Treatment of Displaced Posterior Table Frontal Sinus Fractures: To Cranialize or Not to Cranialize? A Systematic Review and Meta-Analysis.

Frontal sinus fractures account for 5% to 11% of craniofacial traumas in the United States, and up to 40% of them involve the posterior table. Moderate to severe displacement or comminution of posterior table fractures has traditionally been addressed with cranialization. However, noncranialization surgical strategies, endoscopic surgical repair or observation have been proposed as alternative treatment options. A systematic review and meta-analysis were performed to evaluate complication rates and outcomes for cranialization versus noncranialization strategies or endoscopic or observation man

A modified anterolateral swashbuckler approach for distal femoral fractures: description and outcomes.

BACKGROUND: The treatment of distal femur fractures represents a challenging task. General and local factors, including bone quality, articular surface involvement, fracture comminution, associated soft tissue injuries and ultimately fixation system, all play a role in the final clinical outcome. Current surgical approaches often undermine patellar vascularization and integrity of the extensor apparatus, not guaranteeing sufficient visualization of the medial condyle. This technical note presents the efficacy and safety a new surgical technique to address distal femoral fractures. METHODS: Th

[Orbital compartment syndrome. The most frequent cause of blindness following facial trauma].

INTRODUCTION: Although orbital compartment syndrome is a rare condition, it is still the most common cause of blindness following simple or complicated facial fractures. Its pathomechanism is similar to the compartment syndrome in the limb. Little extra fluid (blood, oedema, brain, foreign body) in a non-space yielding space results with increasingly higher pressures within a short period of time. Unless urgent surgical intervention is performed the blocked circulation of the central retinal artery will result irreversible ophthalmic nerve damage and blindness. Aim, material and method: A retr

Arthroscopically guided osteotomy for management of intra-articular distal radius malunions.

PURPOSE: A malunion with a step-off of 1 mm or more after an intra-articular distal radius fracture may cause pain and arthritic changes at midterm follow-up. We present our technique for treating intra-articular distal radius malunions by carrying out an osteotomy from inside the joint outward under arthroscopic guidance using the dry arthroscopy technique, with emphasis on the clinical and radiologic outcomes. METHODS: We performed surgery on 11 patients for intra-articular malunion of the distal radius 1 to 5 months after the injury. Preoperative step-offs ranged from 2 to 5 mm (average, 2

The mandible injury severity score: development and validity.

PURPOSE: To develop and validate a clinical method for characterizing and scoring mandible injury severity. MATERIALS AND METHODS: Constituent fracture variables (fracture type, location, occlusion, soft tissue involvement, infection, and interfragmentary displacement [FLOSID]) were used to develop the FLOSID taxonomy for characterizing injury. Each component was assigned an empirical weight to help derive a summary measure of injury severity called the UCLA Mandible Injury Severity Score (MISS). Subsequently, MISS values were calculated for a group of 336 patients treated for mandible fractu

[Treatment of fragmented femur fracture following unilateral hip hemiarthroplasty and total knee arthroplasty].

In this paper is presented a case of the 80 years old patient with femur fracture after unilateral hip arthroplasty because of femur neck fracture and knee arthroplasty following osteoarthritis. First was done the stabilization of the fracture using a plate. Because of second fracture and damage the plate stabilization next operation was done using intramedullary Küntscher nail which was non typically inserted and with additional fixation by bone cement. The result were good stabilization and healing of the fracture after 4 months.

3D printing technology-assisted total hip arthroplasty for acute proximal femoral fracture and Hartofilakidis type II developmental dysplasia of the hip: a case report and literature review.

Developmental dysplasia of the hip (DDH) associated with an acute proximal femoral fracture on the same side is uncommon, and performing a one-stage total hip arthroplasty (THA) with concurrent fracture stabilization in such cases poses significant technical challenges. We present a case involving a 57-year-old female with a long history of right-sided DDH who was involved in a vehicular accident, leading to acute pain in her right hip, shortening of the limb, and restricted movement. Imaging studies indicated Hartofilakidis type II DDH along with a comminuted intertrochanteric fracture of the

Radiographic assessments of pediatric supracondylar fractures and mid-term patient-reported outcomes.

Radiographic parameters are commonly used to determine the need for surgical supracondylar humeral (SCH) fracture reduction and the postoperative quality of reduction. We studied whether such parameters are correlated with mid-term patient-reported outcome (PRO) scores in pediatric patients. We retrospectively reviewed data from 213 patients (104 girls) treated surgically for Gartland type-II (n = 84) or type-III (n = 129) SCH fractures from 2008-2016. Mean (± standard deviation) age at surgery was 5.1 ± 2.1 years. Mean time from initial treatment to outcome survey completion was 5.0 ± 2.1 year

[Mandibular fractures. Retrospective study of the experience of the Department of Maxillo-facial Surgery and Stomatology of the University of Poitiers Medical Center from 1978 to 1997].

