Do vasoactive neuropeptide autoimmune disorders explain pyridostigmine's association with Gulf War syndrome?

Gulf War syndrome (GWS) is a perplexing multi-symptom condition comprising a constellation of signs and symptoms consistently described in t … icotinic (e.g., MG) acetylcholine neurotransmission. Hence theoretically, treatment options such as thymectomy, corticosteroids, plasma exchange, anti-idiotype antibodies and receptor genomic expression reactivation/suppression may be considered. Paradoxically pyridostigmine may prove to have a role in therapy although VN treatment/replacement may be associa …

Clinical characteristics and prognosis of myasthenia gravis in older people.

OBJECTIVES: To investigate the characteristics of myasthenia gravis (MG) in older people and to evaluate the benefits of immunosuppressive treatments at this age. BACKGROUND: Myasthenia gravis in older adults has not been extensively studied. In patients with disease onset after the age of 60, treatment mainly relies on medical therapy because thymectomy is generally not performed unless a thymoma is present. METHODS: Of 837 myasthenic patients followed since 1978, we identified 172 cases with onset after age 60. All patients were treated with anticholinesterases. In the decade from 1978 to …

Prevalence and clinical aspects of immigrants with myasthenia gravis in northern Europe.

INTRODUCTION: Multiethnic studies can provide etiological clues toward the genetic and environmental influence of a disease. The aim of this study was to determine prevalence and clinical features of myasthenia gravis (MG) in immigrants compared with native patients in 2 population-based cohorts. METHODS: This cross-sectional study included 843 MG patients (375 from Norway and 468 from the Netherlands). Ethnic background was defined by questionnaires. RESULTS: Among the participating MG patients, 163 of 843 (19.3%) were first or second generation immigrants, mainly from Europe, Asia, and Sou

Thymoma-associated immunodeficiency: a diagnostic challenge for the clinician.

Good's syndrome or thymoma-associated immunodeficiency is a rare clinical entity that is often presumed to be common variable immunodeficiency, due to lack of awareness and recognition of this syndrome. This syndrome more often goes unrecognized if a thymoma is not detected. An appropriate immunological work-up that aids timely diagnosis and adequate therapy with antimicrobials and intravenous immunoglobulins are mandatory to prevent the long-term complications and mortality associated with this syndrome. We present the clinical and immunological profile of a young man with Good's syndrome tha

Ectopic thymic carcinoma presenting as an intrathoracic mass.

An asymptomatic 83-year-old man was found to have a right intrathoracic tumor. Computed tomography demonstrated a soft-tissue density mass measuring 55 × 25 × 22 mm adjacent to the right anterior chest wall. At surgery, the tumor was found to adhere to the diaphragm and right lung, contiguous with the mediastinal fat tissue. Histology of the resected specimen demonstrated proliferation of spindle and sarcomatous cells with multinucleated giant cells. Thus the tumor was diagnosed as undifferentiated thymic carcinoma and was considered to have arisen from ectopic thymic tissue. At 2 years postop

Closed trauma, Mycoplasma hominis osteomyelitis, and the elusive diagnosis of Good's syndrome.

Mycoplasma hominis septic arthritis has a well-established association with hypogammaglobulinaemia, but is rarely seen in immunocompetent hosts. An association also exists with a closed trauma and a predisposition to M hominis bone infections. In this clinical case report, a patient with M hominis osteomyelitis following a closed trauma was diagnosed using 16S ribosomal studies, and led to the diagnosis of a severe underlying immunodeficiency syndrome known as Good's syndrome.

Thymic clear cell carcinoma.

We report a 42-year-old man with a rare thymic clear cell carcinoma. Marked nodal metastases involved right hilar, mediastinal, and left supraclavicular regions. Complete resection including thymothymectomy, cervicomediastinal nodal dissection, and right upper lobectomy with hilar lymphadenectomy was successful. Postoperative chemoradiation therapy was uneventful. The patient had no recurrence or metastasis until brain metastasis occurred 1 year after surgery.

Neuroendocrine carcinoma of the thymus gland with sternal invasion.

Large-cell neuroendocrine carcinoma of the thymus is a rare cancer that is more aggressive than other thymic epithelial tumors with poorer prognosis. We report one such case in a 44-year-old male patient with invasion into the upper 2/3rd of the sternum but no endocrinopathy. Complete enbloc resection of the tumor with adherent pericardium, upper 2/3rd of the sternum and mediastinal lymphadenectomy was performed. The chest wall was reconstructed with polypropylene mesh and bilateral pectoralis major muscle advancement flap cover. A search of PubMed database with user query words 'neuroendocrin

Multimodality treatments in locally advanced stage thymomas.

BACKGROUND AND OBJECTIVES: Complete surgical resection is the main goal in the treatment of thymoma, but is not always achievable in stage III and IVA thymoma because of local invasion of the neighboring organs or the presence of diffuse pleural or pericardial implants. We reviewed our experience in multimodality treatment of advanced stage (III and IVA) thymic tumors to evaluate the effectiveness and safety in patients with locally advanced unresectable thymoma. PATIENTS AND METHODS: We studied patients with newly diagnosed, histologically proven, unresectable malignant thymoma who underwent

Surgical outcome in thymic tumors with myasthenia gravis after plasmapheresis--a comparative study.

Plasmapheresis has been used widely in the treatment of myasthenia gravis and also in symptomatic thymectomized patients with short-term clinical improvement. But the utility of preoperative plasmapheresis in the outcome has not been widely studied. The authors analyzed its impact in the surgical outcome of thymic tumors with myasthenia gravis. We studied a total of 19 patients, who were operated on in the period from January 2000 to July 2006 for thymic tumors with myasthenia gravis. Of these 19 patients, preoperative plasmapheresis was performed in 10 patients (group B) and the remaining nin

Early autoimmune complications after thymomectomy in a patient with interstitial lung disease. Case report.

Thymoma has been associated with a variety of autoimmune disorders. We report a case of myasthenia gravis and pancytopenia in a 53-year-old man with lymphoepithelial thymoma and interstitial lung disease. Preoperative examination revealed neither hematologic abnormality nor myasthenia gravis. The patient had enteritis prior to thymomectomy, sternal infection in the first month of operation, and urinary infection at the third month. About three months after thymomectomy, he required mechanical ventilation support due to myasthenia gravis-related respiratory failure. One month later, a rapidly p

Donor-specific tolerance in a murine model: the result of extra-thymic T cell deletion?

Previously, we established a murine model, that involves the engraftment of fully allogeneic T cell depleted donor bone marrow cells in sublethally irradiated and single dose anti-CD3 treated recipient mice. These mice developed permanent stable multilineage mixed chimerism and donor-specific tolerance without graft-versus-host disease. Recently, we have shown that donor-specific tolerance is not induced and/or maintained by clonal anergy, neither by a Th1/Th2 shift, nor by suppressor or other regulatory processes. In the present study, we investigated whether clonal deletion plays a role in t

[Surgical therapy of neuroendocrine tumors of the thymus].

Neuroendocrine tumors of the thymus, also known as thymic carcinoids, are rare tumors of the anterior mediastinum. They occur sporadically or in association with MEN I syndrome. We present five patients (four male, one female; age of first manifestation 19-53 years) who were operated on at our hospital between 1984 and 1995 for neuroendocrine thymic tumors. A hormone-producing tumor presented with Cushing's syndrome in two patients. Two patients had MEN I syndrome. Only in the female patient the primary tumor was confined to the thymus. We found lymph node metastases in three patients and a di
