Atypical Clinical Manifestations of Cerebral Amyloid Angiopathy.

PURPOSE: Cerebral amyloid angiopathy is a vasculopathy caused by β-amyloid deposition in cerebral arterioles and capillaries. It is closely linked to Alzheimer's disease and predisposes elderly patients to intracerebral hemorrhage, transient focal neurological episodes, and cognitive impairment. Because of a predilection for symptomatic hemorrhage, particularly in the frontal lobes, cerebral amyloid angiopathy may also cause a dysexecutive syndrome. RECENT FINDINGS: In this case series, we describe presentations of classic clinical dementia syndromes which are not are widely thought to be ass

Multi-atlas segmentation of the hydrocephalus brain using an adaptive ventricle atlas.

Normal pressure hydrocephalus (NPH) is a brain disorder caused by disruption of the flow of cerebrospinal fluid (CSF). The dementia-like symptoms of NPH are often mistakenly attributed to Alzheimer's disease. However, if correctly diagnosed, NPH patients can potentially be treated and their symptoms reversed through surgery. Observing the dilated ventricles through magnetic resonance imaging (MRI) is one element in diagnosing NPH. Diagnostic accuracy therefore benefits from accurate, automatic parcellation of the ventricular system into its sub-compartments. We present an improvement to a whol

Young-onset dementia.

Young-onset dementia is a neurologic syndrome that affects behavior and cognition of patients younger than 65 years of age. Although frequently misdiagnosed, a systematic approach, reliant upon attainment of a detailed medical history, a collateral history, neuropsychological testing, laboratory studies, and neuroimaging, may facilitate earlier and more accurate diagnosis with subsequent intervention. The differential diagnosis of young-onset dementia is extensive and includes early-onset forms of adult neurodegenerative conditions including Alzheimer's disease, vascular dementia, frontotempor

Acute cerebellar ataxia, acute cerebellitis, and opsoclonus-myoclonus syndrome.

Acute cerebellar ataxia and acute cerebellitis represent a process characterized by parainfectious, postinfectious, or postvaccination cerebellar inflammation. There is considerable overlap between these entities. The mildest cases of acute cerebellar ataxia represent a benign condition that is characterized by acute truncal and gait ataxia, variably with appendicular ataxia, nystagmus, dysarthria, and hypotonia. It occurs mostly in young children, presents abruptly, and recovers over weeks. Neuroimaging is normal. Severe cases of cerebellitis represent the other end of the spectrum, presentin

Pathogenesis of cerebellar deformity in experimental Chiari type I malformation caused by mumps virus.

We sought to elucidate the pathogenesis of Chiari type I malformation using an experimental model of hydrocephalus produced by inoculating hamsters with mumps virus. Dilatation of the lateral ventricules was detected in all brains inoculated at 2, 10, and 25 days of age. The cerebellum in hamsters inoculated at 2 and 10 days of age showed elongation and flattening of the vermis and protrusion or notching of the uvula. All layers, i.e., the molecular, Purkinje cell, and granular layers, and the white matter were preserved, but had become narrow. Purkinje cells remained normal. Hamsters inoculat

Clinical criteria for the diagnosis of vascular dementia.

The clinical diagnosis of dementia includes medical history, neurological examination, psychiatric interview and dementia scale. The identification of conditions producing dementia can only be achieved by adding to the clinical information the data gathered from ancillary investigations. The usual ancillary diagnostic investigations (biochemical tests, cerebrospinal fluid (CSF), EEG, CT, MRI, angiography) can rather easily identify brain disorders due to tumors, vascular malformations, hematomas, infections, toxins and drugs, deficiency diseases, normal-pressure hydrocephalus, metabolic and en

[Surgical experience of aged patients with ruptured intracranial aneurysms (author's transl)].

Outcome of 77 cases with ruptured intracranial aneurysms above 60 years of age was reported. 44 cases were treated conservatively and 33 cases surgically. 1. Among conservatively treated group, 10 cases died after initial hemorrhage and 14 cases after second hemorrhage. The mortality rate was 55%. 2. Among surgically treated group, 11 cases died and 4 cases had severe neurological deficits following the direct intracranial operation. The mortality and morbidity rate was 33% and 12% respectively. 3. These results indicate that the direct intracranial operation should be the first choice of trea

Pediatric idiopathic intracranial hypertension and the underlying endocrine-metabolic dysfunction: a pilot study.

AIM: To unravel the potential idiopathic intracranial hypertension (IIH) endocrine-metabolic comorbidities by studying the natural (and targeted drug-modified) history of disease in children. IIH is a disorder of unclear pathophysiology, characterized by raised intracranial pressure without hydrocephalus or space-occupying lesion coupled with normal cerebrospinal fluid (CSF) composition. METHODS: Retrospective study (years 2001-2010) of clinical records and images and prospective follow-up (years 2010-2013) in 15 children (11 girls, 4 boys; aged 5-16 years) diagnosed previously as "IIH", acco

Increased perinatal intracranial pressure and prediction of early puberty in girls with myelomeningocele.

An increased risk of developing precocious puberty has been reported in children with myelomeningocele. In order to evaluate this further and to study factors associated with early or precocious puberty the medical records of all girls with myelomeningocele, born from 1970 onwards (n = 64), who were admitted to our unit were reviewed. Early/precocious puberty (E/PP) was defined as breast development or pubic hair corresponding to Tanner stage 2 before the age of 9.2 years. In 32 out of 62 cases data were sufficient for evaluation of the timing of puberty. Twenty girls had E/PP and 12 girls nor

Cerebrovascular physiology in perinates with congenital hydrocephalus.

