[Mechanisms of bronchial hyperreactivity. Bronchial edema, mechanical and vascular factors].

Hindrance to gas flow in the bronchi is affected not only by airway smooth muscle tone but also by airway circulation. Congestion and oedema increase airway wall thickness and act in series with airway smooth muscle contraction to reduce airway calibre, an effect which is more marked in small and intermediate bronchi. Many mediators, neuromediators, paracrine mediators produced by resident (epithelium) or migrant (inflammatory cells) cells share bronchomotor and vascular effects. In addition, contraction of airway smooth muscle and vascular phenomena are mechanically coupled. Contraction of ai

An Adolescent With Progressive Shortness of Breath.

Shortness of breath and wheezing are common presenting signs for children in the emergency department. In adolescence, it is often due to asthma or lower respiratory tract infections. We present a rare pediatric case of an adolescent with biphasic stridor and progressive exercise-induced shortness of breath who was found to have severe idiopathic subglottic stenosis.

Targeted metabolomic analysis of exhaled breath condensate before and after bronchial provocation by eucapnic voluntary hyperventilation in adolescents with asthma: an exploratory study.

OBJECTIVE: To analyze endogenous metabolites in exhaled breath condensate before and after eucapnic voluntary hyperventilation (EVH) in adolescents with asthma with and without exercise-induced bronchoconstriction (EIB). METHODS: This was an exploratory study including 44 adolescents with asthma in the 10- to 20-year age bracket. They were divided into two groups: the group of patients with EIB (n = 25) and that of those without (n = 19). EIB was defined as a ≥ 10% reduction in FEV1 after a six-minute EVH challenge using 5% carbon dioxide. Ventilation was maintained at 21 times the baseline F

Efficiency and limitations of the upper airway mucosa as an air conditioner evaluated from the mechanisms of bronchoconstriction in asthmatic subjects.

To elucidate a limit to the efficiency of the upper airway mucosa as an air conditioner, the temperatures of the inspiratory air and mucosa were measured in the cervical trachea. Both of them were affected only minimally by change of atmospheric air temperature during resting nose breathing, but were affected greatly by change of mode of breathing. During hyperventilation through the mouth, when the atmospheric air temperature was 1 degree C, a temperature difference of 9 degrees C was noted between inspiratory air in the cervical trachea and body temperature, together with a mucosal temperatu

[Clinical forms and lung physiology changes in children with bronchial asthma].

The clinical findings, allergoimmunological results and lung function data of 208 asthmatic children (aged 4-18 years), admitted to the Pediatric Outpatient Department were investigated. In respect to the case history and the clinical findings the patients were divided into 5 etiopathogenetic groups including infectious asthma (6%), seasonal asthma (29%), exogenous perennial asthma (56%), intrinsic asthma (4%) and exercise-induced asthma (5%). All patients presented with increased airway resistance measured plethysmographically (Raw greater than 120% predicted) as indicator of bronchial hyperr

Effect of an intranasally administered corticosteroid (budesonide) on nasal obstruction, mouth breathing, and asthma.

The effect of intranasally administered corticosteroid (budesonide) on nasal symptoms, mode of respiration (nasal versus mouth breathing), and asthma was investigated in 37 asthmatic children who were mouth breathers because of chronic nasal obstruction. After a 2-wk run-in period, the children were allocated randomly to 4 wk of intranasal therapy with either budesonide (400 micrograms/day) or placebo spray. A double-blind, parallel design was used. Diaries for peak expiratory flow, asthma, and rhinitis symptom scores and degree of mouth breathing were recorded at home. Nasal eosinophilia, nas

Inducible laryngeal obstruction during exercise: moving beyond vocal cords with new insights.

Exercise as an important part of life for the health and wellness of children and adults. Inducible laryngeal obstruction (ILO) is a consensus term used to describe a group of disorders previously called vocal cord dysfunction, paradoxical vocal fold motion, and numerous other terms. Exercise-ILO can impair one's ability to exercise, can be confused with asthma, leading to unnecessary prescription of asthma controller and rescue medication, and results in increased healthcare resource utilization including (rarely) emergency care. It is characterized by episodic shortness of breath and noisy b

Which pulmonary function tests best differentiate between COPD phenotypes?

We are still at the early phase of finding useful phenotypes in COPD that can guide therapy. However, in a simple sense, "sick patients die." Many phenotypic measurements of severity correlate with mortality in COPD: FEV(1), the ratio of inspiratory capacity to total lung capacity (IC/TLC), diffusing capacity of the lung for carbon monoxide (D(LCO)), 6-min walk distance, and maximum oxygen (O(2)) consumption or maximum watts on exercise testing. However, composite parameters, such as the BODE index (body mass index, air flow obstruction, dyspnea, exercise capacity), perform better, likely beca

Prognostic bases of asthma. Natural history?

Different causes of asthma have been established. The most common cause is conditioned to a genetic predisposition towards atopy (atopic asthma), although other factors can also give rise to bronchial inflammation, such as over-exposure to environmental irritants (occupational asthma), altered arachidonic acid metabolism (aspirin-induced asthma) and also exercise - in which different thermal and osmotic mechanisms are known to intervene. The prognosis of these different variants of asthma depends on the severity of the condition; patient age at onset of the disease; patient age at the time of

Exercise-induced anaphylaxis: a serious but preventable disorder.

