Twiddler's Syndrome and Neuromodulation-Devices: A Troubled Marriage.

OBJECTIVE: The occurrence of Twiddler's syndrome in subjects with neurostimulator devices is poorly understood and might be influenced by age, sex, BMI, use of medication or psychologic disorders. METHODS: Two hundred thiry-five patients who received a neuromodulator were included in this retrospective study in a period between 2008 and 2015. The subjects were divided into a group of Twiddler's syndrome patients (TS) and a group of non-Twiddler's patients (NTS). Outcome measures were gender, age at implantation, type of neuromodulation, use of antipsychotics, antidepressants and opioids, the

Minimally invasive pectus excavatum repair: migration of bar and ossification.

Minimally invasive repair of pectus excavatum is an effective treatment option, with satisfactory results. Nevertheless, complications may occur during bar removal. We describe the case of a 16-year-old boy operated on for pectus excavatum with the Nuss procedure, who developed bar migration into the rib, with ossification, making its removal very difficult.

Percutaneous retrograde intramedullary single wire fixation for metacarpal shaft fracture of the little finger.

This study aimed to evaluate the anatomic and functional outcome of retrograde intramedullary single wire fixation for metacarpal shaft fractures of the little finger. Forty consecutive patients with closed metacarpal shaft fractures of the little finger who have been treated with retrograde intramedullary single wire fixation were evaluated. Fracture union and angulation were analyzed radiologically, and clinical evaluations were performed including the percentage of total active motion (%TAM), disabilities of the arm, shoulder and hand (DASH) score, and complications. Fracture union was achi

Cardiac tamponade caused by broken sternal wire after pectus excavatum repair: a case report.

Complications of pectus excavatum surgery include pneumothorax, pleuritis, hemothorax, pericardial effusion, displacement of bar, pericarditis and cardiac injury, etc. This is the case of a 15-year-old boy with cardiac tamponade caused by pericarditis who had taken the operation for a pectus excavatum repair one year previously. The cause was a sternal wire which was used for attachment of the bar to sternum that had fractured and migrated through the pericardium causing a pericardial injury and a pericarditis.

Central venous catheter embolisation.

Tunnelled central venous catheters are commonly used for a variety of indications, including home parenteral nutrition, but are rarely associated with fracture and embolisation; the risk of embolisation is reported to be greater with catheters placed via the subclavian vein rather than the internal jugular route. We report the case of a 64-year-old woman with type 3 (chronic and irreversible) intestinal failure who presented with pain and swelling on infusion of parenteral nutrition through her internal jugular catheter. A chest x-ray showed fracture and embolisation of her catheter into the r

Successful management of multiple esophagorespiratory fistulas using two types of stent: report of a case.

We herein describe a 41-year-old man with esophageal cancer who developed three esophagorespiratory fistulas (ERFs) that were successfully treated using one esophageal and three airway stents. A self-expandable metallic stent (SEMS) was initially inserted into the esophagus to close an ERF in the right bronchus. However, two new ERFs developed in the trachea and the left main bronchus 3 months later because of pressure necrosis and penetration of the esophageal SEMS. These secondary ERFs were subsequently closed using two silicone stents, together with one SEMS in the airway. This experience s

Unusual cause of acute back pain mimicking aortic dissection: a case report.

We report the case of a 62-year-old woman who initially presented with symptoms suggesting acute type A aortic dissection. Imaging studies revealed hemorrhagic pericardial fluid without the evidence of dissection. Foreign body material was noted floating in the inferior vena cava (IVC) and also piercing the right ventricular wall. Upon surgical exploration, the extracted material could be identified to be acrylic bone cement (palacos). The patient had reported a history of kyphoplasty in 2008.

Polymethylmethacrylate augmentation of the pedicle screw: the cement distribution in the vertebral body.

