Silent chorioamnionitis and associated pregnancy outcomes: a review of clinical data gathered over a 16-year period.

OBJECTIVE: To assess neonatal outcomes and associated findings in pregnant women identified after delivery as having had underlying subclinical chorioamnionitis by either histology or bacterial culture. METHODS: In 16 years, 8974 clinical, histological, and bacterial culture data were obtained retrospectively. RESULTS: Placental histology was analyzed in 4237 pregnancies (2785 term and 1452 preterm) and 4737 amniotic cavity cultures were obtained during 5446 cesarean deliveries (3268 term and 1469 preterm). Histological results and bacterial cultures were both available in 1270 of the preter

Incidence of invasive group B streptococcal disease and pathogen genotype distribution in newborn babies in the Netherlands over 25 years: a nationwide surveillance study.

BACKGROUND: Group B streptococcus is the most common cause of neonatal infections. We studied the clinical and molecular epidemiology of invasive group B streptococcus infection in children younger than 3 months in the Netherlands over 25 years. We assessed the effect of the Dutch guidelines, introduced in 1999, for prevention of group B streptococcus, consisting of intravenous antibiotic prophylaxis during labour in cases of premature labour, prolonged rupture of membranes, or fever during delivery. METHODS: We did this nationwide surveillance study with data from 1987 to 2011, from the Neth

Rapid identification of group B streptococcus carriage by PCR to assist in the management of women with prelabour rupture of membranes in term pregnancy.

BACKGROUND: Management of prelabour rupture of membranes at term (37 weeks gestation or later) (TPROM) remains complicated in the absence of a rapid assay for group B streptococcus (GBS) colonisation. AIMS: To evaluate the accuracy and clinical utility of a commercial PCR assay, compared with culture, for detection of GBS colonisation in pregnant women presenting with TPROM. METHODS: A prospective study of women presenting with TPROM conducted in a large tertiary hospital (Westmead Hospital, Australia). Five hundred and seventy-four consecutive women with TPROM were enrolled between July 200

Genital tract group B streptococcal colonization in pregnant women: a South Indian perspective.

INTRODUCTION: During the last few decades, group B Streptococcus (GBS) has emerged as an important pathogen. The major reservoirs for GBS are the vagina and the peri-anal regions/rectum, and the colonization of these regions is a risk factor for subsequent infection in pregnant women and newborns. METHODOLOGY: A prospective study was performed to determine the prevalence of GBS colonization in the vagina and rectum of pregnant women and the antibiotic susceptibility pattern of the isolates. We also aimed to identify risk factors associated with GBS colonization. The vaginal and rectal swabs w

Group B streptococcal disease in infants: progress in prevention and continued challenges.

The burden of early-onset disease caused by group B Streptococcus (GBS) has decreased dramatically in the United States over the past 20 years. Universal culture-based screening at 35 to 37 weeks gestational age and use of intrapartum antibiotic prophylaxis are the cornerstones of prevention measures that have led to this decline. GBS, however, remains the leading cause of early-onset neonatal sepsis in the United States. Revised guidelines for prevention of perinatal GBS are planned for issuance in 2010. This article discusses implementation challenges for clinicians caring for pregnant women

Group B streptococcus colonization of Romanian women: phenotypic traits of isolates from vaginal swabs.

In the attempt to enrich the local contemporary laboratory data regarding the group B streptococcus (GBS) colonization, isolates obtained from the vaginal swab cultures were characterized for their serotype distribution and antibiotic susceptibility. The 100 GBS isolates analyzed were collected during a four-month period of year 2009 from women screened in ambulatory for vaginal carriage of GBS. The GBS isolates were classified based on their capsular polysaccharide structures using commercially available antisera. Susceptibility to penicillin, ampicillin, erithromycin, clindamycin, tetracycli

Group B Streptococcus infection: epidemiology, serotypes, and antimicrobial susceptibility of selected isolates in the population beyond infancy (excluding females with genital tract- and pregnancy-related isolates) at the University Malaya Medical Centre, Kuala Lumpur.

Group B Streptococcus (GBS) infection was studied in 49 patients collected at convenience (convenience sampling), excluding infants and women with genital tract- and pregnancy-related isolates, according to the availability of stocked isolates and easy accessibility to epidemiological data. The data were examined both prospectively and retrospectively from 2003-2005 at a tertiary-level multidisciplinary hospital in Kuala Lumpur, Malaysia. Skin and soft-tissue infections in 35 patients (71.4%) were the most common clinical presentation, while diabetes mellitus was the most common underlying con

Group B Streptococcus colonization in pregnancy: prevalence and prevention strategies of neonatal sepsis.

Early onset neonatal sepsis due to Group B streptococci (GBS) is responsible for severe morbidity and mortality of newborns. While different preventive strategies to identify women at risk are being recommended, the optimal strategy depends on the incidence of GBS-sepsis and on the prevalence of anogenital GBS colonization. We therefore aimed to assess the Group B streptococci prevalence and its consequences on different prevention strategies. We analyzed 1316 pregnant women between March 2005 and September 2006 at our institution. The prevalence of GBS colonization was determined by selective

Intracellular viability in human non-polarized respiratory epithelial 16 HBE 14o- cells by group B Streptococcus serotype III clinical isolates presenting 162-kb and 183-kb virulence markers.

