BACKGROUND: Recalcitrant fracture non-union is a condition that is difficult to treat and may require multiple surgeries, sometimes requiring treatment with periosteal flaps. The use of periosteal flaps can be preferred for the treatment of non-unions that do not yet have extensive bone defects. This study aims to share our experience with medial femoral condyle periosteal flap for the treatment of recalcitrant non-union in long bones of the upper limb.
METHODS: Seven patients who underwent treatment for upper limb non-union with a free medial femoral condyle periosteal flap between 2015 and

OBJECTIVE: Occupational therapy is often prescribed after the acute treatment of upper extremity fractures. However, high out-of-pocket expenses and logistical constraints can reduce access to formal therapy services. We aimed to quantify preferences of patients with upper extremity fracture for attending occupational therapy, when considering possible differences in clinical outcomes.
DESIGN: Discrete choice experiment.
SETTING: Level 1 trauma centre in Baltimore, Maryland, USA.
PARTICIPANTS: 134 adult patients with upper extremity fractures.
PRIMARY OUTCOME MEASURES: The scenarios were d

BACKGROUND: Sport climbing and bouldering has developed from a rarely performed to a very popular sport in the last 30 years. Sport physicians are increasingly faced with its specific and otherwise uncommon injury pattern. Overall, the sport is relatively safe, particularly if performed in indoor facilities.
INJURIES: The injuries frequently occur on the upper extremity, with the fingers being the most affected. Acute traumatic lesions such as sprains, fractures or ligament lesions are much rarer than overload and overuse caused by repetitive and highly stressing climbing movements. The most

A 59-year-old woman was admitted three times over a six-month period with recurrent upper extremity deep venous thrombosis (UEDVT). It was determined that this patient was suffering from an unusual presentation of Paget-Schröetter syndrome secondary to a 20-year-old non-union of a midshaft clavicle fracture. Following thrombolysis the patient underwent resection and plate fixation of the clavicle fracture non-union. Despite the anatomic proximity of the subclavian vessels to the clavicle, vascular complications from fracture are rare. Treatment of midshaft clavicle fractures is often non-opera

The pediatric musculoskeletal system differs greatly from that of an adult. Although these differences diminish with age, they present unique injury patterns and challenges in the diagnosis and treatment of pediatric orthopedic problems.

PURPOSE: The main purpose of the present study was to evaluate if there is a difference between objective or subjective administration of the MSTS score in a cohort of patients affected by musculoskeletal oncological diseases.
MATERIALS AND METHODS: All patients who underwent surgery for bone or soft tissue localization of neoplastic disease in lower or upper limb from June 2015 to June 2020 were considered eligible. In order to administer the score as a PROM, the MSTS was first translated and cross-culturally adapted in Italian. During follow up visits, all patients filled out Italian versio

Distal ulna and radius fractures are the most frequent upper extremity fractures seen in emergency rooms. The axis of rotation for forearm pronation and supination runs through the radial head (proximal) and the ulnar fovea (distal). Throughout pronation and supination, the radius can rotate relative to the ulna, thanks to the way its head articulates with it. The ulna remains relatively stable during these movements. However, in cases of fractures of these bones, surgery to repair the radius is usually the best course of action for a distal ulna fracture. Most distal ulna fractures heal succe

CASE: A 23-year-old man presented with a right upper limb injury after a 10-m fall. Initial evaluation demonstrated a terrible triad elbow injury associated with a trans-scaphoid perilunate dislocation. Elbow stabilization with radial head replacement and carpal fixation was performed. Radiographs on postoperative day 7 demonstrated an ipsilateral Essex-Lopresti injury, which had been initially missed, and revision surgery was performed to reconstruct the interosseous membrane.
CONCLUSION: Surgeons should maintain a high degree of suspicion for an ipsilateral Essex-Lopresti injury in patients

The co-occurrence of ophthalmologic abnormality and intrinsic skeletal dysplasia is uncommon. We describe eight instances of a unique form of spondylometaphyseal dysplasia (SMD) associated with cone-rod dystrophy (although documentation is insufficient to be certain of that diagnosis in some). This is a new, syndromic form of SMD for which there is evidence for autosomal recessive transmission. Recognition of the specific bony features described here should precipitate comprehensive ophthalmologic assessment, since vision impairment becomes significantly disabling with age.