Many studies have proven that the polymethylmethacrylate (PMMA) augmentation of the pedicle screw can significantly increase stiffness and strength of spinal fixation. Some major complications have also been reported. However, there are no reports discussing cement distribution and its morphology in the osteoporotic vertebral body, which is critical in the analysis of the biomechanical strength of the pedicle screw and the risk of cement leakage after pedicle screw augmentation. In this study, we used computed tomography (CT) to evaluate the cement distribution in the osteoporotic vertebral bo

Hypopharyngeal extrusion of 2.5 feet (76 cm) of polytetrafluoroethylene (Gore-Tex): initial laser-assisted office-based removal and micropharyngeal completion.

Extrusion of an implant after medialization laryngoplasty is unusual and warrants removal. Most commonly, it extrudes through the laryngeal introitus, but rarely, it extrudes through the pyriform sinus. A case report in which 2.5 feet (76 cm) of polytetrafluoroethylene (Gore-Tex) was removed from an 80-year-old female patient is presented to evaluate factors that led to this surgical complication and strategies that solved the problem. Because of the patient's multiple medical problems, initial removal of the foreign body was attempted in the office with topical anesthesia. When the Gore-Tex w

Percutaneous removal of endocardial implantable cardioverter-defibrillator lead displaced to the right pulmonary artery.

We describe a case of percutaneous removal of endocardial implantable cardioverter-defibrillator lead displaced to the right pulmonary artery. The procedure was performed from two accesses; from the lower one (femoral) and then, due to technical problems, from the upper one (subclavian). In the last stage the flattened Dotter's basket was introduced to the heart inside the Byrd dilator and then fastened to the described lead as the external 'splint'. This solution is an alternative to the recommended use of the internal metal leader with anchoring function in case of significant malformation o

Clinical outcomes and patency of self-expanding metal stents in patients with malignant colorectal obstruction: a prospective single center study.

BACKGROUNDS AND AIMS: Self-expanding metal stents (SEMS) have been used as a palliative treatment for malignant colorectal obstruction. However, the reports about primary stent patency rate and associated factors have been limited. This study was performed to evaluate clinical outcomes and factors associated with long-term complications and patency of SEMS in patients with malignant colorectal obstruction. MATERIALS AND METHODS: Patients who underwent palliative endoscopic placement with uncovered SEMS for a malignant colorectal obstruction were prospectively enrolled at Seoul National Univer

Surgically treated perforations of the gastrointestinal tract caused by ingested foreign bodies.

OBJECTIVE: Intestinal perforation due to foreign body (FB) ingestion is rare (1%). We describe our experience in treating these lesions surgically. METHOD: From 1995 to 2006, data were collected prospectively in 33 patients (18 women and 15 men; mean age 64 years) operated on for intestinal perforation due to an ingested FB. The type of object, preoperative diagnosis, perforation site, treatment, morbidity and mortality were reviewed. RESULTS: Foreign body ingestion was predominantly involuntary (88%). The mean time from ingestion to perforation was 10.4 days. The most frequently ingested ob

Complications with cervical arthroplasty.

OBJECT: Spinal arthroplasty is becoming more widely performed in the treatment of degenerative cervical disc disease. Although this new technology may offer benefits over arthrodesis, it also requires that the surgeon acquire new operative techniques, and new potential complications are introduced. To determine the incidence and distribution of perioperative complications, the authors analyzed their early data obtained in a series of patients treated with the Bryan Cervical Disc prosthesis. METHODS: The authors prospectively recorded operative data, complications, and clinical and radiographi

Failure of posterior titanium atlantoaxial cable fixation.

BACKGROUND CONTEXT: The operative treatment of rotatory atlantoaxial instability remains controversial. The use of cable fixation has largely replaced the use of wire for interlaminar fixation. Although cable fixation offers biomechanical advantages over wire fixation, it is still at risk of fatigue failure. The authors were unable to locate any published reports of fatigue failure of titanium cables in the fixation of atlantoaxial instability. PURPOSE: The purpose of this case study is to describe an unusual case of fatigue failure of a titanium cable used to aid in atlantoaxial fusion for t
