[Effect of etmozin on the atrioventricular conduction system function of the heart in paroxysmal subventricular tachycardia].

With the aid of intracavitary electrophysiological investigation in hospital the influence of ethmosin, an antiarrhythmic agent, on the function of atrio-ventricular conducting system was studied in 52 patients with paroxysmal supraventricular tachicardias. It was established that the drug depresses significantly the electrophysiological values of Kent's bundle, the His-Purkinje system and atrioventricular node (mainly in the retrograde conduction): and also prolongs considerably the time of the intra-atrial conduction. Ethmosin exerts practically no influence on the refractory periods of the …

Electrocardiography of Atrioventricular Block.

Delayed atrioventricular (AV) conduction most commonly occurs in the AV node, resulting from AH prolongation on an intracardiac electrocardiogram and PR prolongation on a surface electrocardiogram. AV conduction may be blocked in a 2:1 manner, with a normal PR interval and wide QRS suggesting infranodal disease, whereas a prolonged PR interval and narrow QRS are more suggestive of AV nodal disease. Block within the His is suspected when there is 2:1 AV block with normal PR and QRS intervals. Complete heart block occurs when the atrial rhythm is totally independent of a junctional or lower esca

Ablation of AVNRT in a Setting of Slow Pathway-Dominant Conduction.

BACKGROUND: We report a case of paroxysmal supraventricular tachycardia with slow pathway-dominant conduction and prolonged PR interval in sinus rhythm. CASE SUMMARY: A 33-year-old man presented with persistent palpitations and chest tightness, with narrow QRS tachycardia with a prolonged PR interval on electrocardiogram. After the administration of isoproterenol, the PR interval normalized. During the electrophysiology study, atrioventricular node conduction jumping, echoes, and dual ventricular responses were observed. After ablation, the fast pathway function of the atrioventricular node w…

[Cryoablation for cardiac arrhythmias].

The development of radiofrequency ablation has revolutionized the treatment of certain arrhythmias such as supraventricular tachycardia. Rad … ck. This technique is therefore especially beneficial if ablation is planned in the vicinity of the atrioventricular node. This paper describes this new procedure along with the experience with the first cases of cryoablation in Iceland.

Mechanisms of AV node modulation in AV node reentry tachycardia.

Atrioventricular nodal reentrant tachycardia has been recognized for many years as a very common cause of supraventricular tachycardia. First curable by surgery, this tachycardia is now successfully approached by selective radiofrequency current application to the slow pathway of the reentrant circuit. Importantly, these curative treatments brought evidence that the reentrant circuit of this arrhythmia was not confined to the compact atrioventricular (AV) node. These findings stimulate anatomists and basic and clinical electrophysiologists to fundamentally reconsider the atrioventricular junct

[Transient total atrioventricular dissociation induced by junctional premature depolarization].

The authors describe a case of temporary complete atrioventricular dissociation in prognostically less serious conduction disorders in the area of the atrioventricular node manifested clinically as an atrioventricular block grade 1. Dissociation episodes were induced by premature junctional depolarization and maintained, thanks to retrograde latent conduction into the area of the atrioventricular junction and the small difference of frequencies of the sinus and ectopic substitution rhythm.

Clinically oriented device programming in bradycardia patients: part 2 (atrioventricular blocks and neurally mediated syncope). Proposals from AIAC (Italian Association of Arrhythmology and Cardiac Pacing).

: The purpose of this two-part consensus document is to provide specific suggestions (based on an extensive literature review) on appropriate pacemaker setting in relation to patients' clinical features. In part 2, criteria for pacemaker choice and programming in atrioventricular blocks and neurally mediate syncope are proposed. The atrioventricular blocks can be paroxysmal or persistent, isolated or associated with sinus node disease. Neurally mediated syncope can be related to carotid sinus syndrome or cardioinhibitory vasovagal syncope. In sinus rhythm, with persistent atrioventricular bloc

A new mechanism for atrioventricular nodal gap-vagal modulation of conduction.

The well-known paradoxic behavior of atrioventricular conduction, the so-called gap phenomenon, that occurs when impulses within a certain range of coupling intervals are blocked while impulses with shorter coupling intervals are conducted is attributed to differences in properties of refractoriness in neighboring regions of the conduction system. In contrast, in the present study a model was developed showing a similar phenomenon, dependent on different electrophysiologic mechanisms and localized within the atrioventricular node in an isolated rabbit heart tissue preparation (n = 11). The hea…

Radiofrequency Ablation of the Distal Vein of Marshall Prior to Ethanolization.

CASE SUMMARY: A woman with atrial fibrillation underwent radiofrequency ablation combined with vein of Marshall (VOM) ethanol infusion. To prevent ethanol-induced sinus node injury via the distal VOM, we performed targeted radiofrequency ablation to occlude the distal VOM before ethanol infusion. No sinus arrest or atrioventricular block occurred. Pulmonary vein isolation was successfully achieved, and atrial fibrillation terminated during the procedure. TAKE-HOME MESSAGES: Sinus node and atrioventricular node injury are rare but critical complications of VOM ethanol infusion. Preemptive occl…

['Uninterrupted' anterograde atrioventricular excitation conduction in patients with paroxysmal reciprocal atrioventricular nodal tachycardias].