BACKGROUND: Firearm-related injuries are a growing cause of morbidity and mortality in the United States, with varying odds of acute compartment syndrome (ACS) and other complications reported in the research. ACS is associated with higher complications after injury, including infection, need for repeat surgery, and functional loss, with the forearm being one of the most common sites for ACS. Although ACS in gunshot-related injuries has been studied, existing research documents wildly variable odds of developing ACS, with much of these data derived from military sources with high-energy weapon

BACKGROUND: Obtaining good functional outcomes with reconstruction following resection of primary bone tumors of the trunk and extremities is a significant challenge. The authors present their reconstructive experience using vascularized fibula flap transfer to improve bone healing and optimize functional outcomes.
METHODS: From 2001 to 2010, 52 consecutive patients received 53 fibula flaps for trunk (n = 19), lower extremity (n = 21), and upper extremity (n = 13) composite reconstructions.
RESULTS: Extremity segmental bone defects were repaired using an intramedullary (n = 14) or onlay tech

BACKGROUND: The recommendation of Anderson and colleagues to bone graft even relatively minor amounts of comminution (a third of the bone diameter) in the treatment of diaphyseal forearm fractures with plate-and-screw fixation has been questioned. This study examines factors related to nonunion in adult patients with comminuted diaphyseal fractures of both the radius and the ulna to determine the relative influence of bone grafting.
METHODS: Over a 15-year period at two hospitals, 41 patients with diaphyseal fractures of both forearm bones satisfied the following criteria: comminution (Orthop

PURPOSE: Animal bite injuries are often encountered in daily practice. In particular, these injuries of the upper limbs can result in severe functional impairment. We have performed early debridement of contaminated tissue and primary closure for these injuries.
METHODS: The subjects consisted of 15 patients (6 males and 9 females) aged 1-91 years (mean 53.6 years) who visited our hospital due to animal bite injuries (dog in 9 patients, cat in 6). The bite site was the forearm in 5 patients and the hand in 10. In the operating room, contaminated tissue was removed, and primary wound closure w

BACKGROUND: Corticosteroid injections (CSIs) are commonly used for symptomatic management of upper extremity osteoarthritis (OA). While systemic corticosteroids are associated with impaired bone health, the fracture risk of intra-articular CSIs in younger patients remains unclear. This study aimed to determine whether intra-articular CSIs of the shoulder, elbow, or wrist increase short-term fragility fracture risk in non-geriatric patients with upper extremity OA.
METHODS: We retrospectively reviewed patients aged < 65 years who received intra-articular CSIs in the shoulder, elbow, or wrist b

Upper extremity reconstruction may be greatly facilitated by "thinking nerve" in the initial evaluation. Frequently, the obvious soft tissue deformity or bone or joint pathology distracts the reconstructive surgeon from an underlying nerve problem. When pain is a portion of the presenting complaints, thinking nerve becomes more obvious. This article attempts to direct the reconstructive surgeon approaching the upper extremity to look for underlying nerve injury in the region of a damaged wrist capsule, neuromas of the cutaneous nerve, and sites of nerve entrapment.

INTRODUCTION: Limb-salvage surgery is standard for extremity sarcoma, and upper extremity amputation has become uncommon. Contemporary data on who still requires it are scarce. We characterised the indications for, and the perioperative and survival outcomes of, upper extremity amputation for sarcoma at a tertiary sarcoma centre.
MATERIALS AND METHODS: Retrospective single-centre series of all patients undergoing upper extremity amputation for histologically confirmed bone or soft tissue sarcoma (2010-2019), classified by indication (primary versus after previous limb-salvage treatment). Over

Focal fibrocartilaginous dysplasia (FFCD) is an uncommon, benign bone lesion that causes angular deformities of the long bones in young children. Most deformities were seen around the knee. Diagnostic criteria are based on clinical and radiological signs: unilateral angular deformity in a long bone of a young child, associated with on X-ray a typical lucent bony defect with surrounding sclerosis at the concavity of the deformity with a cortical defect. The etiology is not understood. There were 16 cases described previously involving the upper extremity: 9 cases with FFCD of the ulna, 3 of the

Twenty patients with intractable diseases in the upper extremity were treated using free vascularized fibula grafts. There were 13 men and seven women. Three patients had traumatic bone defects, five had post-traumatic nonunions, two had congenital pseudoarthroses, seven had defects after tumor resection, and three had other lesions. The reconstructed sites were the humerus in two patients, the radius and/or ulna in 17, and the metacarpal and phalangeal bones in one. The length of the bone defect ranged from 3 to 18 cm (mean: 8.4 cm). Follow-up periods ranged from 6 to 204 months. No patient r
