HLA B27 positive helicopter pilot with reactive arthritis responsive to sulfasalazine.

An aviator with prolonged right sided low back pain is described. The diagnostic workup is presented leading to diagnosis of HLA B27 positive reactive sacroiliitis. Trial of sulfasalazine lead to resolution of his pain. The spondylarthropathies are briefly reviewed. Implications on his aeromedical status are discussed.

C - reactive protein predicts proximal femur osteomyelitis in suppurative hip arthritis.

BACKGROUND: Ultrasound is capable of detecting hip joint effusion and guiding puncture procedures, which is cheap and convenient, but can not identify osteomyelitis. Magnetic resonance imaging (MRI) has high sensitivity and specificity for diagnosing osteomyelitis, but it is time-consuming and expensive. The purpose of this study was to find out certain indicators to identify suppurative hip arthritis with proximal femur osteomyelitis in order to optimize MRI. METHODS: Data of 48 children with acute suppurative arthritis of the hip diagnosed and treated from March 2018 to September 2024 were

Reactive Arthritis in Children: Case Report, Narrative Review and Proposed Therapy.

Reactive arthritis is an acute inflammatory aseptic arthritis that is preceded by an infectious process in genetically predisposed individuals. It has been associated with gastrointestinal or genitourinary infection. Reactive arthritis is rare in children. In this review, we present two index cases that need biologic treatment followed by a thorough review of reactive arthritis in children and adolescents with proposed treatment algorithm.

Acute Rheumatic Fever or Post-streptococcal Reactive Arthritis: Two Different Entities.

The authors present the case of a 71-year-old man with fever, polyarthralgia, and migratory arthritis after the diagnosis of pharyngitis one week earlier. The patient showed unequivocal evidence of recent group A streptococcal (GAS) infection with a significantly elevated antistreptolysin O (ASO) titer that normalized during follow-up. The symptoms, laboratory results, and clinical examination raised uncertainty about whether the patient was suffering from acute rheumatic fever (ARF) or post-streptococcal reactive arthritis (PSReA), as the patient exhibited characteristics of both conditions.

Reactive arthritis following COVID-19 current evidence, diagnosis, and management strategies.

BACKGROUND: Immune-mediated conditions associated to Corona Virus Disease-19 (COVID-19) have been reported, including vasculitis, antiphospholipid antibody syndrome, myositis, and lupus. Emerging studies have reported the potential occurrence of reactive arthritis in patients previously infected with COVID-19. This systematic review summarised the current evidence on the occurrence of reactive arthritis in patients previously infected by COVID-19. METHODS: This study was conducted according to the 2020 PRISMA guidelines. All the clinical investigations describing the occurrence of reactive ar

Reactive arthritis following COVID-19: clinical case presentation and literature review.

Reactive arthritis (ReA) is a clinical condition typically triggered by extra-articular bacterial infections and often associated with the presence of HLA-B27. While ReA has traditionally been associated with gastrointestinal and genitourinary infections, its pathogenesis involves immune and inflammatory responses that lead to joint affections. The emergence of COVID-19, caused by SARS-CoV-2, has prompted studies of plausible associations of the virus with ReA. We present a case of ReA in a patient who survived COVID-19 and presented with joint affections. The patient, a 31-year-old man, prese

A Case Report of Reactive Arthritis After COVID-19 AstraZeneca Vaccination.

In early 2021, multiple vaccinations for the coronavirus disease 2019 (COVID-19) in various immunological formulations were administered successfully to humans worldwide. Although numerous encountered side effects were expected, there were some effects that were unexpected. We report a case of a patient who experienced a rare occurrence of reactive arthritis in the right knee joint that manifested insidiously as pain, heat, and swelling on the second day following vaccination with the Oxford-AstraZeneca COVID-19 vaccine. The patient underwent a series of investigations, confirming the suspecte

Reactive oxygen species-responsive dendritic cell-derived exosomes for rheumatoid arthritis.

Although tolerogenic dendritic cell-derived exosomes (TolDex) have emerged as promising therapeutics for rheumatoid arthritis (RA), their clinical applications have been hampered by their poor in vivo disposition after systemic administration. Herein, we report the development of stimuli-responsive TolDex that induces lesion-specific immunoregulation in RA. Responsiveness to reactive oxygen species (ROS), a physiological stimulus in the RA microenvironment, was conferred on TolDex by introducing a thioketal (TK) linker-embedded poly(ethylene glycol) (PEG) on TolDex surface via hydrophobic inse

Reactive arthritis induced by active extra-articular tuberculosis: A case report.

RATIONALE: Rare cases of reactive arthritis induced by active extra-articular tuberculosis (Poncet disease) have been reported. Complete response to antitubercular treatment and evidence of active extra-articular tuberculosis are the most important clinical features of Poncet disease. We report the case of successfully treated a patient with reactive arthritis induced by active extra-articular tuberculosis with a TNF inhibitor after sufficient antitubercular treatment. PATIENT CONCERNS: A 56-year-old Japanese man was admitted to our department with polyarthralgia, low back pain, and high feve

Bilateral interstitial keratitis with anterior stromal infiltrates associated with reactive arthritis.

A previously healthy 48-year-old man presented with a 1-week history of migrating polyarthropathy preceded by a viral illness, dysuria and bilateral red eyes. Ocular examination revealed anterior and interstitial stromal keratitis. He was systemically well but had raised erythrocyte sedimentation rate and C reactive protein and was positive for human leucocyte antigen B27 on extensive infective, rheumatological and autoimmune investigations. Although the exact triggering pathogen was not identified, clinical findings were consistent with reactive arthritis. Bilateral interstitial keratitis is

Reactive oxygen species and serum antioxidant defense in juvenile idiopathic arthritis.

In autoimmune inflammatory diseases, including juvenile idiopathic arthritis (JIA), which leads to joint destruction, there is an imbalance between production of reactive oxygen species (ROS) and their neutralization which, as a consequence, leads to "oxidative stress." The aim of the study was to assess the concentration of oxidative stress markers: nitric oxide (NO), a degree of lipid membrane damage, and total antioxidant plasma capacity in children with JIA. Thirty-four children with JIA were included into the study. A degree of lipid membrane damage (lipid peroxidation products) was estim

Methicillin-resistant Staphylococcus aureus induced reactive arthritis treated with systemic corticosteroid.

Reactive arthritis consists of the classic clinical triad of arthritis, urethritis and conjunctivitis generally occurring within 6 weeks of an infection, typically of the gastrointestinal or genitourinary systems. Staphylococcus aureus is not usually implicated in this condition. Staphylococcal septicemia, while frequently associated with arthralgia, has rarely been associated with a sterile arthritis, although it infrequently results in septic arthritis. It is important to consider reactive arthritis, rather than solely an infectious cause of joint effusions, and arthropathy in a patient with
