Upper Limb Ataxia Among Residents With Chronic Inorganic Arsenic Exposure: A Quantitative Pilot Study.

Background Organic arsenic compounds can cause cerebellar ataxia, but the neurological profile of chronic inorganic arsenic exposure remains unclear. In Toroku, Japan, a historically exposed community, many residents report dizziness, yet it is unknown whether chronic inorganic arsenic exposure is associated with cerebellar dysfunction. We focused on upper limb ataxia as an accessible and quantifiable manifestation of cerebellar function in this setting. Methods We conducted a retrospective pilot study of residents who had lived within 1,000 meters of the Toroku mine roaster before 1962 and we

Engineering vascularized inner ear organoids: Challenges and advances in recapitulating the cochlear microenvironment.

The inner ear is a highly specialized structure responsible for auditory and vestibular functions; however, research progress has been limited by its deep anatomical location and the difficulty of obtaining human tissue samples. Recent advances in inner ear organoids (IEOs) and organoid-on-a-chip technologies offer powerful platforms for studying hearing loss mechanisms and testing therapies. This review outlines progress in generating IEOs from human pluripotent stem cells, including differentiation into hair cells and spiral ganglion neuron-like cells, and improvements in 3D culture, co-cult

Vestibulotoxicity and ototoxicity of gentamicin in newborns at risk.

Gentamicin is a potentially ototoxic drug routinely used for treatment of life-threatening infectious diseases in neonatology. In study 1, of 8,333 children examined for hearing disorders, 134 (1.6%) had received previous treatment with gentamicin. Only eight (6.0%) suffered from various extents of sensorineural hearing impairment, and all eight had a history of other risk factors of hearing loss (e.g., perinatal asphyxia, acidosis, icterus gravis, or meningitis). In study 2, 30 children (mean age, 13.2 months) with normal hearing had received gentamicin during the newborn phase, and 30 health

[Use of central electroanalgesia to restore the body's functional state in motion sickness].

Central electroanalgesia (CEAN) is a technique that involves application of pulsating current to the central nervous system in order to enhance its tonicity recover autonomic equilibrium and to reduce parasympathetic cardiovascular reactions. In two experimental runs in which 9 volunteers participated the efficacy of CEAN in preventing motion sickness symptoms produced by Markaryan's test was studied. After two rotations repeated with a one-hour interval vestibular reactions persisted for 7-10 hours. CEAN had a positive effect: after a one-hour CEAN session applied between two rotations the to

Endolymphatic hydrops in the rabbit: auditory brainstem responses and cochlear morphology.

A rabbit model of endolymphatic hydrops was studied using detailed functional and cytohistologic methods. Immediately following surgical destruction of the endolymphatic sac and the distal portion of the duct, measures of the evoked auditory brainstem response (ABR) revealed mild to profound losses specific to low- and high-frequency test stimuli while responses to mid-frequency signals remained unchanged for the majority of animals. Rabbits exhibited varying degrees of vestibular upset involving both overt behavior and reduced responses to caloric stimulation. Histologic processing of the pla

Injury of the Vestibulocerebellar Tract and Signs of Ataxia in Patients with Cerebellar Stroke.

BACKGROUND: The vestibulocerebellar tract (VCT) is responsible for maintaining balance, spatial orientation, and coordination. Damage to the vestibular system is accompanied by symptoms of balance disorder or ataxia. This study aimed to compare cerebellar dysfunction according to VCT damage in patients with cerebellar stroke. METHODS: Six patients with cerebellum injury were recruited. This study measured ataxia and hand function related to visuomotor integration and manual dexterity using the Purdue pegboard test. The primary and bilateral secondary VCTs were reconstructed to investigate the

Long-term effects of vestibular compensation on balance control and sensory organisation after unilateral deafferentation due to vestibular schwannoma surgery.

The time-course of central adaptive mechanisms after vestibular schwannoma surgical removal allows, 3 months after surgery (middle term), a satisfactory recovery of balance control. However, the long-term evolution of postural control beyond the end of usual medical follow-up remains unknown. This longitudinal prospective study aimed to assess the long-term effects of vestibular compensation on balance control and sensory organisation in patients operated on for vestibular schwannoma. Thirty-six patients with vestibular schwannoma underwent vestibular and sensory organisation tests, shortly be

Audiometric and vestibular features in a second Dutch DFNA20/26 family with a novel mutation in ACTG1.

OBJECTIVES: We analyzed the phenotype in a 5-generation DFNA20/26 family with a novel missense mutation in the ACTG1 gene (c.151G>A) and compared the findings to previous reports on DFNA20/26 families. METHODS: Audiometric data were collected from the family members of a Dutch kindred with the novel ACTG1 mutation. Cross-sectional and/or longitudinal analyses were performed on pure tone and speech audiometry data of the mutation carriers. Age-related typical audiograms were constructed. Vestibular examination was performed in all mutation carriers. RESULTS: Overall, high-frequency hearing im

Auditory dysfunction in stroke.

The auditory and vestibular systems share the same end organ and cranial nerve, yet vestibular signs and symptoms are common with stroke, whereas hearing disturbances are much less frequent. Several reasons would appear to account for this striking dissimilarity. One is that the auditory pathway is less ubiquitous than the vestibular pathways. The likelihood that a stroke involves the auditory pathway is, therefore, less on this basis alone. A second difference, to our knowledge not previously reported, is that the auditory pathway is often spared by the most common strokes. This is because ma

[Radiosurgery or microsurgery of vestibular schwannomas?].

PURPOSE: Microsurgical excision is the reference treatment of vestibular schwannomas. Yet, morbidity and functional risk of this surgery is significant, as Pellet et al have demonstrated in their study. In order to define the role of gamma knife surgery we have designed a prospective study concerning effectiveness and tolerance of this treatment. PATIENTS AND METHODS: Between July 14, 1992 and August, 1997, 400 patients were included. All the treatments were carried out according to a homogeneous methodology, with multiisocentric planning. We use low marginal doses dependent mainly on the tre

Neurocognitive factors in sensory restoration of early deafness: a connectome model.

Progress in biomedical technology (cochlear, vestibular, and retinal implants) has led to remarkable success in neurosensory restoration, particularly in the auditory system. However, outcomes vary considerably, even after accounting for comorbidity-for example, after cochlear implantation, some deaf children develop spoken language skills approaching those of their hearing peers, whereas other children fail to do so. Here, we review evidence that auditory deprivation has widespread effects on brain development, affecting the capacity to process information beyond the auditory system. After se
