[Morphofunctional changes in human flatfoot].

The process of the flatfoot formation is accompanied not only by stretching but also by pressing of the plantar ligaments and muscles. In the plantar aponeurosis and ligaments subjected to pressing, adaptive rearrangements are observed; they are accompanied by transformation of the fibrillar connective tissue into cartilaginous one, that rather prevents the ligaments to affect the heigh of the fornix. In the structure of the plantar muscles rather noticeable changes occur, destructive ones including, that is seen in their microscopical picture.

Progressively worsening ptosis in a woman: A case report.

KEY CLINICAL MESSAGE: Filler injections into the upper eyelid may cause levator aponeurosis fibrosis and ptosis. This risk must be considered. When ptosis appears, treatment might be difficult. Understanding the upper eyelid anatomy and procedures is essential to prevent eyelid damage. ABSTRACT: Ptosis is a prevalent condition in cosmetic surgery that occurs due to malfunction of the levator palpebrae superioris or insufficient Müller muscle action. It is characterized by the upper eyelid edge appearing lower than usual when seen at eye level. Ptosis may be categorized into congenital and acq…

Collagenase clostridium histolyticum for Dupuytren's disease: a comprehensive systematic review and comparative analysis against percutaneous needle aponeurotomy and limited fasciectomy.

This systematic review compared the efficacy of collagenase clostridium histolyticum (CCH) against percutaneous needle aponeurotomy (PNA) and limited fasciectomy (LF) for Dupuytren's contracture. Searches were conducted in PubMed, Embase, and Web of Science databases to March 2024. Randomised controlled trials (RCTs) were included. Primary outcomes included successful contracture correction as defined by included studies. Secondary outcomes included recurrence rates, patient-reported outcomes, and adverse events. A total of 11 studies (969 patients) were included. Meta-analysis showed no diffe

Percutaneous Plantar Fascia Release With Needle: Anatomic Evaluation with Cadaveric Specimens.

Percutaneous plantar fascia release with needle is a novel procedure for the treatment of plantar fasciitis. The objective of this cadaveric study is to perform an anatomic evaluation of the percutaneous plantar fascia release method using a conventional hypodermic needle. In this study, we used 14 fresh-frozen cadaveric trans-tibial amputation specimens. Percutaneous plantar fasciotomy with a conventional hypodermic needle was performed. After a proper dissection, the width of the plantar fascia, the thickness of the medial border, and the width of the cut segment were measured and recorded.

Rupture of plantar fascia: Current standard of therapy: A systematic literature review.

BACKGROUND: The aim of the present systematic literature review is to give an overview of ruptures of the plantar fascia. For this purpose, a detailed description of the patient collective is provided. However, the focus of this analysis is based on the current therapy concepts. Based on the results the authors propose a standardized therapy concept. MATERIAL AND METHODS: A systematic literature review was performed using the PubMed database using the terms: ("rupture plantar fascia" OR "plantar fascia tear" OR "rupture plantar aponeurosis"). All articles published in the PubMed database unti…

Soft-tissue osteochondroma of the heel pad: a case report and review of literature.

Extraskeletal osteochondroma of the foot are rare benign cartilaginous tumours. We present a case of soft-tissue osteochondroma in the heel pad superficial to the postero-inferior aspect of the calcaneus. We propose the pathogenesis of this lesion might be related to metaplasia in the plantar aponeurosis as described in literature, or it may be a fracture of the calcaneal osteochondroma, growing and presenting as soft-tissue lesion in the heel pad.

[Dupuytren's disease].

Dupuytren's disease is a contracture of the hand derived from the retractile fibrosis of the palmar aponeurosis, that leads to a progressive deformity in flexion of fingers. It has been named from the French surgeon Guillaume Dupuytren (1777-1835), that described it in 1831. In this note it is sketched a short biography of Dupuytren and the main clinical features of the disease are described, underlining some particular aspects of therapy.