PURPOSE: This study investigated changes in regional cerebral blood flow (rCBF), autoregulation (AR), and mean CO(2) reactivity (CO(2)r) in nine neonates, who underwent cerebrospinal fluid (CSF) diversion for congenital hydrocephalus. METHODS: During shunt insertion, a thermal diffusion probe inserted adjacent to the ventricular catheter in the right parietal region recorded rCBF. Changes in rCBF, mean arterial pressure, intracranial pressure (ICP), and expired CO(2) tension were recorded before and after removing CSF. RESULTS: Mean baseline rCBF for the entire group was 19.5 mL/100 g/min (r

Systemic Review: Neurological Deficits following Ventriculoperitoneal Shunt (VPS) Insertion.

A reduction in fluid absorption or an obstruction of normal outflow is a common cause of hydrocephalus. It typically requires medical attention, which frequently entails the placement of a ventriculoperitoneal shunt (VPS) to lower intracranial pressure. We intend to list the few, documented examples of neurological impairments resulting from the installation of a VPS in this systematic study. Two search engines (PubMed and Cochrane) were used to conduct a systematic review from 1975 to December 12, 2021. The following search terms were employed: neurological deficits or neurological injury or

Yield of early postoperative computed tomography after frontal ventriculoperitoneal shunt placement.

Despite being widely used, ventriculoperitoneal (VP) shunt placement is a procedure often associated with complications and revision surgeries. Many neurosurgical centers routinely perform early postoperative cranial computer tomography (CT) to detect postoperative complications (e.g., catheter malposition, postoperative bleed, over-drainage). Because guidelines are lacking, our study aimed to evaluate the yield of early routine postoperative CT after shunt placement for adult hydrocephalus. We retrospectively reviewed 173 patients who underwent frontal VP shunting for various neurosurgical co

A new method for the relative quantification of rCBF examined by 99Tcm-HMPAO SPECT.

A new method for the analysis of regional cerebral blood flow (rCBF) studies, the 'Min-Max' method, was compared with the conventional method, the 'Average' method. Regional CBF was examined by single photon emission computed tomography (SPECT) using 99Tcm-hexamethylpropyleneamine oxime (99Tcm-HMPAO). The two methods were used to quantify the rCBF in a population of patients with various dementia disorders, and in healthy controls. Results from a phantom were also used. In the Average method the average counts per pixel (cp-1) within irregular manually drawn regions of interest (ROIs) was regi

[Cisternography with 111Indium-DTPA (author's transl)].

The experiences of more than 200 cisternographic examination with 111Indium-DTPA are reported. The radiopharmaceutical was tolerated without adverse reactions and the suboccipital intrathecal injection more appropriate than the lumbar. With 111In-DTPA excellent scans were obtained up to 48 h after application. 111In is compared with other radiopharmaceuticals for cisternography. The cisternography remains valuable as an additional examination of pneumencephalography and cranial computerized tomography.

[Gastric perforation in ventriculo-peritoneal shunt--a case report (author's transl)].

The case is a 69-year-old female who was admitted to our hospital under the diagnosis of normal pressure hydrocephalus secondary to cerebral infarction. V-P shunt was performed and postoperative course was uneventful until 7 months after operation, when she developed meningitis. Since then the level of consciousness became gradually worse down to akinetic mutism. Two years and 3 months after the operation she died of pneumonia. At autopsy peritoneal end of the shunt tube was found to have perforated the antero-inferior wall of the stomach. In this report, we discussed the mechanism of developm

[Continuous negative pressure (cnp) in infants with idiopathic respiratory distress syndrome (irds) (author's transl)].

13 unselected premature infants with idiopathic respiratory distress syndrome (IRDS) have been treated with continuous negative pressure (CNP). A chamber was applied to the infants' body, so that the head remained outside in normal atmospheric pressure. 4 infants died during the period of treatment. 6 out of 13 differently treated IRDS-infants died over the same 6 months period. In comparison 18 out of 27 infants with IRDS died the previous year, during the same period. The frequency of IRDS in both observation periods was the same, as was the therapy of acidosis and hypoxia. The infants respo

Invention of a non-invasive intracranial pressure (ICP) monitoring system - an enlightenment from a hydrocephalus study.

BACKGROUND: Mechanical obstruction is the most common cause of shunt failure for hydrocephalic patients. However, the diagnosis is extremely challenging and often requires invasive testing methods. Thus, a simple and non-invasive technique is in urgent need to predict the intracranial pressure (ICP) of hydrocephalic patients during their post-surgical follow-up, which could help neurosurgeons to determine the conditions of the shunt system. MATERIALS AND METHODS: Two groups of patients were enrolled in the current study. In group I, patients were enrolled as they were diagnosed with high ICP

Incidence and causes of nondegenerative nonvascular dementia: a population-based study.

BACKGROUND: Information on the incidence of nondegenerative and nonvascular dementia is limited. DESIGN: We used the records-linkage system of the Rochester Epidemiology Project to ascertain incident cases of dementia in Rochester, Minn, from January 1, 1990, through December 31, 1994. To define causes of dementia, we reviewed all diagnoses, imaging study results, laboratory test results, and clinical courses, as recorded historically in the patient dossier. RESULTS: We found 560 incident cases of dementia, and 60 of them (10.7%) had onset before the age of 70 years (younger-onset group). Fo

Adult dementia: history, biopsy, pathology.

The historical events in the evolution of Alzheimer's disease are reviewed, including the initial description by Alois Alzheimer and the subsequent controversy regarding the nosological specificity of this entity. The similarity of senile dementia and Alzheimer's disease is emphasized. The basis for the modern concept of Alzheimer's disease as premature or accelerated aging is included in the review. The pathological correlates of the major categories of adult dementia have been described. The traditional criteria of neurofibrillary tangles and senile plaques have been re-evaluated using the c
