Malignant bihemispheric cerebral edema after cranioplasty - An extension of the Monro-Kellie doctrine and predictive factors.

BACKGROUND: Several changes in normal pressure dynamics on the brain occur with a decompressive craniectomy and subsequent cranioplasty. Dead space volume is an important factor contributing to intracranial volume postcranioplasty. A decrease in this volume due to negative suction drain along with relative negative pressure on the brain with the loss of external atmospheric pressure may lead to fatal cerebral edema. CASE DESCRIPTION: A 52-year-old gentleman with a 210 mL volume and middle cerebral artery territory infarction underwent an emergency craniectomy and 6 months later a titanium mol

Spontaneous intracranial hypotension associated with bilateral chronic subdural hematomas--case report.

A 34-year-old female presented with spontaneous intracranial hypotension (SIH) manifesting as severe postural headache and meningism. Magnetic resonance (MR) imaging with gadolinium showed diffuse pachymeningeal enhancement. She developed bilateral chronic subdural hematomas 4 weeks after the onset of the symptoms. MR imaging showed descent of the midline structures of the brain. The bilateral chronic subdural hematomas were surgically drained, with no remarkable pressure. Postoperative MR imaging showed complete resolution of the pachymeningeal enhancement and relevation of the midline struct

Superficial middle cerebral vein connection to the cavernous sinus is not infrequent in brain arteriovenous malformations: an argument against their congenital origin?

INTRODUCTION: The aim of this study was to test the hypothesis that the superficial middle cerebral vein (SMCV) is frequently absent or fails to connect with the cavernous sinus (CS) in the presence of brain arteriovenous malformations (AVMs), a frequently reported argument for the congenital origin of brain AVMs. METHODS: The SMCV was retrospectively compared between patients with a brain AVM and a control group. The presence or absence of the SMCV, its direct or indirect connection to the CS and its termination in a laterocavernous sinus (LCS), paracavernous sinus (PCS), or directly in the

Possible role of adjunctive thalidomide therapy in the resolution of a massive intracranial tuberculous abscess.

We present the case of a young child who developed a massive tuberculous abscess of the posterior fossa while being treated for pulmonary tuberculosis. Clinical improvement after surgical excision of the abscess was followed by recurrence of symptoms of acutely raised intracranial pressure on standard antituberculosis and corticosteroid therapy. Magnetic resonance (MR) imaging of the brain showed that a multiloculated abscess had developed anterior to the excision site of the original abscess. The recurring abscess was partly excised and drained but could not be removed completely because of i

Unexpected death following single blow to the orbit.

Severe brain injury is rare after assault to the head with a fist. Our patient was a 39-year-old white male who was punched in a parking lot. The subject fell on his right head. He did not lose consciousness. The subject was taken to the hospital. On admission, his blood alcohol concentration was 229 mg/dL. A CT scan of the head showed no injury. He was admitted to the detoxification unit. He became sober, and was seen ambulating at 3:30 am. By 5:20 am, he was unresponsive. A CT scan of the head showed that an epidural hematoma had developed over the right temporal lobe. The hematoma was drain

Dural arteriovenous fistula, a rare cause of rapidly progressive dementia in a patient with bilateral thalamic lesions: a case report.

There are many causes of bilateral thalamic lesions, but few cases of dural arteriovenous fistula (DAVF) associated with such lesions have been reported previously. Here, we describe an adult man with reversible rapid progressive dementia (RPD) in whom bilateral thalamic lesions were caused by a DAVF that had six supply arteries and drained into both the venous sinus and cortical veins. A 53-year-old man presented with memory decline and abnormal behavior. Head computed tomography (CT) revealed insignificant low density in the bilateral thalami and high density in the right occipital lobe. Bra

Trigeminal neuralgia caused by brain arteriovenous malformations: A case report and literature review.

Few cases of trigeminal neuralgia (TGN) induced by brain arteriovenous malformations (bAVMs) have previously been reported. The present case report described one case of TGN caused by bAVMs in a 32-year-old male patient who suffered from recurrent pain in his right cheek for a period of two years, for whom the seizure frequency and duration of pain increased for 6 months. Magnetic resonance imaging was performed, which demonstrated flow-void signals in the abnormal vessels in the right cerebellopontine angle. Subsequent digital subtraction angiography confirmed the diagnosis of bAVMs, and show

Hemodynamic aspects of cerebral arteriovenous malformations.

