Flaws in advance directives that request withdrawing assisted feeding in late-stage dementia may cause premature or prolonged dying.

BACKGROUND: The terminal illness of late-stage (advanced) Alzheimer's and related dementias is progressively cruel, burdensome, and can last years if caregivers assist oral feeding and hydrating. Options to avoid prolonged dying are limited since advanced dementia patients cannot qualify for Medical Aid in Dying. Physicians and judges can insist on clear and convincing evidence that the patient wants to die-which many advance directives cannot provide. Proxies/agents' substituted judgment may not be concordant with patients' requests. While advance directives can be patients' last resort to at

Neuroscience and Social Problems: The Case of Neuropunishment.

Neuroscientific interventions are increasingly proposed as solutions for social problems, beyond their application in biomedicine. For example, there is increasing interest, particularly from outside commentators, in harnessing neuroscientific advances as an alternative method of punishing criminal offenders. Such neuropunishments are seen as a potentially more effective, less costly, and more humane alternative to incarceration, with overall better results for offender, communities, and societies. This article considers whether neuroscience as a field should engage more actively with such pro

Accidental Thawing of Embryos, Cryopreserved for Transfer. Two Italian cases, Milan and Rome.

The bioethical and juridical debate on the status of frozen embryos sometimes adds new issues arising from new scientific evidence or by accidental occurrences that bring to the attention of the scientific community the need for new practical solutions. Within this scenario, there have been, in recent years, episodes concerning the accidental thawing of embryos, which have been cryopreserved for transfer. Two Italian cases (the Milan and the Rome cases) are here reported: the Milan case involves a couple undergoing artificial insemination. Three eggs were collected for insemination and two of

[Ethical considerations in the presymptomatic diagnosis of Alzheimer's disease].

Research into the human genome has undoubtedly opened up a new perspective in medicine. The ability to identify the cause of specific diseases, especially neurodegenerative diseases, will definitively change the concepts of disease and treatment, while advances such as antibiotic therapy and anesthesia will be relegated to history. However, the arrival of genome medicine poses major bioethical challenges, many of which remain to be resolved. We review the applicability, results and consequences of predictions based on genetic tests for presymptomatic Alzheimer's disease, as well as the dilemma

[Scientific ethics of the abortion with anencephalic fetus].

author proposes, from the perspective of the Scientific Ethics, to assimilate anencephalic fetuses to non-human fetuses because they have a large deficiency of the brain that is the organ for human specificity. This proposal comes after considering arguments and facts from ontogeny, phylogeny, from the situation of loss of the brain in the adult life and from the organic specificity of the human condition given by the brain. If anencephalic fetuses are not human, the interruption of their pregnancy cannot be considered as abortion, regardless the pregnancy stage.

Liver transplant for intentional acetaminophen overdose and hepatic encephalopathy: a conflict between beneficence and justice.

In cases of severe acetaminophen-induced acute liver failure and hepatic encephalopathy, liver transplant can be the only real hope for neurologic recovery and indeed survival. In such cases, the bioethical principles of beneficence and justice often come into conflict. This article examines a case in which a neurologist managing an acetaminophen-overdose patient in the neurologic intensive care unit is faced with a conflict between her patient's need for a liver transplant and the needs of other patients on the transplant list.

Ethical considerations in pediatric dysphagia.

Speech-language pathologists (SLPs) are bound by ethical codes that reflect professional and institutional commitments. When professional activities involve working with children with dysphagia and other complex medical issues, SLPs frequently must make judgments that have bioethical implications. This article reviews general ethical theories and principles, examines SLPs' ethical commitments, and presents an approach for ethical decision making and reasoning. Case studies of children with dysphagia are presented to illustrate ethical dilemmas.

Efficacy of focal high-dose-rate brachytherapy in the treatment of patients diagnosed with low or favourable: intermediate-risk prostate cancer-a protocol for a randomised controlled trial.

INTRODUCTION: Prostate cancer (PCa) is men's second most predominant cancer worldwide. Because the prostate-specific antigen test is used in diagnostics, PCa is more often diagnosed in the early stages, making radical treatment of the disease possible. However, it is estimated that over a million men worldwide suffer from radical treatment-related complications. Thus, focal treatment has been proposed as a solution, which aims to destroy the predominant lesson that determines the progression of the disease. The main objective of our study is to compare the quality of life and efficacy of patie

Vulnerability in Biomedical Research: A Historical Reflection and Practical Implications for HIV Cure-Related Research.

The concept of vulnerability in bioethics was first referenced in 1979, when the Belmont Report highlighted the need for special consideration of certain populations in the application of its general principles of respect for persons, beneficence, and justice in research with human participants. Since then, a body of literature has emerged regarding the content, status, and scope, as well as ethical and practical implications of vulnerability in biomedical research. The social history of HIV treatment development has at various points reflected and actively influenced bioethics' debate on vuln

Inventing the Pedophile in the Journal of Homosexuality.

