Natural History of Residual Vestibular Schwannoma After Incomplete Microsurgical Resection: Delayed Progression During Long-Term Surveillance.

BACKGROUND: Deliberate incomplete resection of vestibular schwannoma (VS) is increasingly accepted to preserve cranial nerve function, but the long-term behaviour of remnants remains uncertain. We describe the natural history of residual VS during extended radiological surveillance. METHODS: Of 194 patients who underwent VS microsurgery at a single centre between 2013 and 2019, 68 had complete pre- and postoperative MRI and formed the study cohort; 22 (32%) had a residual tumour on baseline MRI and constituted the analysis population. Progression was defined as a ≥2-mm increase in maximum lin

Quantitative pupillometry as a marker of autonomic dysregulation in vestibular migraine.

BACKGROUND: Vestibular migraine (VM) has been associated with altered central sensory processing; however, objective measures of autonomic involvement remain insufficiently characterized. Quantitative pupillometry enables standardized assessment of both static and dynamic pupil responses. This study compared pupillary parameters between patients with VM and healthy controls under controlled illumination conditions. METHODS: Seventy participants were enrolled, including 40 patients diagnosed with VM according to the International Classification of Headache Disorders, 3rd edition (ICHD-3) crite

[A Patient with Early-Stage Multiple System Atrophy Showing Augmented Nystagmus in Light].

The ability to fix the eyes on a target, visual fixation, is important for the maintenance of equilibrium. The visual suppression (VS) test is one method of measuring the function of visual fixation. The test records caloric nystagmus by electrooculography, and the maximum slow phase velocity of caloric nystagmus in darkness is compared with the slow phase velocity in light with eyes fixed. Lesions of the cerebellum, brain stem, and cerebrum cause abnormalities of VS. We report a patient whose VS became a clue in the diagnosis of a disorder of the central nervous system. A 54-year-old man comp

Radiotherapeutic treatment of a fighter pilot with nasopharyngeal carcinoma.

BACKGROUND: Radiotherapy is the standard and most effective treatment for nasopharyngeal carcinoma (NPC) in its early stages. However, its application in fighter pilots returning to flying duties with NPC has not been previously reported, presumably due to post-radiotherapeutic complications. CASE REPORT: A 36-yr-old male fighter pilot had a painless mass in the left neck for 5 mo. Pathological diagnosis demonstrated nonkeratinizing squamous cell carcinoma in the left nasopharynx which had metastasized to lymph nodes in the left side of the neck. He was diagnosed and staged with NPC (T1N2M0)

Physiological and morphological assessment of the saccule in Guinea pigs after noise exposure.

OBJECTIVE: To investigate whether the saccule exhibits temporary or permanent functional loss resembling threshold shifts in auditory brainstem response (ABR) of guinea pigs following noise exposure. DESIGN: Randomly bred guinea pigs were divided into 3 groups: A (short-term noise exposure, 30 minutes, n = 15), B (long-term noise exposure, 40 hours, n = 9), and C (no noise exposure, n = 5). SETTING: University hospital. MAIN OUTCOME MEASURES: All animals underwent vestibular-evoked myogenic potential (VEMP) and ABR tests. Chronological changes of VEMP and ABR responses following noise expos

Sensorineural Hearing Loss in Patients With the m.1555A>G Mutation in the MTRNR1 Gene.

OBJECTIVE: Mutations in the MTRNR1 gene of mitochondrial DNA are associated with non-syndromic hearing loss and increased susceptibility to aminoglycoside ototoxicity. The aim of our study was to determine the clinical characteristics of sensorineural hearing loss caused by the m.1555A>G mutation in MTRNR1. METHODS: An observational retrospective study of the m.1555A>G mutation was conducted in patients with suspected hereditary bilateral sensorineural hearing loss in the Department of Otolaryngology of the Marqués de Valdecilla University Hospital (Cantabria, Spain) and in 100 controls with

Anorectal malformations in female children--10 years experience.

Despite enormous interest in the development of an ideal operative procedure to treat anorectal malformations, the situation is as confusing as it was 30 years ago. The reasons could be inconsistency of the nature of the lesions found in patients along with multi-system associated malformations primarily affecting the prognosis of available procedures. High imperforate anus is a complex anomaly that requires a combination of careful preservation of structures and precise anatomic reconstruction for optimal results. A retrospective study, comprising a consecutive sample of female children with

The Effect of Different Auditory Stimuli and Optokinetic Stimuli on Vestibulo-Ocular Reflex Function.

This study investigated how auditory stimuli and optokinetic stimulation modulate functional Head Impulse Test (fHIT) correct response rate (CR%) across semicircular canals (SCCs) and the associated cognitive load in healthy adults. Fifty participants (20-57 years) completed repeated-measures fHIT under four conditions: silence, music, white noise, and optokinetic stimulation. The CR% from all SCCs were recorded and workload was assessed with the NASA Task Load Index (NASA-TLX). Optokinetic stimulation produced the most significant CR% decrement (e.g., right lateral 90 vs. 100 in silence; p<0.

Sporadic pediatric vestibular schwannoma: a case report in a 4-year-old boy.

Sporadic vestibular schwannomas (VSs) are rare in children. When occurred in the pediatric population, they usually appear bilaterally and are related to neurofibromatosis type 2 (NF2). The current study reports a 4-year-old boy without family history of VS or NF2 who presented with a large (5.7-cm) VS involving the right cerebellopontine angle and internal auditory canal. Through seven-staged surgical interventions and two stereotactic γ‑knife radiosurgery, the disease was stabilized. At 2-year follow-up, the child had right ear hearing loss, grade IV facial palsy, and normal motor function a

The vestibulo-collic reflex.

Until now there has no been no simple, reliable clinical test that assesses the function of the otolith organs. However, recent work has suggested the occurrence of an easily identifiable electromyographic (EMG) waveform in isometrically contracting sternomastoid muscles in response to loud clicks. This is thought to represent an otolith evoked disynaptic reflex. Equipment routinely used for evoked response audiometry (ERA) has been adapted to investigate this response. A short latency (8 ms) positive-negative EMG waveform has been recorded in 31 of 32 normal subjects. The latency of the posit

Video head impulse testing in patients with isolated (hemi)nodular infarction.

BACKGROUND: Isolated (hemi)nodular strokes as underlying cause of acute dizziness are rare, thus there are still gaps of knowledge in the clinical presentation of affected patients. Clinical and experimental evidence has suggested that lesions involving the nodulus lead to various vestibulo-ocular deficits including prolonged velocity-storage, periodic-alternating nystagmus, positional nystagmus, abolished suppression of post-rotatory nystagmus by head-tilt and impaired verticality perception. At the bedside, the angular vestibulo-ocular reflex (aVOR), as assessed by the horizontal head-impuls

Panic disorder in otoneurological experience.

The evaluation of cochleovestibular dysfunction in psychiatric patients often causes a difficult problem in neurootological experience. The authors discuss here the neurobiological basis of panic disorders and cochleovestibular dysfunction. In this multicenter study, we examined 63 patients with vertigo and panic disease. Twenty patients with primary panic disease and consecutive vertigo composed group 1, whereas group 2 comprised 43 vertiginous patients with secondary panic disease. The most interesting question is whether the patients have an organic vestibular lesion, which would explain wh
