Spondylolysis of the axis. A case report and review of the literature.

Bilateral congenital defects in the pedicles of the second cervical vertebra were noted on conventional X-rays and tomography of a 42-year-old male. He had been assaulted after which he suffered upper neck and occipital pain. Subsequent CT scanning demonstrated the incomplete nature of the axis defects. This is felt to be important in prognosticating the likelihood of spondylolisthesis formation and counselling the patient with regard to acceptable future physical activity. The role of CT examination for this purpose has not been stressed before. The importance of differentiation from old trau

Acute traumatic atlas fractures: management and long term outcome.

Fractures of the 1st cervical vertebra (C1) represent 7% of all acute cervical spine fractures. Isolated atlas fractures are most commonly bilateral or multiple fractures through the ring of C1. Frequently (44% of cases), the atlas will be fractured in combination with the axis. Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute atlas fractures is reviewed. Nonoperative external immobilization was used in 53 patients (with 1 failure)

Three-dimensional assessment of the intervertebral kinematics after Mobi-C total disc replacement at the cervical spine in vivo using the EOS stereoradiography system.

BACKGROUND: Because 3-dimensional computed tomography and magnetic resonance imaging analysis of the spinal architecture is done with the patient in the supine position, stereoradiography may be more clinically relevant for the measurement of the relative displacements of the cervical vertebrae in vivo in the upright position. The innovative EOS stereoradiography system was used for measuring the relative angular displacements of the cervical vertebrae in a limited population to determine its feasibility. The precision and accuracy of the method were investigated. METHODS: In 9 patients with

[Roles of posterior parietal cortex in stereopsis: characteristics of responses of axis-orientation-selective neurons in monkey caudal intraparietal area].

Источник пометил публикацию как отозванную

Patients with parietal lesions may fail to adjust the orientation of their hand to that of a target object, or may make errors in judging the orientation of a bar. This suggests that the parietal cortex has a function in the discrimination of the orientation of objects. In this study we investigated the responses of axis-orientation-selective neurons in caudal intraparietal (CIP) area to stereoscopic stimuli. Among the characteristics we investigated responses to the length and thickness of objects, sensitivity to binocular disparity, and position invariance in depth. Computer generated stereo

Harris or axis ring: an aid in diagnosing low (type 3) odontoid fractures.

OBJECTIVE: To present our experience of diagnosing fractures of the odontoid process on lateral radiographs of the cervical spine that show the Harris (axis) ring. DESIGN: Retrospective study. SETTING: Teaching hospital, Belgium. SUBJECTS: 12 patients with multiple injuries, including cervical spine, 8 of them unconscious or uncooperative. INTERVENTIONS: Cross table lateral view of the cervical spine. MAIN OUTCOME MEASURE: Identification of otherwise hidden type 3 axial fractures. RESULTS: Diagnosis of low odontoid fractures in all cases. CONCLUSION: The Harris ring is disrupted in low

[Long-term prognosis of the hangman's fracture].

23 patients with traumatic spondylolisthesis of CII (hangman's fracture), all treated conservatively by external immobilisation and/or bed rest, were followed for an average of 9.6 years, 10 of them for 10-20 years. Radiographically two groups could be distinguished: one with bone formation between the vertebrae of CII and CIII and another without bone formation. Irrespective of the amount of bone formation their long-term prognosis was good: no new symptoms, old symptoms diminished and no limitation of neck movements. Degenerative changes in the neighbouring motor segments were not observed.

Fractures of the craniovertebral junction associated with other fractures of the spine: overlooked entity?

In a review of 155 craniovertebral fractures (occiput-C1-C2), 40 of these had associated fractures and/or dislocations or subluxations elsewhere in the spine. This rather common occurrence, one of four, has not been emphasized in the recent literature, indicating that the radiologic examination should not stop after the craniovertebral fracture is identified. Furthermore, in 13 patients, neurologic deficits were encountered that in all instances were from associated lower-level fracture. From this experience it was believed that a minimum of anteroposterior and lateral views of the entire spin

Atypical hangman's fractures.