UNLABELLED: Described for the first time approximately 30 years ago, exercise-induced anaphylaxis is a rare disorder characterized by development of a severe allergic response occurring after mild-to-strenuous physical activity. This disorder is especially important to recognize with the recent increase in physical activity and health fitness fads. A number of predisposing factors (eg, prior ingestion of particular food groups) linked to exercise-induced anaphylaxis has been outlined over the years. Mechanisms governing the condition are still being unveiled, and it is likely that one mechanis

Role of bronchoprovocation tests in identifying exercise-induced bronchoconstriction in a non-athletic population: a pilot study.

BACKGROUND: Studies on diagnostic tests for exercise-induced bronchoconstriction (EIB) have centered around the asthmatic and elite athletic population. Traditionally, the exercise challenge test (ET) was recommended to assess EIB. We aimed to compare the performance of surrogate testing, mainly the hypertonic saline (HS) test, and methacholine challenge test (MCT) versus ET in identifying EIB among non-athletic subjects. METHODS: We prospectively recruited subjects who did not have confirmed active asthma, but who reported exercise-induced dyspnoea. The participants underwent HS and ET on se

Chapter 9: Asthma classification.

Asthma is a chronic inflammatory disorder of the airways resulting physiologically in hyperreactivity and clinically in recurrent episodes of wheezing, chest tightness, or coughing. Airway inflammation, smooth muscle contraction, epithelial sloughing, mucous hypersecretion, bronchial hyperresponsiveness, and mucosal edema contribute to the underlying pathophysiology of asthma. Diagnostic tests such as methacholine or mannitol challenges or spirometry (pre- and postbronchodilator responses) help to identify such underlying pathophysiology via assessments of bronchial hyperreactivity and lung me

Exercise increases sulfur dioxide-induced bronchoconstriction in asthmatic subjects.

We undertook a study to determine whether moderate exercise modifies the bronchoconstriction produced by sulfur dioxide (SO2) in subjects with mild asthma. In 7 subjects, we compared the changes in specific airway resistance (SRaw) produced by 10 min of exercise alone (400 kpm/min on a cycle ergometer), inhalation of SO2 alone, and the combination of exercise and SO2. During all studies, a subject breathed SO2 and/or air from a mouthpiece. In 6 additional subjects, we compared the increase in SRaw produced by inhalation of SO2 during exercise with that produced by eucapnic hyperventilation wit

Moderating the inflammation of asthma: inhibiting the production or action of products of the 5-lipoxygenase pathway.

The products of the 5-lipoxygenase pathway of arachidonic acid metabolism, particularly the leukotrienes, can mediate bronchoconstriction, mucous secretion, airway mucosal edema, chemotaxis, and mobilization of cells into the airway in the inflammatory process of asthma. This paper will review the current data on the efficacy of available leukotriene antagonists or inhibitors that act on the 5-lipoxygenase pathway in induced bronchoconstriction in humans and in clinical studies in asthma. Studies of asthmatic responses to antigen and cold air hyperventilation and exercise will be discussed. Da

Asthma, allergy and the Olympics: a 12-year survey in elite athletes.

OBJECTIVE: There are no comprehensive surveys relating the reported high prevalence of asthma and allergic diseases in athletes to comorbidities and immune changes associated with intense chronic exercise. This 12-year survey aims to evaluate several clinical, functional and immunological parameters in order to assess features, trend and burden of asthma, allergy, infections and autoimmune diseases, in a large homogeneous population of Olympic athletes. METHODS: Six hundred and fifty-nine Italian Olympic athletes were studied through four cross-sectional surveys performed between 2000 and 201

Goalie Phenomenon: Unveiling Hidden Health Conditions in Young Athletes.

Jaradat et al. (2025) first describe the goalie phenomenon in youth sports in which children with undiagnosed asthma gravitate toward less physically demanding positions, such as goalkeepers, to avoid exercise-induced respiratory symptoms like breathlessness, wheezing, or coughing. This compensatory behavior may inadvertently mask underlying health conditions, underscoring the importance of clinical vigilance and early diagnostic assessment in pediatric athletes.

Timeliness of diagnosis of asthma in children and its predictors.

BACKGROUND: There is a paucity of literature using medical records to evaluate the timeliness of asthma diagnosis in children and the predictors associated with timeliness of asthma diagnosis. METHODS: Subjects were obtained from a convenience sample of 839 children, aged 5-13 years. We conducted comprehensive medical record reviews for these children to determine their asthma status by applying predetermined criteria for asthma. Predictors were evaluated for an association with timeliness of asthma diagnosis. RESULTS: Of 839 children, 276 children met the criteria for asthma before 18 years

Exercise-Induced Bronchospasm and Allergy.

Sport is an essential part of childhood, with precious and acknowledged positive health effects but the impact of exercise-induced bronchoconstriction (EIB) significantly reduces participation in physical activity. It is important to recognize EIB, differentiating EIB with or without asthma if the transient narrowing of the airways after exercise is associated with asthmatic symptoms or not, in the way to select the most appropriate treatment among the many treatment options available today. Therapy is prescribed based on symptoms severity but diagnosis of EIB is established by changes in lung