Infected subgaleal hematoma in a 4-month-old girl.

Subgaleal hematoma is an uncommon, but potential sequela of birth trauma and instrument-assisted delivery of neonates, as well as head trauma in young children. A rare complication is an infection of the subgaleal hematoma, which typically happens due to concomitant scalp lacerations. Escherichia coli is the most common causative pathogen in peripartum cases, and Staphylococcus aureus predominates in trauma cases. An even more rare complication is infection of the hematoma with intact overlying skin, the proposed mechanism of action of which is a hematogenous spread of the bacteria. In this ca

Ultrasonographic Evaluation of the Femoral Cartilage, Achilles Tendon, and Plantar Fascia in Young Women Wearing High-Heeled Shoes.

BACKGROUND: Specific attention on the musculoskeletal impact of wearing high-heeled shoes (HHS) has mainly focused on knee osteoarthritis and the literature is limited to biomechanical changes. The distal femoral cartilage has not been morphologically studied. Additionally, although heel elevation is coupled with a shear stress at the heel and overloaded calf muscles, Achilles tendon (AT) and plantar fascia (PF) thicknesses have not been assessed either. OBJECTIVE: To investigate whether the distal femoral cartilage, AT, and PF were different in women wearing HHS and flat-heeled shoes (FHS) a

Inguinal hernia repair with the peduncled fascial flap: a new surgical technique.

INTRODUCTION: This paper presents a new surgical technique of inguinal hernia repair in which both crura of the aponeurosis of the external oblique abdominal muscle and transverse fascia were used for complex reconstruction of the entire musculopectineal hiatus. MATERIAL AND METHODS: Between 2nd December 2003 and 29th April 2005, 250 patients (233 male and 17 female) underwent inguinal hernia repairs using our own technique. The inguinal canal was opened together with the posterior wall, dividing the transverse fascia into two flaps. The lower flap was inserted into the femoral opening and se…

Functional results of microsurgical reconstruction of plantar defects.

Of 57 patients who underwent microvascular free flap reconstruction of the plantar surface, 21 developed recurrent trophic ulcerations. Clinical and roentgenographic examination demonstrated that the recurrences were primarily traumatic in origin, resulting either from exogenous factors such as footwear or from endogenous factors such as osteophytes. A new method of surgically reconstructing the plantar aponeurosis was found to be effective in reducing the rate of recurrence.

Simplified aponeurotic ptosis surgery.

I present a simplified aponeurotic ptosis repair that minimizes surgical dissection and enhances the predictability of postoperative eyelid positions. Indications for the procedure are unilateral ptosis, bilateral ptosis where preaponeurotic fat excision is not indicated, mild congenital ptosis, and reoperations from standard levator surgery. The procedure is best suited for surgeons who have a thorough knowledge of the levator aponeurosis anatomy.

Gastrocnemius musculotendinous rupture: a condition confused with thrombophlebitis.

Over a 24-year interval I have seen 22 cases of rupture of the medial head of the gastrocnemius muscle where it inserts into the soleus aponeurosis. The rupture may result in a swelling that is easily confused with thrombophlebitis and result in unnecessary hospitalization and anticoagulant therapy with its attendant expense and possible complications. The process is self-limiting and heals without sequelae in three to six weeks.

Re-evaluation of the orbicularis oculi muscle as revealed by the analysis of isolated muscle specimens.

PURPOSE: This study aimed to clarify the origin, insertion, and subdivision of the orbicularis oculi in detail with in situ specimens, as well as with isolated muscle specimens from the superficial and deep aspects. METHODS: The periorbital muscles in fifteen cadavers fixed with formalin were examined in situ and/or in isolated muscle specimens, and evaluate their actions. RESULTS: The orbicularis oculi arose via the nasoglabellar aponeurosis, which was observable in the deep aspect of isolated specimens as a rigid sheet of connective tissue in the middle of both orbits and adhered to the do…
