Quadriceps Tendon Autograft Anterior Cruciate Ligament (ACL) Reconstruction With Internal Brace Augmentation: A Case Series.

INTRODUCTION: Anterior cruciate ligament reconstruction (ACLR) is among the most common knee procedures in the United States, particularly in athletes who participate in cutting and pivoting sports. While bone-patellar tendon-bone (BTB) autografts have long been considered the gold standard for this group, drawbacks such as anterior knee pain and difficulty kneeling have prompted interest in quadriceps tendon autografts. With a larger cross-sectional area and higher load-to-failure, the quadriceps autograft is a promising alternative. This study reported return-to-play timelines and re-rupture

Effectiveness of Occupational Therapy Interventions for Adults With Musculoskeletal Conditions of the Forearm, Wrist, and Hand: A Systematic Review.

Occupational therapy practitioners are key health care providers for people with musculoskeletal disorders of the distal upper extremity. It is imperative that practitioners understand the most effective and efficient means for remediating impairments and supporting clients in progressing to independence in purposeful occupations. This systematic review provides an update to a previous review by summarizing articles published between 2006 and July 2014 related to the focused question, What is the evidence for the effect of occupational therapy interventions on functional outcomes for adults wi

Cost analysis of brachial plexus injuries: variability of compensation by insurance companies before and after surgery.

Traumatic paralysis of the brachial plexus is an extremely disabling pathology. The type of trauma most frequently suffered by this group of patients is due to motorcycle injuries. It therefore affects a population of young patients. In the majority of cases, these patients receive compensation for permanent damage from insurance companies. Surgery of the brachial plexus enables various forms of functional recovery, depending on the number of roots of the brachial plexus involved in the injury. The aim of this study is to compare the functional deficit and the extent of the related compensatio

[Transosseous refixation of the common extensor muscle tendons in chronic lateral epicondylitis with and without additional reconstruction of the LUCL--a retrospective evaluation of 101 patients].

INTRODUCTION: The chronic lateral epicondylitis (EHR) as a common pathology of the elbow is often associated with posterolateral rotatory instability of the elbow (PLRI). After evaluation of intra-articular pathology by prior diagnostic arthroscopy, we aimed to build patient groups regarding the stability of the elbow joint. In patients with a stable elbow joint, open surgery with a debridement to the origin of the common extensor tendon and transosseous refixation was performed. Patients with relevant posterolateral rotatory instability, however, underwent an additional LUCL complex stabilisa

[Patellar fracture after patellar stabilization].

We report on a 20-year-old female patient with a fracture of the patella after she fell on"black ice" 2 months after medial patellofemoral ligament (MPFL) reconstruction for patellar instability. For reconstruction of the MPFL, a single hamstring tendon graft was passed through the medial intermuscular septum and was fixed to the superomedial pole of the patella. The fracture was reduced by wire cerclage. Intraoperatively it was shown that the fracture line went through the patellar drill hole. No complications occurred during the further postoperative period. The literature contains only a fe

Medial patellofemoral ligament reconstruction fixed with a cylindrical bone plug and a grafted semitendinosus tendon at the original femoral site for recurrent patellar dislocation.

BACKGROUND: The medial patellofemoral ligament (MPFL) is the most important factor for stabilizing the patella and preventing lateral patellar dislocation. Medial patellofemoral ligament reconstruction is an accepted surgical technique to restore patellofemoral stability after lateral patellar dislocation. The authors recently developed a new anatomical MPFL reconstruction method using a cylindrical bone plug and grafted semitendinosus tendon at the anatomical femoral attachment site to mimic the native MPFL. This study evaluated the new technique for stabilizing recurrent patellar dislocation

Recovery of brachial plexus injury after shoulder dislocation.

Brachial plexus injury is an underestimated complication from anterior dislocation of the shoulder. To our knowledge, there is limited information available about the factors that influence neurological recovery of this injury. We reviewed 15 upper extremities in 14 patients with brachial plexus injuries caused by anterior shoulder dislocation. Two-thirds of the cases had total brachial plexus palsy. With the conservative treatment, the motor recoveries of all cases are full or nearly full within 20 months except intrinsic muscle of the hand. Intrinsic muscle recovery may be better in a younge

Anatomical basis of latissimus dorsi and teres major transfers in rotator cuff tear surgery with particular reference to the neurovascular pedicles.

