Surgery for small asymptomatic abdominal aortic aneurysms.

BACKGROUND: An abdominal aortic aneurysm (AAA) is an abnormal ballooning of the major abdominal artery. Some AAAs present as emergencies and require surgery; others remain asymptomatic. Treatment of asymptomatic AAAs depends on many factors, but the size of the aneurysm is important, as risk of rupture increases with aneurysm size. Large asymptomatic AAAs (greater than 5.5 cm in diameter) are usually repaired surgically; very small AAAs (less than 4.0 cm diameter) are monitored with ultrasonography. Debate continues over the roles of early repair versus surveillance with repair on subsequent e

Thematic characteristics and research hot-spots of China's healthy aging policy: topic modeling study.

BACKGROUND: China faces unprecedented aging challenges, with its population aged 60 + projected to exceed 310 million by 2024. To address this, China integrated “healthy aging” into national strategies like the Healthy China 2030 initiative. However, a systematic analysis of policy themes and research hot-spots was lacking. This study aims to fill this gap by examining China’s healthy aging policies and academic focus areas from 2016 to 2025. OBJECTIVE: To identify core thematic characteristics of China’s healthy aging policies and research hot-spots using topic modeling and content analysis,

Care of incontinence-associated dermatitis in older adults: A randomized controlled trial comparing zinc oxide-petroleum jelly and centella asiatica-aloe vera preparations.

BACKGROUND: Incontinence-associated dermatitis is common among older adults in intensive and semi-intensive care units. Impaired consciousness, incontinence, and limited self-care increase the risk of severe skin damage, negatively affecting physical, psychological, and social well-being. AIM: We compared incontinence-associated dermatitis risk, severity, skin surface pH (a measure of acidity/alkalinity), and skin moisture between the control and experimental groups. DESIGN: Double blind block-randomized controlled trial, two groups pre-posttest design. METHODS: Participants were randomly a

Quality of Trauma Meeting Presentations Before and After Introduction of a Structured Template at a Major Trauma Centre: A Before-and-After Quality Improvement Study.

BACKGROUND: Morning trauma meetings are central to the daily trauma and orthopaedic workflow in major trauma centres. When case presentations are inconsistent, important clinical details may be missed, clarification questions increase, decision-making becomes less efficient, and documentation of management plans can vary. Structured presentation tools, including acronym-based templates embedded in digital systems, may improve reliability while still allowing clinicians to use judgement. OBJECTIVE: The objective of this study is to evaluate whether introducing a structured trauma meeting prese

Impact of middle meningeal artery embolization on neurological complications and discharge outcomes in chronic subdural hematoma: A machine learning-based comparative study.

INTRODUCTION: Chronic subdural hematoma (cSDH) predominantly affects older adults, often those with prior head trauma, anticoagulation therapy, or chronic comorbidities. Traditional management involves surgical evacuation; however, middle meningeal artery (MMA) embolization has emerged as a less invasive alternative with the potential for fewer complications. This study compares outcomes of surgical evacuation, MMA embolization, and combination therapy using machine learning to identify predictors of neurological complications and discharge disposition. METHODS: Patients with cSDH from the 20

Randomised, sham-controlled, double-blinded, multicentre international trial to evaluate the efficacy of the Ventfree Respiratory Muscle Stimulator to assist ventilator weaning in critically ill patients: a study protocol of a randomised controlled trial.

INTRODUCTION: Nearly half of patients who receive invasive mechanical ventilation for acute respiratory failure require over 4 days of ventilator support, each day of which is associated with increased morbidity, mortality and cost. Many of these patients develop expiratory muscle atrophy and weakness, which are linked to failed extubation and weaning. We seek to test the hypothesis that exhalation synchronised abdominal functional electrical stimulation reduces mechanical ventilation duration. METHODS AND ANALYSIS: This pivotal superiority trial will be performed in up to 30 intensive care u

Reducing Inappropriate Surgical Referrals Through Patient Assessment Algorithm Utilising Early Imaging: A Closed-Loop Audit.

INTRODUCTION: Emergency department (ED) referrals to specialist services are integral to patient management within the National Health Service (NHS). However, rising pressures, the lack of standardised referral protocols, incomplete assessments and time constraints often result in inappropriate referrals, compromising patient care and resource utilisation. This study evaluated the appropriateness of referrals from the ED to general surgery (GS) through a two-cycle audit with a targeted intervention, acute abdominal pain pathway, incorporating early computed tomography (CT) imaging. METHODS: A

Evaluating AI-based comprehensive clinical decision support for sepsis and ARDS: protocol for a Clinician Turing Test.

INTRODUCTION: Few artificial intelligence (AI) clinical decision support systems (CDSSs) are ever evaluated in practice. Although some signal of clinical effectiveness may be needed to justify AI deployment and testing, such data are typically unavailable in early-stage research. This conundrum is especially relevant in the intensive care unit (ICU), where conditions like sepsis and acute respiratory distress syndrome (ARDS) require high-stakes decisions. Our group developed the AI ventilator assistant (AVA), a novel AI CDSS for patients with sepsis ARDS receiving invasive mechanical ventilati

Conventional and tablet-supported physical training to reduce falls and fall-related injuries in community-dwelling older adults: protocol of the randomised SURE-footed into the future Fall Intervention Trial (SURE-FIT).

INTRODUCTION: Exercise-based interventions are well-established in reducing falls and fall-related injuries, but adherence and accessibility remain key challenges, particularly in rural areas. While conventional in-person training is widely used, digital interventions may offer scalable solutions to enhance engagement and reach. However, pragmatic trials evaluating the real-world effectiveness of conventional and digitally supported fall prevention interventions are lacking, limiting the evidence base for their implementation in routine healthcare settings. The SURE-Footed into the Future Fall

Efficacy and safety of moxidectin compared with ivermectin against Strongyloides stercoralis infection in adults in Laos and Cambodia: a randomised, double-blind, non-inferiority, phase 2b/3 trial.

BACKGROUND: Infection with the soil-transmitted helminth Strongyloides stercoralis affects up to 600 million people globally, most of whom live in rural areas with poor sanitation. If untreated, infection leads to long-lasting morbidity and might even be life-threatening. Moxidectin might be a promising alternative to ivermectin, the only currently recommended single-dose treatment. We aimed to assess whether moxidectin is non-inferior in terms of efficacy and safety compared with ivermectin. METHODS: In this randomised, double-blind, parallel-group, non-inferiority, phase 2b/3 trial in commu

Effectiveness of a scalable group-based education and monitoring program, delivered by health workers, to improve control of hypertension in rural India: A cluster randomised controlled trial.

BACKGROUND: New methods are required to manage hypertension in resource-poor settings. We hypothesised that a community health worker (CHW)-led group-based education and monitoring intervention would improve control of blood pressure (BP). METHODS AND FINDINGS: We conducted a baseline community-based survey followed by a cluster randomised controlled trial of people with hypertension in 3 rural regions of South India, each at differing stages of epidemiological transition. Participants with hypertension, defined as BP ≥ 140/90 mm Hg or taking antihypertensive medication, were advised to visit

Pipelle for Pregnancy (PIP): study protocols for three randomised controlled trials.

BACKGROUND: The success rate of infertility treatments remains modest. Endometrial injury has been suggested as an intervention to increase the probability of pregnancy in women undergoing assisted reproductive technologies such as in vitro fertilisation (IVF). The majority of studies and systematic reviews have reported that endometrial injury improves the outcomes of IVF, intrauterine insemination and natural conception; however, the size and quality of the studies are poor. The low quality of the available evidence questions the presence of any real beneficial effect, and the applicability