Nineteen cases of traumatic spondylolisthesis of the axis, including 13 standard hangman's fractures and 6 anterolistheses of the C2 vertebral body associated with Effendi's atypical hangman's fractures, were studied. Unlike the standard Effendi Type I or Type II fractures, atypical hangman's fractures, occurring through the posterior aspect of the vertebral body with unilateral or bilateral continuity of the posterior cortex or pedicle, routinely narrow the spinal canal because of the fracture pattern and degree of subluxation. These atypical C2 injuries were more frequent and more often acco

Traumatic spondylolisthesis of the axis.

A prospective study of 39 patients with traumatic spondylolisthesis of the axis is reported. Classification of the fracture was based on the criteria of White and Punjabi (1978). In previous studies (Effendi et al., 1981; Levine and Edwards, 1985) stability of the undisplaced and slightly displaced fracture was based on flexion/extension radiographs. Union was obtained in all fractures without using a rigid cervical orthosis. We feel that assessing stability dynamically soon after injury when there is marked muscle spasm does not provide an accurate assessment, nor does it not alter the manage

Airway compromise secondary to upper cervical spine injury.

BACKGROUND: Airway compromise secondary to isolated injury at the atlas (C1) and axis (C2) without an associated spinal cord injury is a rare, but recognized phenomenon that results in significant morbidity and mortality. No previous study in the literature has reported the incidence of this potentially lethal complication of these relatively common fractures. METHODS: The medical records for 625 consecutive patients who presented to a Level I trauma center with C1 and C2 fractures during the years from 1996 to 2005 were reviewed retrospectively. Strict inclusion and exclusion criteria were a

Screw fixation of the upper cervical spine in the pediatric population. Clinical article.

OBJECT: Rigid fixation of the upper cervical spine has become an established method of durable stabilization for a variety of craniocervical pathological entities in children. In children, specifically, the use of C1-2 transarticular screws has been proposed in recent literature to be the gold standard configuration for pathology involving these levels. The authors reviewed the use of rigid fixation techniques alternative to C1-2 transarticular screws in children. Factors evaluated included ease of placement, complications, and postoperative stability. METHODS: Seventeen patients, ranging in

Upper cervical spine injuries.

The unique anatomy of the upper cervical spine and the typical mechanisms of injury yield a predictable variety of injury patterns. Traumatic ligamentous injuries of the atlanto-occipital joint and transverse atlantal ligament are relatively uncommon, have a poor prognosis for healing, and often respond best to surgical stabilization. Bony injuries, including occipital condyle fractures, atlas fractures, most odontoid fractures, and traumatic spondylolisthesis of the axis, generally respond well to nonsurgical management. Controversy in management remains, however, especially with type II odon

Segmental relationship between somites and vertebral column in zebrafish.

The segmental heritage of all vertebrates is evident in the character of the vertebral column. And yet, the extent to which direct translation of pattern from the somitic mesoderm and de novo cell and tissue interactions pattern the vertebral column remains a fundamental, unresolved issue. The elements of vertebral column pattern under debate include both segmental pattern and anteroposterior regional specificity. Understanding how vertebral segmentation and anteroposterior positional identity are patterned requires understanding vertebral column cellular and developmental biology. In this stu

First evidence of spinal arthropathy and congenital block of the cervical vertebrae in Temnospondyli.

Despite recent reports, the fossil record of pathologies in vertebrates is still patchy in terms of phylogeny, geological age and paleogeography. Here, we report several cases of pathological vertebrae in several specimens of the Late Triassic temnospondyl Metoposaurus krasiejowensis from Krasiejów. The presence of cervical block vertebrae (atlas-axis complex) is the oldest such case reported for any fossil tetrapod and the first such case reported in temnospondyls. Spinal arthropathy (or spinal arthritis) is the first such case for a non-amniote. Based on different levels of fusion among the
