Olfactory impairment and traumatic brain injury in blast-injured combat troops: a cohort study.

OBJECTIVE: To determine whether a structured and quantitative assessment of differential olfactory performance-recognized between a blast-injured traumatic brain injury (TBI) group and a demographically comparable blast-injured control group-can serve as a reliable antecedent marker for preclinical detection of intracranial neurotrauma. METHODS: We prospectively and consecutively enrolled 231 polytrauma inpatients, acutely injured from explosions during combat operations in either Afghanistan or Iraq and requiring immediate stateside evacuation and sequential admission to our tertiary care me

Disruption of caudate working memory activation in chronic blast-related traumatic brain injury.

Mild to moderate traumatic brain injury (TBI) due to blast exposure is frequently diagnosed in veterans returning from the wars in Iraq and Afghanistan. However, it is unclear whether neural damage resulting from blast TBI differs from that found in TBI due to blunt-force trauma (e.g., falls and motor vehicle crashes). Little is also known about the effects of blast TBI on neural networks, particularly over the long term. Because impairment in working memory has been linked to blunt-force TBI, the present functional magnetic resonance imaging (fMRI) study sought to investigate whether brain ac

Modelling the blast environment and relating this to clinical injury: experience from the 7/7 inquest.

This paper addresses the computational modelling of a series of specific blast-related incidents and the relationships of clinical and engineering interpretations. The Royal Centre for Defence Medicine and the Defence Science and Technology Laboratory were tasked in 2010 by the UK Ministry of Defence to assist the Coroner's inquests into the 7 July 2005 London bombings. A three phase approach was taken. The first phase included an engineering expert in blast effects on structures reviewing photographs of the damaged carriages and bus to give a view on the likely physical effects on people clos

Common femoral vein reconstruction using internal jugular vein after blast injury.

Common femoral vein traumatic injuries are rare. Surgical management is controversial and by nature case specific. In this report, we present an unusual case of an isolated common femoral vein injury from a gunshot blast repaired with an interposition internal jugular vein bypass. To our knowledge, this is the first reported case of an isolated common femoral vein reconstructed in this manner.

Accidental head explosion: an unusual blast wave injury as a result of self-made fireworks.

A 33-year old hobby pyrotechnician sustained a lethal craniofacial trauma secondary to a salute fireworks blast. He was examining a misfire of a self-constructed salute gun, when it detonated, causing an explosively rupture of his forehead, which led to his immediate death. An autopsy was performed to achieve knowledge of the injury and to be able to reconstruct the events that lead to it. The pressure effect of the explosion caused a shredded rupture of the forehead with a regional spread of brain tissue and small polygonal skull fragments up to 30m from the detonation site. Furthermore multi

Pericardial tamponade consequent to a dynamite explosion: blast overpressure injury without penetrating trauma.

Acute cardiac tamponade is a life-threatening emergency that requires prompt treatment by either percutaneous or surgical pericardiocentesis. It may occur after penetrating or blunt chest trauma. We report a case of pericardial tamponade in the absence of penetrating trauma, due to blast overpressure injury after a dynamite explosion-which has not, to our knowledge, been reported before. Physicians should be aware of the possibility of pericardial tamponade in victims of barotraumatic events such as dynamite or bomb explosions, even in the absence of penetrating trauma. Cardiac tamponade, alth

Perforation of the colon from the primary blast effect of an extraperitoneal shotgun injury: case report.

Blast trauma can result in injuries to peritoneal organs. Penetrating extraperitoneal shotgun wounds and even tangential gunshot or shotgun injuries of the abdominal or chest wall can result in damage to abdominal organs despite an intact peritoneum and diaphragm. Delays in diagnosis and operative repair of such bowel injuries are frequently associated with high morbidity and mortality rates. We present a case of a 47-year-old man with a history of depression and a self-inflicted shot-gun wound to the chest wall, which was tangential and never entered the chest cavity. Computed tomography of t

Macrophages/microglia as 'sensors' of injury in the pineal gland of rats following a non-penetrative blast.

The pineal gland of adult rats was examined immunohistochemically and electron microscopically following exposure of the animals to a single blast equivalent to 110 kg TNT explosive. The most dramatic feature in rats killed at 7, 14 and 21 days after the blast was the upsurge of a large number of macrophages/microglia intensely immunostained with OX-42, OX-18, OX-6 and ED1 antibodies. These antibodies recognise the complement type three (CR3) receptors, major histocompatibility complex class I and class II (MHC I and MHC II) antigens and monocyte/macrophage antigens. Cell counts in OX-42 immun

Diagnostic accuracy of blast-induced traumatic brain injury: a systematic review and meta-analysis.

OBJECTIVE: The purpose of this review was to systematically evaluate the existing diagnostic approaches of primary blast-induced brain injury through meta-analysis. METHODS: A systematic review and meta-analysis were conducted in PubMed, Web of Science, Cochrane Library, and Defense Technical Information Center databases. Reference lists from the 15 identified studies mainly focused on clinical interviews, vestibular/ocular motor screening tools, neuroimaging, and non-specific biomarkers. Univariate meta-regression and subgroup analyses were employed to identify the sources of heterogeneity.

In silico investigation of brain tissue responses associated with mild blast traumatic brain injury.

