Prosthetic hip infection related to a dental procedure despite antibiotic prophylaxis.

A 39-year-old man with systemic lupus erythematosus who was taking corticosteroids had a prosthetic hip infection with Streptococcus oralis after a dental procedure despite prophylaxis with erythromycin. The causative organism was resistant to erythromycin. For patients with prosthetic joints the literature does not support dental prophylaxis, which even if given appropriately, may fail to prevent infection. Some experts advocate giving antibiotic prophylaxis to patients at high risk, although data supporting this practice are limited. However, considering that most orthopedic surgeons and man

Role of antibiotic prophylaxis in retropubic surgery for stress urinary incontinence.

The effectiveness of prophylactic antibiotics was studied prospectively in 26 women undergoing retropubic urethropexy for genuine stress incontinence. Each of the 14 patients in the antibiotic prophylaxis group received three 1-g doses of intravenous cefazolin administered before, during, and 6-8 hours after surgery. Twelve women served as a control group and did not receive prophylactic antibiotics. Postoperative febrile morbidity and hospital stay were significantly less (P less than .01 and P less than .05, respectively) in patients who received prophylactic antibiotics.

Septic arthritis following arthroscopy, with cost/benefit analysis of antibiotic prophylaxis.

Nine cases of septic arthritis following arthroscopy are reviewed retrospectively. All cases of septic arthritis followed arthroscopic surgery. The average age of the patients was 49 years. The diagnosis was confirmed by bacterial cultures of the joint aspirates. All cases were treated by appropriate antibiotics supplemented by repeat arthroscopy and placement of a suction irrigation system. The average stay in hospital was 21 days. The authors suggest that it is not possible to predict such a major complication based on risk factors and therefore present a cost/benefit analysis of antibiotic

Mitral valve prolapse and bacterial endocarditis: when is antibiotic prophylaxis necessary?

We investigated the association between mitral valve prolapse (MVP) and bacterial endocarditis (BE) in a case-control study of 56 patients with BE and 168 age- and sex-matched controls who had had echocardiography. Cases and controls were selected from patients without other known cardiovascular risk factors for BE. Twenty percent of the BE cases (11 of 56) and 4% of the controls (7 of 168) had MVP; the odds ratio of 5.3 (95% confidence interval 2.0 to 14.4) indicated a significantly greater risk of BE in patients with MVP. This increased those who had preexisting systolic murmurs (9 of 11). E

Transient bacteremia due to suction abortion: implications for SBE antibiotic prophylaxis.

The incidence and character of the bacteremia associated with elective suction abortion was investigated in volunteer subjects aged 19 to 35 years who were to undergo first trimester abortion by suction curettage. One hundred and forty-four blood cultures were obtained from thirteen pregnant and four non-pregnant (control) subjects matched for age. Transient bacteremia occurred during or soon after suction abortion in 11 of 13 (84.7%) study subjects. Four of these patients were bacteremic after bimanual pelvic examination, just prior to initiation of the abortion procedure. Seven others develo

[Antibiotic prophylaxis in pediatric pulmonology (excluding cystic fibrosis): which indications for rotating (or alternating) antibiotics and prolonged antibiotic therapy?].

Some children with chronic lung disease associated with mucociliary clearance impairment and chronic bronchial congestion develop, during their evolution, bronchial bacterial colonization and recurrent infections. Therefore, antibioprophylaxis (ABP) is proposed by pediatric pulmonology specialists. Although some children seem improved by this strategy, it is worthy to note that no pediatric study supports its effectiveness, and no guidelines from society currently recommend its prescription. Demonstrated clinical benefits of ABP involve rare and highly targeted diseases. These children require

Antibiotic prophylaxis with cefoxitin in colorectal surgery: effect on the colon microflora and septic complications--a clinical model for prediction of the benefit and risks in using a new antibiotic in prophylaxis.

