Postoperative metabolic and nutritional complications of bariatric surgery.

Bariatric surgery has become an increasingly important method for management of medically complicated obesity. In patients who have undergone bariatric surgery, up to 87% with type 2 diabetes mellitus develop improvement or resolution of their disease postoperatively. Bariatric surgery can reduce the number of absorbed calories through performance of either a restrictive or a malabsorptive procedure. Patients who have undergone bariatric surgery require indefinite, regular follow-up care by physicians who need to follow laboratory parameters of macronutrient as well as micronutrient malnutriti

Ascorbic acid deficiency in bariatric surgical population.

BACKGROUND: To determine the prevalence of ascorbic acid deficiency in the surgical population, whether the body mass index (BMI) has an effect on ascorbic acid concentrations; and whether an association exists between ascorbic acid deficiency and adverse surgical outcomes. METHODS: Preoperative plasma ascorbic acid concentrations were prospectively assessed in 20-60-year-old patients undergoing elective abdominal surgery. Ascorbic acid deficiency was defined as any concentration < or =0.3 mg/dL and depletion as any concentration >0.3-0.59 mg/dL. RESULTS: Of the 266 patients evaluated, 167 h

Post-bariatric abdominoplasty: skin sensation evaluation.

BACKGROUND: The number of bariatric surgeries is progressively increasing in the USA and in Brazil. The number of post-bariatric plastic surgeries also increases as a response to this phenomenon. Abdominoplasties performed in former morbidly obese patients present a larger number of postoperative complications. Studies show that abdominoplasty caused postoperative skin hypesthesia. This study aims at evaluating skin sensibility/sensation in post-bariatric patients submitted to anchor-line abdominoplasty with clinical, qualitative, reliable and reproductive methods. METHODS: Thirty-nine former

[Can bariatric surgery cure metabolic syndrome?].

OBJECTIVES: To evaluate the impact of bariatric surgery on the metabolic syndrome (MS) and what are the criteria that contribute the most for its exclusion after surgery. The progress of leucometry was also analyzed. METHODS AND PROCEDURES: 47 obese women with MS were evaluated. All patients were operated with the Roux-en-Y vertical gastric bypass technique, with the insertion of a contention ring on the gastro-jejune anastomosis (Fobi-Capella). Patients were evaluated before and on the first year post-surgery. RESULTS: Fasting glucose presented a relevant decrease at 3 months after surgery.

[Nutritional implications of bariatric surgery on the gastrointestinal tract].

Anatomical change in the anatomy of the gastrointestinal tract after bariatric surgery leads to modification of dietary patterns that have to be adapted to new physiological conditions, either related with the volume of intakes or the characteristics of the macro- and micronutrients to be administered. Restrictive diet after bariatric surgery (basically gastric bypass and restrictive procedures) is done at several steps. The first phase after surgery consists in the administration of clear liquids for 2-3 days, followed by completely low-fat and high-protein content (> 50-60 g/day) liquid diet

Impact of bariatric surgery on type 2 diabetes.

The management and prevention of diabetes through lifestyle modifications and weight loss should be the mainstay of therapy in appropriate candidates. Although the results from the Diabetes Prevention Trial and the Finnish Prevention Study support this approach, over 95% of patients not participating in a prevention research study are unable to achieve and maintain any significant weight loss over time. Bariatric surgery for weight loss is an emerging option for more sustainable weight loss in the severely obese subject, especially when obesity is complicated by diabetes or other co-morbiditie

Predictive factors for rhabdomyolysis after bariatric surgery.

BACKGROUND: Prevalence of obesity is increasing internationally. Obesity is also incriminated in the development of postoperative rhabdomyolysis (RML). Its major risk is the development of renal failure which is associated with high mortality. The aim of this study was to determine the risk factors for RML in patients undergoing bariatric surgery. METHODS: Over a 12-month period, 49 consecutive patients were studied. They underwent gastric banding (n=32) or intestinal or gastric bypass (n=17) for morbid obesity (BMI >40 kg/m(2)). Surgery was performed in the supine position with a lumbar pad

Outcome of portal injuries following bariatric operations.

BACKGROUND: Portal vein thrombosis is rare following Roux-en-Y gastric bypass (RYGBP). Its natural history is dependent on the etiology of the thrombosis. Iatrogenic injuries at bariatric operations resulting in portal vein thrombosis are lethal complications typically necessitating a liver transplant, whereas postoperative portal vein thrombosis without an injury to the portal vein has a benign course. There are currently no data on management or prognostic factors of portal vein thrombosis after bariatric operations. METHODS: 3 patients referred for liver transplantation secondary to portal

Nutritional management of patients after bariatric surgery.