Blast induced traumatic brain injury (bTBI) is a significant concern in the era of tactical warfare. The biomechanical response of the human head to blast loading is actively sought. While blast wave propagation through brain tissue is reasonably well understood, the direct relationship between external blast parameters and brain biomechanical responses remains unclear. In this study, a 3D computational human model was used to investigate these relationships under free-field blast conditions relevant to mild bTBI, with peak incident pressure (IOP:70-145 kPa) and positive phase duration (PPD:2-

Genetic Diversity Influences Response and Outcome to Blast Traumatic Brain Injury in a Murine Model.

Blast-induced traumatic brain injury (bTBI) is a significant health concern for military personnel, causing potential long-term neurological consequences. Given the unique and heterogeneous genetic makeup of the human population, the acute responses and evolving sequelae from blast exposure are difficult to predict. To address these challenges, murine models are invaluable in the study of bTBI, as they allow researchers to carefully control exposure parameters while examining physiological changes across genetic, cellular, and whole-organism levels over time. In this study, we investigated the

CT radiomics to assess severity of explosion-induced primary blast lung injury in goats.

Previous studies on primary blast lung injury have mostly been small-sample simulation experiments, primarily relying on morphological identification and lacking imaging-based classification of severity. Herein, we have established a large-sample model of goats exposed to real natural field explosions and employed CT radiomics to assess the severity of lung injury. By extracting 1288 radiomics features and combining baseline data, baseline, radiomics, and comprehensive models were built. Results showed that the radiomics and comprehensive models outperformed the baseline model. Decision curve

Neutrophil extracellular traps mediate inflammation and coagulation dysregulation in primary blast lung injury.

Primary blast lung injury (PBLI) manifests as respiratory distress and hemoptysis syndrome resulting from exposure to blast waves, yet its pathogenesis remains incompletely understood, leading to a dearth of effective treatment options. Neutrophil extracellular traps (NETs) may play important roles in acute lung injury. However, the role of NETs in PBLI was unclear. Therefore, the objective of the present study was to investigate the role of NETs in PBLI. Our study found that, compared to the Con group, blast wave induced the destruction of lung tissue structure, the increase of lung inflammat

Single-cell proteomics delineates murine systemic immune response to blast lung injury.

Victims of explosive events frequently suffer from blast lung injuries. Immune system has been implicated in the pathogenesis of this disease. However, systemic immune responses underlying the progression and recovery of injury repair remain poorly understood. Here, we depict the systemic landscape of immune dysregulation during blast lung injury and uncover immune recovery patterns. Single-cell analyses reveal dramatic changes in neutrophils, macrophages, monocytes, dendritic cells, and eosinophils after a gas explosion, along with early involvement of CD4 T, CD8 T, and Th17 cells. We demonst

Case Report and Literature Review: A Severe Case of Blast-Related Traumatic Brain Injury.

Blast-related traumatic brain injuries (bTBIs), once considered the signature wound of wars, have increasingly affected civilian populations due to the rise in terrorist attacks and industrial accidents. These injuries are complex, resulting from a combination of primary blast effects, secondary projectiles, tertiary impacts, and quaternary injuries from burns and toxic gas inhalation. Understanding the clinical presentation, management strategies, and outcomes of bTBIs is essential for enhancing patient care and improving prognosis. We report a case of industrial-related severe bTBI with open

Research Application of Laser-induced Shock Wave for Studying Blast-induced Cochlear Injury.

The ear is the organ most susceptible to explosion overpressure, and cochlear injuries frequently occur after blast exposure. Blast exposure can lead to sensorineural hearing loss (SNHL), which is an irreversible hearing loss that negatively affects the quality of life. Detailed blast-induced cochlear pathologies, such as the loss of hair cells, spiral ganglion neurons, cochlear synapses, and disruption of stereocilia, have been previously documented. However, determining cochlear sensorineural deterioration after a blast injury is challenging because animals exposed to blast overpressure usua

Characteristics and mechanism of lower limb injury induced by landmine blast: A research in a rabbit model.

BACKGROUND: Limb injuries caused by landmine explosions are tricky to treat and difficult to protect. It is necessary to establish an animal model for studying lower limb injury and to investigate the characteristics and mechanisms of lower limb injury induced by landmine blasts. METHODS: Twenty-six mature white rabbits were randomly divided into sham group (n=10) and injury group (n=16). Landmine blast was simulated by electric detonators under the right lower limb in upright state by a special modified fixation frame. High-speed photography was used to observe the body movements. Establishment and evaluation of an in vitro blast lung injury model using alveolar epithelial cells.

BACKGROUND: Gas explosion is a fatal disaster commonly occurred in coal mining and often causes systematic physical injuries, of which blast lung injury is the primary one and has not yet been fully investigated due to the absence of disease models. To facilitate studies of this field, we constructed an in vitro blast lung injury model using alveolar epithelial cells. METHODS: We randomly divided the alveolar epithelial cells into the control group and blast wave group, cells in the blast wave group were stimulated with different strengths of blast wave, and cells in the control group receive

Localizing Clinical Patterns of Blast Traumatic Brain Injury Through Computational Modeling and Simulation.

Blast traumatic brain injury is ubiquitous in modern military conflict with significant morbidity and mortality. Yet the mechanism by which blast overpressure waves cause specific intracranial injury in humans remains unclear. Reviewing of both the clinical experience of neurointensivists and neurosurgeons who treated service members exposed to blast have revealed a pattern of injury to cerebral blood vessels, manifested as subarachnoid hemorrhage, pseudoaneurysm, and early diffuse cerebral edema. Additionally, a seminal neuropathologic case series of victims of blast traumatic brain injury (T