Cefoxitin was administered parenterally in doses of 2 g, every six hours for two days, to 20 patients undergoing colorectal surgery. Blood and fecal specimens were obtained for five days for analysis of cefoxitin concentrations. Tissue samples from the gut wall were obtained at surgery. The maximum serum concentrations during surgery varied between 25 and 100 microgram/ml, and the cefoxitin concentrations for the fecal samples were 1.5-35 microgram/g and for the tissue samples, 2.0-50 microgram/g. Fecal samples for cultivation of aerobic and anaerobic bacteria were also obtained daily during t

The effect of peri- and postoperative antibiotic prophylaxis on surgical site infection in surgeries with elective antibiotic administration.

The aim of this study was to investigate the effect of peri- and postoperative antimicrobial prophylaxis (AMP) on surgical site infection (SSI) in surgeries with elective antibiotic administration in a large university hospital with a high volume of people in the operating room. In this retrospective study, 1060 cats and dogs belonging to private owners were analysed for the occurrence of SSI over a period of almost 5 years, except during the COVID pandemic. Both the patient files were included, and the patient owners were contacted by questionnaire. The type of surgery, the use and type of AM

Prospective, randomised study on antibiotic prophylaxis in colorectal surgery. Is it really necessary to use oral antibiotics?

BACKGROUND AND AIMS: The use of prophylactic antibiotics in addition to mechanical cleansing is the current standard of care prior to colonic surgery. The question of whether the antibiotics should be administered intravenously or orally, or by both routes, remains controversial. Our aim was to compare three methods of prophylactic antibiotic administration in elective colorectal surgery. METHODS: Three hundred consecutive elective colorectal resections were studied. All patients had preoperative mechanical colon cleansing with oral sodium phosphate and intravenous antibiotic prophylaxis with

Antibiotic Stewardship Interventions Improve Choice of Antibiotic Prophylaxis in Total Joint Arthroplasty in Patients with Reported Penicillin Allergies.

BACKGROUND: Most patients who report a penicillin allergy can tolerate cefazolin, the preferred prophylaxis in a total joint arthroplasty (TJA). Regardless, patients with a reported penicillin allergy are less likely to receive first-line perioperative antibiotics as a result of inaccurate penicillin allergy documentation and misconceptions regarding cross-reactivity between penicillin and cephalosporins. The over-reporting of penicillin allergies and the safety of cephalosporins in patients with reported penicillin allergies have been well established throughout the evidence [13]. QUESTIONS/

Evaluating the necessity of topical and subconjunctival antibiotic prophylaxis in ocular surgery.

PURPOSE OF REVIEW: The role of topical and subconjunctival antibiotics in reducing infection risk in major ocular surgeries remains unclear despite their widespread use. This review discusses the efficacy of both administration routes across various forms of surgery, underscoring evidence-based recommendations for quality practice patterns and limitations in current research. RECENT FINDINGS: Although intracameral antibiotics consistently result in low infection rates in phacoemulsification procedures, there may be some role for topical and subconjunctival antibiotic use, also influenced by r

Recurrent Cellulitis: Who is at Risk and How Effective is Antibiotic Prophylaxis?

Recurrent cellulitis following successful treatment is common and prevention should be a major component in the management of cellulitis. Conditions that increase the risk of recurrence include chronic edema, venous disease, dermatomycosis and obesity. These risk factors should be actively managed as further episodes of cellulitis increases the risk of recurrence. The role of non-antibiotic measures is important and should be first-line in prevention. Antibiotic prophylaxis is effective, but its role is limited to non-purulent cellulitis where risk factors are appropriately managed.

Post-splenectomy antibiotic prophylaxis--unfinished story: to treat or not to treat?

Overwhelming infection in asplenic patients is a well documented occurrence in the literature. The introduction of immunization with polyvalent pneumococcal, Hemophilus Influenza and meningococcal vaccines significantly cut down the incidence of post-splenectomy sepsis and mortality. However, the issue of prophylactic antibiotic therapy in these patients remains inconclusive. There are contradictory reports bouncing between life-long treatment to no treatment. There are also more flexible approaches emerging from patient difficulties complying with a prolonged therapy. This debate calls for de