Musculotendinous transfers (MTT) of latissimus dorsi (LD) and teres major (TM), either in isolation or combination, have recently been advocated to treat irreparable rotator cuff tears. The purpose of this study was to (1) review the anatomy of the LD and TM neurovascular pedicles, and (2) undertake experimental MTT to humeral insertions of either supraspinatus or infraspinatus to evaluate tension on their nutrient arteries in six positions of the arm. Twenty-six shoulders were studied, 22 of which had previously been injected with red latex. Gross dissection, using 4.3 x magnification when ne

Regional anesthesia in knee arthroscopy: a new technique involving femoral and sciatic nerve blocks in knee arthroscopy.

We describe a new anesthetic technique of femoral and sciatic nerve blocks in knee arthroscopy. The sciatic nerve block is administered through a posterior approach, and the block of the femoral and lateral femoral cutaneous nerves in a single anterior approach. This type of regional anesthesia is safe and effective, providing excellent intraoperative and postoperative analgesia as well as minimizing postoperative complications.

Reanimation of the brow and eye in facial paralysis: Review of the literature and personal algorithmic approach.

Facial palsy patients suffer an array of problems ranging from functional to psychological issues. With regard to the eye, lacrimation, lagophthalmos and the inability to spontaneously blink are the main symptoms and if left untreated can compromise the cornea and vision. There are a multitude of treatment modalities available and the surgeon has the challenging prospect of choosing the correct intervention to yield the best outcome for a patient. The accurate assessment of the eye in facial paralysis is described and by approaching the brow and the eye separately the treatment options and ind

[Filling a humeral head defect with a chondrocostal autograft combined with subscapularis transfer for chronic posterior glenohumeral dislocations].

Mac Lauglhin defects of the right humeral head developed in a 44-year-old woman with drug-resistant epileptic seizures, subsequent to recurrent posterior dislocations. The preoperative work-up demonstrated a large anterior defect corresponding to more than 50% loss of the osterochondral tissue. The humeral head lodged on the posterior rim of the scapular glenoid, like an inveterate posterior dislocation, causing major pain and joint dysfunction. After adjustment of the antiepilepsy treatment, the patient underwent joint reconstruction with a chondrocostal autograft harvested from the ninth hom

Early versus late start of open kinetic chain quadriceps exercises after ACL reconstruction with patellar tendon or hamstring grafts: a prospective randomized outcome study.

The purpose of the present investigation was to evaluate physical outcome after anterior cruciate ligament (ACL) reconstruction with early versus late initiation of open kinetic chain (OKC) exercises for the quadriceps in patients operated on either patellar tendon or hamstring grafts. Sixty-eight patients, 36 males and 32 females, with either patellar tendon graft (34 patients) or hamstring graft (34 patients) were enrolled in this study. All patients were randomly allocated to either early (the 4th postoperative week) or late (the 12th postoperative week) start of OKC exercises for the quadr

Eden-Hybinette Procedure as Salvage Treatment for Failed Latarjet Surgery: A Case Report.

INTRODUCTION: Anterior shoulder instability commonly affects young, active individuals and is often associated with traumatic dislocations and glenoid bone loss. The Latarjet procedure is widely performed in cases with significant bone loss, providing stability through coracoid transfer, the sling effect of the conjoint tendon, and capsulolabral repair. Despite its success, complications such as malpositioned bone block, hardware failure, graft resorption or non-union, and patient-related factors like laxity or inadequate rehabilitation may lead to recurrence. Revision surgery after failed Lat

Augmentation of hip abductor repair with fascia of vastus lateralis.

BACKGROUND: Hip abductor tears are common and can cause severe limp and pain. Repair of gluteal tears often requires augmentation. Although current methods of gluteal augmentation have good reported outcomes, they can be costly and add an additional layer of complexity if they become infected because they rely on non-autologous materials. Based on the case series below, we report an alternative technique to augment gluteal tendon repair that uses vastus lateralis fascia. METHODS: Between 2022 and 2025, 23 hip abductor repairs augmented with a vastus lateralis fascia flap, following a primary
