PURPOSE: To describe techniques and evaluate outcomes of computed tomography (CT)-guided percutaneous lung biopsy in children.
MATERIALS AND METHODS: Between April 1992 and June 2003, 64 patients (32 male, 32 female) with a mean age of 10.8 years (0.6-20 years) were referred for 75 lung biopsies. Most biopsies were performed for suspected malignancy (n = 24; 32%) or to distinguish posttransplantation lymphoproliferative disorder from fungal infection in immunocompromised patients (n = 17; 23%). All children referred to the pediatric interventionalists in two children's hospitals for CT-guided

Because major surgery is usually required to obtain biopsies of abdominal organs, regulations tend to limit the number of procedures on individual animals to one. This study was conducted to develop a more humane, minor, comparatively cost-effective, minimally invasive surgical procedure, which reduces surgical trauma and the number of animals used. Biopsy techniques were developed in two nonsurvival rabbit surgeries. Safety and efficacy of multiple procedures were assessed in survival studies on four rabbits. Anesthesia was induced with ketamine/xylazine and maintained with isoflurane. Initia

OBJECTIVE: To describe the surgical technique and early clinical results after a one-stage laparoscopic-assisted endorectal colon pull-through for Hirschsprung's disease.
SUMMARY BACKGROUND DATA: Recent trends in surgery for Hirschsprung's disease have been toward earlier repair and fewer surgical stages. A one-stage pull-through for Hirschsprung's disease avoids the additional anesthesia, surgery, and complications of a colostomy. A laparoscopic-assisted approach diminishes surgical trauma to the peritoneal cavity.
METHODS: The technique uses four small abdominal ports. The transition zone

During the past decade, lung transplantation has emerged as the definitive treatment for children with end-stage lung disease. Pediatric transplantation presents unique challenges with respect to diagnostic indications, donor-recipient size disparities, perioperative management, and growth. Lessons from the early development of cardiac surgery at the University of Minnesota (Green Surgical Service) provide a useful model for novel surgical challenges. Since 1990, 25 lung transplantations have been performed at our institution, including 4 heart-lung, 3 single-lung, 17 bilateral-lung, and 1 liv

Adult intraabdominal surgery performed with the minimal-access technique has created a revolution in surgery. This technique has remained largely unused for pediatric surgical procedures because of concerns regarding its safety and efficacy. Presently, intraabdominal insufflation of CO2 is the preliminary step to performing minimal access surgery. In this study, an animal model was developed to determine the effects of intraabdominal CO2 insufflation in the infant. Eight piglets (4 to 6 kg; 14 to 19 days of age) were instrumented under fentanyl anesthesia to allow measurement of arterial blood

Intravenous (IV) propofol was compared with IV thiopental/pentobarbital as a sedative for children undergoing magnetic resonance imaging (MRI) of the brain or spine. Fifty-eight outpatients (aged 11 mo to 6 1/2 yr, ASA grade I and II) were enrolled in the study and randomized to two groups. After IV cannulation, Group I received IV propofol (1-2 mg/kg), followed immediately by a propofol infusion (75-100 micrograms.kg-1.min-1). Group II received IV thiopental (1-3 mg/kg) followed by a pentobarbital bolus (2-3 mg/kg). Supplemental thiopental doses (1-2 mg/kg) were administrated to maintain adeq

INTRODUCTION: Horner syndrome (HS) is a rare but recognized complication of internal jugular vein (IJV) catheterization in children, resulting from injury to the cervical sympathetic pathway. Neonates and infants are particularly vulnerable due to the compact anatomy of the neck.
PRESENTATION OF THE CASE: We report a 38-day-old female infant who developed ipsilateral HS following open surgical placement of a Broviac catheter via the right IJV. The catheter was inserted under local anesthesia with sedation for prolonged intravenous therapy. Within 24 hours postoperatively, the patient presente

BACKGROUND: Language barriers continue to limit participation in international medical communication, and recent advances in artificial intelligence now enable real-time voice translation to support multilingual scientific exchange.
APPROACH: We describe a demonstration of a bidirectional AI-mediated voice translation configuration at a pediatric anesthesia conference and consider its broader applicability across medical communication contexts. Outbound delivery used real-time speech translation with speaker-matched voice synthesis; inbound comprehension of audience questions used earpiece-ba…

INTRODUCTION: Gastric dysmotility complicates enteral feeding in children, often requiring distal feeding access. While nasoduodenal and gastrojejunostomy tubes are common, frequent dislodgements necessitate hospital visits for fluoroscopic-guided replacement, exposing children to radiation and potential anesthesia risks. The omega-jejunostomy (OJ) tube was developed as a simpler alternative to roux-en-Y feeding jejunostomy, allowing easy at-home tube changes after tract maturation. Despite nearly two decades of use, OJ-tube placement remains poorly characterized. This study examines patient f

Background The optic nerve sheath diameter (ONSD) on ocular ultrasonography has recently emerged as a reliable and useful indirect tool for measuring raised intracranial pressure (ICP). However, findings of previous studies have been inconsistent or inconclusive, and not many studies have evaluated its utility in the pediatric population undergoing neurosurgical interventions. This observational study aimed to evaluate the efficacy of ultrasonographic ONSD as a point-of-care tool for predicting real-time ICP in children with hydrocephalus undergoing shunt surgery. Methodology This study includ

BACKGROUND: In choledocholithiasis, the obstructed common bile duct (CBD) requires clearance either via endoscopic retrograde cholangiopancreatography (ERCP) or intraoperative cholangiogram (IOC) and common bile duct exploration (CBDE) during cholecystectomy. We hypothesized that patients with primary cholecystectomy (PC) and IOC/CBDE will have improved clinical outcomes when compared to primary ERCP (PE) patients.
METHODS: We performed a retrospective cohort study of pediatric choledocholithiasis patients who underwent treatment at our institution between 2019 and 2023. We compared clinical