Bariatric surgery is currently the most effective method of sustainable weight loss among morbidly obese patients. The types of bariatric surgeries can be divided into three categories: restrictive procedures, malabsorptive procedures, and combination (restrictive and malabsorption) procedures. In general, patients undergoing restrictive procedures have the least risk for long-term diet-related complications, whereas patients undergoing malabsorptive procedures have the highest risk. For many patients, the benefits of weight loss, such as decreased blood glucose, lipids, and blood pressure and

[Eating disorder and bariatric surgery: a case report].

OBJECTIVE: The authors discuss, based on a case report and on a literature review, the implications of a bariatric surgery for obesity on the eating behavior. METHOD: A case report is presented in which a patient was submitted to a bariatric surgery and show altered eating behavior like in anorexia nervosa, but without low weight, leading to the diagnostic of an otherwise unspecified eating disorder. This case is compared with the literature review. DISCUSSION: Literature information is discussed according to data from the present case and based on this, aiming to establish a prudent conduct

Pressure-induced rhabdomyolysis after bariatric surgery.

Rhabdomyolisis most commonly occurs after muscle injury, alcohol ingestion, drug intake and exhaustive exercise. Prolonged muscle compression at the time of surgery may produce this complication. Obesity has been reported as a risk factor for pressure-induced rhabdomyolysis, but no reports associated with bariatric surgery could be found in the literature. We report 3 superobese patients who developed rhabdomyolysis after bariatric surgery. This complication was attributed to direct and prolonged pressure of the bed against the dorsal and gluteal muscles.

Laparoscopy in Turkish bariatric surgery: initial experience.

BACKGROUND: The recent application of the laparoscopic method combines minimal invasiveness with reversibility, adjustability and shorter hospital stay. The first laparoscopic bariatric operation in Turkey was performed by us in 1998. METHODS: We report the results in 50 consecutive patients who underwent the laparoscopic application of SAGB between April 1998 and April 1999. The operation setting was the same as for the laparoscopic antireflux procedure. After a closed CO2 pneumoperitoneum (16-18 mmHg), in the first 20 cases five and in the remaining 30 cases four trocars were inserted. A 30

Bariatric surgery: early results with a multidisciplinary team.

BACKGROUND: Bariatric surgery depends on complete preoperative study of morbid obesity, in order to obtain the treatment of choice. A multidisciplinary group was founded in 1998 at the University of Siena. METHODS: During 1998, 16 patients, with median weight 121.8 +/- 31 kg and median body mass index (BMI) 43 +/- 6, underwent bariatric surgery. A multidisciplinary assessment was used in order to evaluate psychological status, food intake problems and patient compliance, and hemodynamic, respiratory, metabolic and arthritic functions. 13 patients were submitted to laparoscopic surgery: in 11

Closing the Gap to Bariatric Surgery: Predictors of Non-Progression in Patients with Obesity Class IV and V at a Community-Based Bariatric Center.

INTRODUCTION: Patients with obesity class IV and V benefit substantially from bariatric surgery, yet many do not complete the preoperative pathway. The contribution of system-level barriers and glucagon-like peptide-1 receptor agonist (GLP-1 RA) therapy to non-progression remains poorly understood, particularly in community settings. METHODS: We conducted a single-center retrospective study of adults with obesity class IV and V evaluated for bariatric surgery (2024-2025). The primary outcome was progression to surgery. Variables included demographics, comorbidities, insurance, primary care ph

The Effects of Preoperative Glycaemic Control (HbA1c) on Bariatric and Metabolic Surgery Outcomes: Data from a Tertiary-Referral Bariatric Centre in the UK.

BACKGROUND: Current recommendations advocate the achievement of an optimal glucose control (HbA1c < 69 mmol/mol) prior to elective surgery to reduce risks of peri- and post-operative complications, but the relevance for this glycaemic threshold prior to Bariatric Metabolic Surgery (BMS) following a specialist weight management programme remains unclear. METHODS: We undertook a retrospective cohort study of patients with type 2 diabetes mellitus (T2DM) who underwent BMS over a 6-year period (2016-2022) at a regional tertiary referral following completion of a specialist multidisciplinary weigh

Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program: Bariatric Surgical Risk/Benefit Calculator: 1-year comorbidity remission.

BACKGROUND: Clinical calculators can provide patient-personalized estimates of treatment risks and health outcomes. The American College of Surgeons Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) set out to create a publicly available tool to assess both short-term postoperative risk and long-term benefits for prospective adult patients eligible for 1 of 4 primary bariatric procedures. The calculator is comprised of multiple prediction elements: (1) 30-day postoperative risk, (2) 1-year body mass index projections, and (3) 1-year comorbidity remission.