Different variants of anterograde atrioventricular conduction of excitation were distinguished basing on data obtained from 150 patients with paroxysmal reciprocal atrioventricular nodal tachycardias. Excitation conduction during programmed and accelerating atrial transesophageal pacing allowed to assess characteristics of excitation conduction along fast and slow conduction pathways in atrioventricular node. Some parameters were suggested that allowed to predict possibility of detection of different variants of anterograde atrioventricular conduction of excitation. Electrophysiological mechan…

Atriofascicular connection or a nodoventricular Mahaim fiber? Electrophysiologic elucidation of the pathway and associated reentrant circuit.

Accessory pathways showing decremental properties and inserting into the right ventricle have been frequently described as "nodoventricular" … n over the accessory pathway and retrograde conduction via the bundle branches, His bundle, and the atrioventricular node. More significantly, late atrial stimuli delivered during tachycardia could preexcite the ventricle via the accessory pathway despite their inability to enter the atrioventricular node. Thus, the upper "turn around" of the reentrant circuit involved …

Simplified primum ASD closure technique in complete VSD repair: shallow suture crossing on the AV node.

The primum atrial septal defect suture line adjacent to the crux is performed carefully with various techniques to avoid atrioventricular block in repair of complete atrioventricular septal defect. We describe our technical modification to simplify the shallow suture line only into the endocardium above the atrioventricular node without conduction disturbance.

Reentrant junctional tachycardias.

Most of the tachycardia arising in the atrioventricular (AV) junction are reentrant in nature. The two most common variety are AV nodal reentry and AV reentry utilizing an accessory pathway of the Kent bundle type. Typically these tachycardias have narrow QRS complex and are regular but an associated right or left bundle branch block could result in a wide QRS complex. Other mechanisms for wide QRS in AV junctional tachycardia include: (a) antidromic reentry; (b) preexcited tachycardia using two accessory pathways; (c) AV nodal reentry with incidental accessory pathway participation; and (d) a

Electrophysiological Testing for the Investigation of Bradycardias.

In this article we review the role of electrophysiological testing in patients presenting with bradycardia due to sinus node or atrioventricular node disease. In sinus bradycardia the role of electrophysiology studies is not established. In AV conduction disturbances, an electrophysiology study may be necessary both for the establishment of atrioventricular block as the main cause of symptoms, and for identification of the anatomic site of block that may dictate the potential need of permanent pacing.

Atrioventricular nodal function in progressive systemic sclerosis: electrophysiological and morphological findings.

Electrophysiological data consistent with atrioventricular nodal dysfunction were obtained in 10 out of 19 patients with progressive systemic sclerosis (scleroderma). Conducting system studies were carried out in a further seven patients with progressive systemic sclerosis. In each the proximal portion of the atrioventricular node was consistently found to be smaller and more slender. It is postulated that there is a relation between proximal atrioventricular nodal structural alterations and impaired atrioventricular nodal function.

[Wolff-Parkinson-White syndrome with orthodromic supraventricular tachycardia associated with a "hyper-conductor" atrio-ventricular node. A therapeutic challenge].

One case of Wolff-Parkinson-White Syndrome with paroxysmal supraventricular tachycardia related to orthodromic atrioventricular reentry using an accessory pathway for retrograde conduction an a rapidly conducting AV node for anterograde conduction is present. The pharmacological therapy with Digoxin, Propranolol, Quinidine, Disopyramide and Propafenone was not effective. An electrophysiologic study showed a reciprocating tachycardia induced by spontaneous ventricular beats. Both the effective refractory period of the AV node and the anterograde effective refractory period of the accessory path

The therapeutic and diagnostic cardiac electrophysiological uses of adenosine.

Adenosine is a purine nucleoside with a rapid onset and brief duration of action after intravenous bolus administration. Its most prominent cardiac effect is impairment or blockade of atrioventricular nodal conduction, but other effects are depression of automaticity of the sinus node and attenuation of catecholamine-related ventricular after-depolarizations. The cardiac cell surface receptor is the A1 purinoceptor. The therapeutic value of adenosine is predominantly in those arrhythmias in which the atrioventricular node forms part of a reentry circuit, as clearly demonstrated by the high suc…

[Atrioventricular block at the nodal level and block of an accessory pathway during percutaneous mitral valvuloplasty].

We describe the simultaneous occurrence of a temporal complete atrioventricular block and a persistent disappearance of preexcitation, in a patient with rheumatic mitral stenosis and a ventricular preexcitation, after an effective percutaneous mitral valvuloplasty. These conduction disturbances were attributed to atrioventricular node and paraseptal atrioventricular pathway trauma, due to transseptal manipulation during the mitral valvuloplasty.