This article serves as one of the supplementary pieces of this special issue on "Mapping Queer Bioethics," in which we take a solipsistic turn to "map" the Journal of Homosexuality itself. Here, the author examines one of the most controversial moments in the history of the journal, whereby a contributor was subject to governmental and popular rebuke for his scholarship on pederasty, pedophilia, and underage queer sexuality. In a chronological and intellectual appraisal of this pedophilia-themed text, the author asks us to recalibrate the disquietude we posit when same-sex affection, youthful

[Posthumous assisted reproductive technologies].

Several types of posthumous ART, all exceptional, can be considered. Using frozen sperm by the wife or concubine results in a conception at a time when the father has been dead for longer or shorter. His ban is broadly consensual in France. Transferring frozen embryos after the man's death raises more questions. The only options in France for the surviving wife are donation to another couple, donation to research or destruction. The National Consultative Ethics Committee was several times favourable to allowing this practice under certain conditions, but it is still prohibited after the 2011 r

Oncofertility in Prepubertal Girls: A Qualitative Study of Canadian and French Pediatric Oncologists' Perspectives on Ovarian Tissue Cryopreservation.

BACKGROUND: It is now acknowledged that possible negative effects of cancer therapies on future reproductive autonomy are a major concern. In the bioethics literature, some advocate that the child's right to fertility preservation (FP) should be recognized as a right to an open future. RESEARCH OBJECTIVES: The aim of this qualitative study is to (1) explore pediatric oncologists' perceptions regarding barriers and facilitators of OTC in prepubertal girls and (2) analyze the ethical, legal, social, and policy implications of these barriers and facilitators in Canada and France. End-of-life decision-making in the neonatal intensive care unit.

Most paediatric deaths occur in the neonatal period, many of them in neonatal intensive care units after withdrawal of life support or the decision not to initiate new treatments. In these circumstances, discussions with families and decision-making are fundamental elements of the care and attention given to newborn babies. In this context, bioethical deliberation can help us to identify the values at stake, the different courses of action to be taken, and the means to ensure that family-shared decision-making is appropriate to the patient's situation and in accordance with the family's values

Conscience clauses and oral contraceptives: conscientious objection or calculated obstruction?

This article examines the ethical and legal implications of conscience clauses, which allow pharmacists to refuse to fill prescriptions for oral contraceptives due to their belief that they may constitute abortifacients. The author provides an informative background on contraception and the history of how abortion has been viewed in this country, then takes a critical look at some of the conscience laws already in existence, using a bioethical framework to examine them. She concludes by providing suggestions to reduce the negative ethical implications resulting from the enactment of conscience

[Translated article] Ethical approach to pain management in pediatric patients in a terminal situation: A systematic review.

OBJECTIVE: Pediatric palliative care aims to provide a professional, scientific, and compassionate response to the needs of terminally ill pediatric patients, with pain management being one of its fundamental pillars. This systematic review analyzes the ethical aspects involved in the management of pain in pediatric patients at the end of life, using the core principles of clinical bioethics: autonomy, beneficence, non-maleficence, and justice as a framework. MATERIALS AND METHODS: A systematic review was conducted following the guidelines of the PRISMA (Preferred Reporting Items for Systemat

Ethical precepts for medical volunteerism: including local voices and values to guide RHD surgery in Rwanda.

At the invitation of the Rwandan Government, Team Heart, a team of American healthcare professionals, performs volunteer rheumatic heart disease (RHD) surgery in Rwanda every year, and confronts ethical concerns that call for cultural sensitivity. This article describes how five standard bioethical precepts are applied in practice in medical volunteerism related to RHD surgery in Rwanda. The content for the applied precepts stems from semiscripted, transcribed conversations with the authors, two Rwandan cardiologists, a Rwandan nurse and a Rwandan premedical student. The conversations revealed

[Circumcision: history, religion and law].

The aim of this article is to describe the circumcision procedure during history, its therapeutic and preventive goals, with focus on bioethical, economic and law issues. The origins of this practice are lost in antiquity. It was performed since 3000 BC by the Egyptians for hygienic and religious reasons. Moreover, male circumcision is a religious commandment in Judaism and Islam, and it is customary in some Oriental Orthodox and other Christian churches of Africa. Nowadays, circumcision is performed as a routine procedure by the Jews and the Muslims for religious reasons. The world prevalence

The responsibility of bioethicists: The case study of Yemen.

In this article, we describe in detail the health and general living conditions resulting from the ongoing armed conflict in Yemen, including the historical and geopolitical underpinnings. In addition to mere reporting, we use Yemen as a case study to examine the responsibility of bioethicists in general. We find it unacceptable that bioethics neglects the largest humanitarian crisis taking place in the world at the moment as well as the largest Cholera outbreak in history. We argue that bioethicists should do more to address armed conflicts and their resulting basic human rights violations. W

A defense of fundamental principles and human rights: a reply to Robert Baker.

This article seeks to rebut Robert Baker's contention that attempts to ground international bioethics in fundamental principles cannot withstand the challenges posed by multiculturalism and postmodernism. First, several corrections are provided of Baker's account of the conclusions reached by the Advisory Committee on Human Radiation Experiments. Second, a rebuttal is offered to Baker's claim that an unbridgeable moral gap exists between Western individualism and non-Western communalism. In conclusion, this article argues that Baker's "nonnegotiable primary goods" cannot do the work of "classi
