Advantages of arthroscopic transosseous suture repair of the rotator cuff without the use of anchors.

BACKGROUND: Although arthroscopic anchor suturing is commonly used for rotator cuff repair and achieves good results, certain shortcomings remain, including difficulty with reoperation in cases of retear, anchor dislodgement, knot impingement, and financial cost. In 2005, we developed an anchorless technique for arthroscopic transosseous suture rotator cuff repair. DESCRIPTION OF TECHNIQUE: After acromioplasty and adequate footprint decortication, three K-wires with perforated tips are inserted through the inferior margin of the greater tuberosity into the medial edge of the footprint using a

Malpositioned olecranon fracture tension-band wiring results in proximal radioulnar synostosis.

BACKGROUND: Tension-band wiring (TBW) is a well-established fixation technique for two-part, transverse fracture types of the olecranon. However, complication rates up to 80 % are reported. By reporting on the enormous impact on the patient if failed the aim of the present report was to emphasize the importance of correct K wire positioning in TBW. CASE PRESENTATION: We present the case of a 49-year-old woman who suffered from a radioulnar synostosis of the forearm due to malpositioned K wires after TBW treatment. The patient was treated by heterotopic bone resection supported by ossification

Does the bone drill bit remain sharp after a collision with the intramedullary pin? a study on isolated canine tibias.

BACKGROUND: Blunting of the tip, as well as smoothing and abrasion of the cutting edges, are natural signs of wear observed under magnification after repeated use of bone drill bits. The lack of a standardized surgical protocol for using drill bits in human and veterinary surgery often results in their use beyond the manufacturer’s recommended limits. Drilling with a dull drill bit results in excessive local material degradation, increased surface roughness, and inaccurate hole dimensions. In bone surgery, an additional factor is thermal damage to the bone. Colliding with a metallic object whi

Ipsilateral Distal Both Bone Forearm and Lateral Humeral Condyle Fractures With Posterolateral Elbow Dislocation: A Rare Injury in a Child.

Lateral humeral condyle fractures are common in children, but concomitant elbow dislocation is rare. This case report involves a 10-year-old girl with an ipsilateral distal both-bone forearm fracture and a lateral humeral condyle fracture accompanied by a posterolateral elbow dislocation. Closed reduction of the elbow was performed in the emergency department without delay, and the patient was operated on as soon as possible. Closed reduction percutaneous pinning (CRPP) was performed for the distal radius fracture. The Weiss type III lateral humeral condyle fracture was reduced through a later

Surgical Management of Congenital Pseudoarthrosis of the Clavicle: A Review of Current Concepts.

Nowadays, surgical intervention is an accepted treatment for congenital pseudoarthrosis of the clavicle (CPC). The purpose of this literature review is to evaluate the current body of evidence for methods and outcomes of surgical intervention for CPC. CPC is a rare deformity of the middle third of the clavicle not often identified until three to five years of age, at which time surgery is often recommended. The most common indication for surgery is cosmetic appearance, but other indications include pain, shoulder dysfunction, and prevention of complications later in life. Surgical intervention

[Fixation techniques for slipped capital femoral epiphysis : Principles, surgical techniques, and complications].

The purpose of this review is to present the pros and cons as well as the surgical techniques of conventional implants used for fixation of slipped capital femoral epiphysis (SCFE). Worth mentioning are K‑wires, Hansson pins, transfixing screws, and gliding screws. We searched PubMed for "ECF" and "SCFE" in combination with "in situ fixation," "pin," "wire," "screw," and "nail." We considered Johansson nail, Knowles pin, and Nyström nail to be obsolete and of historical interest only. We noticed a trend from absolute stability towards some form of dynamic fixation over time, likely related to

Combining a transosseous cerclage wire after patellar tendon reattachment to treat patella distal pole fracture did not improve functional outcome.

This study aims to investigate whether an augmented wire in the treatment of patella distal pole fracture could improve knee range of motion (ROM) and radiographic features. Thirty-five consecutive patients with patellar distal pole fracture were analyzed from January 2014 to July 2019. The treatment is divided into two groups according to the presence or absence of augmented wire. Knee ROM, bone union, extension lag, and patellar height were compared between these two groups as the clinical and radiological outcomes. There was no significant difference in mean knee ROM (110° vs. 108°, p = 0.7

[Arthroscopic scaphoid reconstruction in non-unions : Aspects for indication, technique, and results].

