Maternal and neonatal outcomes and histological chorioamnionitis in preterm premature rupture of membranes complicated by group B streptococcus colonization: a retrospective analysis.

BACKGROUND: Histological chorioamnionitis (HCA) is frequently identified in placental examinations following preterm premature rupture of membranes (PPROM). Colonization of the genital tract by Group B Streptococcus (GBS) represents a major risk factor for serious neonatal infections. This study evaluated the influence of GBS on both maternal and neonatal outcomes in pregnancies complicated by PPROM before 34 weeks’ gestation managed expectantly. METHODS: We conducted a retrospective analysis of clinical records from pregnant women experiencing spontaneous PPROM between 23 + 0 and 33 + 6 weeks

Safety and immunogenicity of a booster dose of a novel hexavalent group B streptococcus conjugate vaccine in healthy, non-pregnant adults: a phase 2, open-label extension of a phase 1/2 randomised controlled trial.

BACKGROUND: A hexavalent group B streptococcus (GBS) polysaccharide conjugate vaccine (GBS6) is being developed to prevent invasive group B streptococcal disease in infants through active maternal immunisation. We previously reported results for GBS6 administered to non-pregnant adults (NCT03170609). Here, we aim to extend these results to determine safety and immunogenicity of a second dose of GBS6 in non-pregnant adults who received a primary dose of GBS6. METHODS: This phase 2, open-label extension of a phase 1/2 randomised controlled trial was done at four sites in the USA to assess the s

Immunogenicity and safety of a group B Streptococcus vaccine (GBS-AlpN) in pregnant women and their infants: a phase 2, multicentre, observer-blind, randomised, placebo-controlled study.

BACKGROUND: GBS-AlpN is a novel group B streptococcus maternal vaccine, based on the N-terminal domains of the alpha-like proteins AlphaCN, RibN, Alp1N, and Alp2/3N, which are responsible for 99% of invasive group B streptococcus strains. We aimed to assess the safety and immunogenicity of GBS-AlpN in pregnant women receiving vaccination or placebo. METHODS: This phase 2, observer-blind, randomised, placebo-controlled study was done in ten health-care facilities in South Africa, Denmark, and the UK. Eligible participants were healthy women carrying a singleton pregnancy without detectable con

Prevalence of Group B Streptococcus in pregnant women in Kerala and relation to neonatal outcomes: a prospective cross-sectional study.

BACKGROUND AND OBJECTIVES: Early onset sepsis (EOS) in neonates is a scourge that contributes to morbidity and mortality. Prominent stakeholders recommend universal screening of antenatal women for Group B Streptococcus (GBS) and intrapartum antibiotic prophylaxis (IAP) for those who are carriers. However, there are controversies. Other guidelines allow region-specific protocols due to sociodemographic, geographical and ethnic differences. We planned to analyze the prevalence of GBS rectovaginal carriage at 36-37 weeks gestation and its effect on early neonatal status. METHODS: This prospecti

Prevention of Group B Streptococcal Early-Onset Disease in Newborns: ACOG Committee Opinion Summary, Number 797.

Group B streptococcus (GBS) is the leading cause of newborn infection. The primary risk factor for neonatal GBS early-onset disease (EOD) is maternal colonization of the genitourinary and gastrointestinal tracts. Approximately 50% of women who are colonized with GBS will transmit the bacteria to their newborns. Vertical transmission usually occurs during labor or after rupture of membranes. In the absence of intrapartum antibiotic prophylaxis, 1-2% of those newborns will develop GBS EOD. Other risk factors include gestational age of less than 37 weeks, very low birth weight, prolonged rupture

Molecular Characterization of Nonhemolytic and Nonpigmented Group B Streptococci Responsible for Human Invasive Infections.

Group B Streptococcus (GBS) is a common commensal bacterium in adults, but is also the leading cause of invasive bacterial infections in neonates in developed countries. The β-hemolysin/cytolysin (β-h/c), which is always associated with the production of an orange-to-red pigment, is a major virulence factor that is also used for GBS diagnosis. A collection of 1,776 independent clinical GBS strains isolated in France between 2006 and 2013 was evaluated on specific medium for β-h/c activity and pigment production. The genomic sequences of nonhemolytic and nonpigmented (NH/NP) strains were analyz
