibula strut graft and cancellous bone grafting provides a reliable means of treating such conditions in developing countries. MATERIA
