BACKGROUND: Rotational atherectomy (RA) is an acknowledged method of percutaneous treatment of highly calcified or fibrotic coronary lesions. However, using the rotablator system in patients presenting with acute coronary syndromes (ACS) remains controversial and is considered as a relative contraindication. The aim of our study was to assess in-hospital and 1-year outcomes in patients undergoing RA presenting with ACS, in comparison to elective RA procedures.
METHODS: This single-center observational study included all consecutive patients who underwent RA and PCI in our institution from Apr…

Percutaneous transluminal coronary rotational atherectomy (PTCRA) is an established treatment forvery calcified and/or undilatable coronary lesions. In contrast to data from directional coronary atherectomy (DCA) procedures, coronary artery aneurysms (CAA) or pseudoaneurysms have not routinely been described as a complication after PTCRA. We present a case of a 74-year-old patient, with a major CAA, found on a routine control coronary angiogram 4 months after a PTCRA of LAD complicated by a coronary perforation. A conservative management guided by coronary CT angiography is proposed.

We report the case of a 53-year-old white man who began complaining of dyspnoea and angina 19 months after coronary artery bypass graft surgery. Coronary angiography revealed the presence of a long and critical stenosis in the native left anterior descending coronary artery, shortly after distal anastomosis of the left internal mammary artery. After failed predilatations with standard or cutting balloons, we successfully used the rotablator system, which allowed us to implant a bare-metal stent in the native left anterior descending coronary artery. However, stent deployment caused long linear

The role of directional coronary atherectomy (DCA) in interventional cardiology remains uncertain. We report the Northern New England regional experience with DCA from 1991 to 1994. Data were collected on 11,178 patients having had an intervention on a single lesion in a single vessel (798 DCAs; 10,380 percutaneous transluminal angioplasties [PTCA]). The use of DCA increased from 1.8% of interventions in 1991 to 10% in 1994. Compared with PTCA, DCA patients were younger, more often men, had more 1-vessel disease and more coronary artery bypass surgery (CABG). DCA was more often used in the lef…

Extraction atherectomy utilizes suction aspiration as an attempt to limit distal emboli during atherectomy. We sought to test the hypothesis that extraction atherectomy produces less distal embolization than balloon angioplasty when treating saphenous vein grafts. Among 163 consecutive, nonrandomized patients, 103 patients underwent transluminal extraction catheter (TEC) atherectomy with or without adjunctive balloon angioplasty, and 60 patients had conventional balloon angioplasty. Both groups showed comparably high procedural success rates (TEC 90.3%, angioplasty 83.3%, P = NS). TEC cases ha…

Although directional atherectomy (DA) reduces the plaque burden, successful revascularization is not associated with a reduced restenosis rate when compared with percutaneous transluminal coronary angioplasty (PTCA). The purpose of this study was to compare and contrast the vascular response to DA-induced and PTCA-induced injury. Six to 8 weeks after induction of atherosclerosis, PTCA (n = 34) was performed in one iliac artery and DA in the other (n = 30). Arteries were obtained at 6 time points: 1, 3, 5, 7, 14, and 28 days. Eleven arteries that did not undergo an intervention acted as control…

BACKGROUND: Excimer laser coronary atherectomy (ELCA) to modify or debulk atherosclerotic plaque during percutaneous coronary intervention (PCI) has been in clinical use for the past four decades. Primarily, ELCA was introduced to mitigate the challenges posed by routine PCI for complex lesions and in crossing resistant lesions.
AIMS: The aim of the present study was to evaluate the efficacy and safety of ELCA in complex coronary lesions.
METHODS: This study included 71 consecutive patients who underwent treatment with ELCA-assisted PCI for complex coronary lesions. The data were analysed fo…

BACKGROUND: Coronary artery calcification (CAC) is frequently observed in the ageing population, affecting 90% of men and 67% of women, and poses significant challenges during percutaneous coronary interventions (PCIs). CAC impedes stent passage, compromises drug delivery, and limits stent expansion, contributing to decreased PCI efficacy and heightened risks of complications such as restenosis, thrombosis, and procedural issues.
AIMS: The study aims to evaluate the real-world clinical outcomes of orbital atherectomy (OA) in patients with severe CAC undergoing PCI.
METHODS: This retrospectiv…

Evaluation of calcified lesions by intravascular imaging has revealed that atherectomy devices have only limited impact. However, subsequent use of coronary intravascular lithotripsy (IVL) may increase treatment effectiveness without increasing risk of complications. This study was designed to evaluate the safety and effectiveness of IVL use after atherectomy in severely calcified coronary lesions as pre-treatment for drug-eluting stents (DES). The Dual-Prep registry is a multicenter, prospective registry of consecutive image-guided percutaneous coronary interventions (PCI). The primary effect…

INTRODUCTION: Coronary artery calcification can lead to suboptimal results when performing coronary angioplasty with conventional techniques. The presence of severe coronary artery calcium increases the complications of percutaneous coronary intervention as it may impede stent delivery and optimal stent expansion. The purpose of this study was to determine the procedural success and safety of orbital atherectomy (OA) in calcified lesions.
MATERIALS AND METHODS: This was a prospective single-centre study regarding the utility of OA in the treatment of calcified coronaries. Intravascular ultras…

PURPOSE: Optimal balloon angioplasty for infrapopliteal lesions is often limited by severe calcification, which has been associated with decreased procedural success and lower long-term patency.
MATERIALS AND METHODS: This was a prospective, randomized, multicenter pilot trial that included adult subjects with calcified lesions located from the popliteal segment below the knee (BTK) joint to within 5 cm above the ankle with ≥70% diameter stenosis by angiography. Patients were randomized 1:1 to undergo orbital atherectomy (OA) with adjunctive drug-coated balloon (DCB) angioplasty versus plain …