Group B streptococci (GBS), mainly serotype III, are the major cause of neonatal pneumonia, sepsis and meningitis. Virulence potential of GBS strains may determine the outcome of host colonization or infection. Because the lung constitutes a first step in GBS systemic invasion processes, we investigated the adherence and invasion mechanisms of GBS-III clinical isolates to non-polarized human bronchial epithelial 16 HBE 14o- cell line. The presence of genotypic 162-kb and 183-kb virulence markers in all strains was also examined by PFGE. The 162-kb fragment was detected in both liquor (GBS-III

Prevention of early-onset neonatal group B streptococcal infection: is universal screening by culture universally applicable?

BACKGROUND: Previous assessments of maternal group B Streptococcus carrier rates in women delivering at Shaare Zedek Medical Center ranged between 3.5 and 11% with neonatal sepsis rates of 0.2-0.9/1000 live births. Because of low colonization and disease rates, routine prenatal cultures of GBS were not recommended and intrapartum prophylaxis was mainly based on maternal risk factors. OBJECTIVES: To determine whether this policy is still applicable. METHODS: We performed prospective sampling and follow-up of women admitted for labor and delivery between February 2002 and July 2002. Vaginal an

Mutagenesis of a bacteriophage lytic enzyme PlyGBS significantly increases its antibacterial activity against group B streptococci.

Group B streptococci (GBS) are the leading cause of neonatal meningitis and sepsis worldwide. Intrapartum antibiotic prophylaxis (IAP) is the current prevention strategy given to pregnant women with confirmed vaginal GBS colonization. Due to antibiotic resistance identified in GBS, we previously developed another strategy using a bacteriophage lytic enzyme, PlyGBS, to reduce vaginal GBS colonization. In this study, various DNA mutagenesis methods were explored to produce PlyGBS mutants with increased lytic activity against GBS. Several hyperactive mutants were identified that contain only the

Capsular antigens of bacteria. Capsular antigens as the basis of vaccines against pathogenic bacteria.

The role of bacterial capsular antigens represented in capsular polysaccharides and exoglycans in pathogenicity and virulence of bacteria is discussed in this review. Using capsular antigens for vaccines against severe diseases caused by capsular microorganisms is considered in detail. The use of conjugates of capsular polysaccharides and their fragments with proteins and peptides for vaccine as well as using liposomes as adjuvants for the capsular antigens are described. Data concerning structural elucidation of bacterial capsular antigens are given in the first part of this review.

Streptococcal fasciitis with toxic shock syndrome in the pediatric patient.

Necrotizing fasciitis (NF) is a distinctive soft tissue infection usually caused by Group A beta-hemolytic streptococcus, often seen in association with varicella in the previously healthy child. Its fulminant course is associated with great morbidity and high case-fatality rates, especially when it occurs in conjunction with streptococcal toxic shock syndrome. It differs from simple cellulitis in clinical presentation, appearance on examination, and need for urgent surgical intervention. The patient is irritable and complains of severe pain in the involved site, which is usually the extremity

Critical role of the complement system in group B streptococcus-induced tumor necrosis factor alpha release.

Group B Streptococcus (GBS) is a major cause of newborn sepsis and meningitis and induces systemic release of tumor necrosis factor alpha (TNF-alpha), believed to play a role in morbidity and mortality. While previous studies have shown that GBS can induce TNF-alpha release from monocytes and macrophages, little is known about the potential modulating effect of plasma or serum on GBS-induced TNF-alpha release, and there are conflicting reports as to the host receptors involved. In a human whole-blood assay system, GBS type III COH-1 potently induced substantial monocyte TNF-alpha release in ad

The necessity of both anorectal and vaginal cultures for group B streptococcus screening during pregnancy.

BACKGROUND: Group B streptococcus (GBS) sepsis affects approximately 2 of every 1000 newborns. In an effort to decrease the incidence of neonatal GBS infection, the Centers for Disease Control and Prevention have established guidelines for screening and treatment during pregnancy. One strategy includes obtaining both vaginal and anorectal GBS cultures, then treating patients whose cultures are positive. Many of our patients are reluctant to undergo anorectal cultures. We conducted a study to determine whether performing cultures of both the vagina and anorectum would change patient management.

Neonatal early onset group B streptococcal infection. A nine-year retrospective study in a tertiary care hospital.

Retrospectively, morbidity and mortality of neonatal early onset group B streptococcal (GBS) infection were established. Risk factors and prognostic factors were determined. Between 1985 and 1993, 78 patients with early onset GBS disease were identified. The overall mortality rate was 23%. In 60 of 73 cases (82%) at least one of the investigated risk factors was present. Low birth weight was not an independent risk factor. Outcome of 44 of 60 survivors (73%) at the age of at least one year was obtained. Almost 30% of them had sequelae. The most important were spastic disorders and delayed psyc