Orthopedic or surgical repair is proposed for mandibular fractures, depending on schools and experience. We reviewed retrospectively 632 cases of mandibular fracture treated at the Poitiers University department of maxillofacial surgery between 1978 and 1997 to assess methods and outcome. We performed a global analysis and compared certain localizations with statistical tests. Different therapeutic protocols were used. The rate of complication was greater, for an equivalent initial lesion, with surgical compared with medical treatment. This was particularly true for fractures involving the jaw

Radial head fractures: indications and outcomes for radial head arthroplasty.

Radial head fractures without associated bony or ligamentous injury can be safely treated with internal fixation, if possible, or arthroplasty if nonreconstructable. However, nonreconstructable radial head fractures in association with elbow dislocation and/or ligamentous injury in the elbow or forearm represent a specific subset of injuries that requires restoration of the radiocapitellar articulation for optimal function. The purpose of this article was to summarize the indications for radial head arthroplasty and discuss the reported outcomes.

Arthrodesis for ankle osteoarthritis after harvesting of free vascularized fibular bone: a case report.

INTRODUCTION: Although free vascularized fibular bone grafting is a good method for the reconstruction of large bone defects, it might cause morbidity of the donor leg. Progression of ankle osteoarthritis, valgus deformity and instability of the donor leg subsequently leading to arthrodesis has rarely been reported. MATERIALS AND METHODS: A 53-year-old man suffered from a left tibial comminuted and Gustilo type IIIb open fracture. A folded free vascularized osteoseptocutaneous flap was harvested from the right fibula and transferred to the left tibial bone defect. After the reconstructive sur

Optimal placement of the temporary fixation pin in tibial plateau leveling osteotomy: a canine ex vivo study.

BACKGROUND: Tibial Plateau Leveling Osteotomy (TPLO) is widely accepted for stabilizing the stifle joint in dogs with cranial cruciate ligament disease. However, postoperative tibial tuberosity fractures remain a significant complication, particularly in small-breed dogs. Recent anatomical findings suggest that Sharpey's fibers (SF) contribute to local structural reinforcement within the tibial tuberosity, but the biomechanical impact of temporary fixation pin positioning relative to these fibers has not been experimentally quantified. RESULTS: Eighteen pelvic limbs from nine small-breed cani

Comparison between screw fixation and plate fixation via sinus tarsi approach for displaced intra-articular calcaneal fractures: a systematic review and meta-analysis.

BACKGROUND: Optimal surgical fixation for displaced intra-articular calcaneal fractures (DIACF) remains a subject of debate, particularly regarding the superiority between screw fixation and plate fixation via the sinus tarsi approach (STA). This review aims to determine the preferred treatment for DIACF and compare the outcomes of minimally invasive surgery options. METHODS: Our study involved thorough searches across multiple electronic databases, including PubMed, Cochrane, Embase, and Web of Science, to identify all relevant publications on distal intra-articular fractures of the calcaneu

Low-profile double plating of unstable osteoporotic olecranon fractures: a biomechanical comparative study.

BACKGROUND: In the treatment of unstable olecranon fractures, anatomically preshaped locking plates exhibit superior biomechanical results compared with tension band wiring. However, posterior plating (PP) still is accompanied by high rates of plate removal because of soft-tissue irritation and discomfort. Meanwhile, low-profile plates precontoured for collateral double plating (DP) are available and enable muscular soft-tissue coverage combined with angular-stable fixation. The goal of this study was to biomechanically compare PP with collateral DP for osteosynthesis of unstable osteoporotic

Surgical stabilization of flail chest injuries with MatrixRIB implants: a prospective observational study.

BACKGROUND: Surgical stabilization of flail chest injury with generic osteosynthesis implants remains challenging. A novel implant system comprising anatomic rib plates and intramedullary splints may improve surgical stabilization of flail chest injuries. This observational study evaluated our early clinical experience with this novel implant system to document if it can simplify the surgical procedure while providing reliable stabilization. METHODS: Twenty consecutive patients that underwent stabilization of flail chest injury with anatomic plates and intramedullary splints were prospectivel

Both column fractures of the acetabulum: epidemiology, operative management and long-term-results.

PURPOSE OF THE STUDY: Both column fractures, defined as an acetabular fracture with no articular fragment in connection with the axial skeleton account for approximately 20% of all acetabular fractures. The typical type of a both column acetabular fracture is the C1.2 fracture with a multifragmentary anterior column fracture extending to the iliac crest and a large posterior column fragment in more than half of the patients. MATERIAL AND METHODS: The analysis of 135 surgically treated patients with both column fractures showed that more than half of these patients had associated injuries. The