BACKGROUND: Arthroscopic reconstruction of the scaphoid is a modern technique for the treatment of scaphoid non-unions. The principle is the arthroscopic resection and filling of the non-union site from the midcarpal joint space. PROCEDURE: Through minimally invasive placement of the scaphoid screw, the critical blood supply of the scaphoid is respected, which achieves high healing rates between 90 and 100%. The use of a cancellous bone graft seems to allow for healing without carpal malalignment, even in the presence of a humpback deformity with extension malposition of the lunate, although

Long oblique/spiral mid-shaft metacarpal fractures of the fingers: treatment with cerclage wire fixation and immediate post-operative finger mobilisation in a wrist splint.

The cerclage wire technique of internal fixation for displaced long oblique/spiral metacarpal shaft fractures has not gained popularity for two reasons: many believe that wire migration is a real possibility and that the fixation technique is not rigid enough to allow immediate postoperative finger mobilisation. In this report, the authors review the results of 19 cases of long oblique/spiral mid-shaft metacarpal shaft fractures of the fingers treated by cerclage wires and immediate postoperative mobilisation of all finger joints. The study included 17 men and two women with a mean age of 35 (

Polymethyl Methacrylate Cement Fill as a Definitive Treatment for Massive Bone Defect After Infected Internal Fixation in Bicondylar Tibial Fracture: A Case Report.

CASE: A 62-year-old woman sustained a bicondylar proximal tibial fracture. She had a deep infection after open reduction and internal fixation resulting in massive metaphyseal bone loss. Because only subchondral bone was left, the cavity was filled with cement after pins were used to support the proximal tibia. Then, a hinged knee-spanning external fixator (EF) was applied. After the removal of the EF, the patient was almost pain free and tolerated full weight-bearing, and no further surgeries were needed. CONCLUSION: The cemented wires technique proved to be a good short-term and possibly a

Use of cyanoacrylate in facial bone fractures.

It is not uncommon to encounter multiple bone fragments during open reduction of facial bone fractures. When these fragments are few and their size large, there is no problem. However, multiple, small, and thin bone fragments are difficult to handle, making accurate reduction with interfragmentary wires problematic. This is frequently encountered in fractures of the anterior maxillary wall, orbital floor, orbital roof, and frontal sinus. Therefore, many surgeons reconstruct the larger segments and discard the smaller segments. In this study, 10 patients who sustained facial fractures with mult

Biomechanical Comparison of Fixation With a Single Screw Versus Two Kirschner Wires in Distal Chevron Osteotomies of the First Metatarsal: A Cadaver Study.

Distal chevron osteotomy is a common procedure for surgical correction of hallux valgus. Osteosynthesis with 1 screw or 2 Kirschner wires has been commonly used. We compared the stability of the 2 techniques in distal chevron osteotomy. Sixteen first metatarsals from fresh-frozen human cadaver feet (9 different cadaveric specimens) were used. A standardized distal chevron osteotomy was performed. One first metatarsal from each pair was assigned to group 1 (3.5-mm cortical screw; n = 8) and one to group 2 (two 1.6-mm Kirschner wires; n = 8). Using a materials testing machine, the head of the fi

The influence of the mode of treatment of zygomatic bone fractures on the healing process of the infraorbital nerve.

Several authors have described the influence of the method of treatment of a fracture of the zygomatic complex on the recovery of the infraorbital nerve. In this study, the results of treatment of 106 patients with an isolated, non-comminuted, unstable fracture of the zygomatic bone are presented. Thirty-eight patients underwent fixation with interosseous wiring and 68 patients were treated with a miniplate osteosynthesis across the frontozygomatic suture. In the group with wire fixation, 50% suffered persistent reduced sensitivity in the infraorbital region at follow-up examination, whereas i

Loop Anchor Tension Band Fixation for Olecranon Fractures and Chevron Olecranon Osteotomy.

Although tension band wiring is effective for treating olecranon fractures, it is associated with a high reoperation rate because of hardware-related problems. The loop anchor tension band technique is a novel modification of the Arbeitsgemeinschaft für Osteosynthesefragen (AO)-modified tension band wiring technique. This technique is suitable for treating olecranon fractures without severe comminution and olecranon osteotomy. In the current case series, 2 intramedullary 1.25-mm Kirschner wires (K wires) were inserted as temporary fixation into the ulnar shaft from the olecranon after anatomic