Local hemodynamics were studied in 16 patients undergoing total extirpation of cerebral arteriovenous malformation (AVM). Directional Doppler technique was used for the registration of blood velocities in vessels feeding and draining the AVM. Calculated flow in single feeding arteries ranged from 3 to 550 ml/min (average, 180 ml/min). An estimation of total AVM flow was possible in nine patients, and ranged from 150 to more than 900 ml/min (average, 490 ml/min). Pressure recordings were made from feeding arteries at their entrance to the AVM. This pressure was well below the systemic arterial

Less Invasive Management of Endovascular Embolization and Neuroendoscopic Surgery for a Dural Arteriovenous Fistula Presenting with Acute Subdural Hematoma.

Acute subdural hematoma (ASDH), which causes midline shift of the brain, rarely arises from a dural arteriovenous fistula (DAVF). Herein, we report the first case of a DAVF manifesting ASDH, which was treated less invasively with endovascular embolization of a drainer of the DAVF and hematoma removal under neuroendoscopy. A 59-year-old man with a sudden onset of headache was transported to our hospital. Left ASDH and intracerebral hematoma in the left occipital lobe were detected. A cerebral angiogram revealed a DAVF fed by the petrosquamous branch of the left middle meningeal artery and jugul

Endovascular Treatment of Traumatic Arteriovenous Fistula in Young Adults with Pulsatile Tinnitus.

Traumatic arteriovenous fistulas (AVFs) involving the external carotid artery are exceedingly rare in young adults. Since an AVF is the most common life-threatening cause for pulsatile tinnitus (PT), meticulous evaluation and treatment of patients with PT is crucial. Here, we present two traumatic AVF cases treated with coil embolization leading to no residual fistulous connections followed by an immediate and complete resolution of PT. A 20-year-old man developed left ear tinnitus three months after a traumatic brain injury involving the right temporal bone fracture. Cerebral angiography demo

[Parasagittal meningioma with unusual venous drainage manifesting through peritumoral hemorrhage. Case report].

A 63-year-old female was admitted with a 5-day history of left hemiparesis. Computed tomography showed an enhanced mass with perifocal edema in the right frontal parasagittal region and an irregular-shaped, high-density mass anterior to the tumor. Right carotid angiography disclosed the prominent middle meningeal artery (MMA) to be the main feeding vessel of the tumor. In the venous phase, the superior sagittal sinus (SSS) was obstructed and a dilated middle meningeal vein (MMV) drained downward to the sphenoparietal sinus. Left carotid angiography demonstrated similar findings. There was a fi

Development of a CT-Compatible, Anthropomorphic Skull and Brain Phantom for Neurosurgical Planning, Training, and Simulation.

BACKGROUND: Neurosurgical procedures are complex and require years of training and experience. Traditional training on human cadavers is expensive, requires facilities and planning, and raises ethical concerns. Therefore, the use of anthropomorphic phantoms could be an excellent substitute. The aim of the study was to design and develop a patient-specific 3D-skull and brain model with realistic CT-attenuation suitable for conventional and augmented reality (AR)-navigated neurosurgical simulations. METHODS: The radiodensity of materials considered for the skull and brain phantoms were investig

Postnatal growth and development of a cerebral arteriovenous malformation on serial magnetic resonance imaging in a child with hemangiomatosis. Case report.

The authors report the case of a 3-week-old girl with two enhancing extraaxial masses in the posterior fossa, one in the left cerebellopontine angle (CPA) and the other to the right of the vein of Galen. Serial magnetic resonance images obtained in this patient at 3 months and then at 2.5 years of age documented regression of the enhancing mass in the left CPA and development of a cerebellar brain arteriovenous malformation (AVM) in the same CPA location. Also documented were regression of the pineal region mass and formation of the major draining vein of the AVM. The findings in this case sup

Subperiosteal abscesses of the orbit due to sinusitis in childhood.

This paper reports 16 cases of subperiosteal abscesses of the orbit due to acute sinusitis in childhood. They comprise 12.4% of 129 patients presenting with an 'acute orbit'. Acute ethmoidal sinusitis was the predominant cause. The typical clinical features, as well as CT scanning of the paranasal sinuses, orbit and brain, should assist in early diagnosis. If possible, the subperiosteal abscess and sinuses should be drained before loss of visual acuity or intracranial complications occur. Streptococcus milleri was the pathogen most commonly cultured. One patient in this series had intracranial

An outbreak of listerial meningo-encephalitis in sheep.

An outbreak of ovine listerial meningo-encephalitis on sheep farms in eastern Gippsland, Victoria, during winter and spring 1978, is recorded. Cases were confirmed by histology of brain or by culture of Listeria monocytogenes from sheep on 21 farms. The morbidity rate in affected flocks ranged from 0.2% to 8.0%, and the case fatality rate was almost 100%. The peak incidence of disease followed a period of continuous heavy rain and flooding of grazing pasture, and the majority of affected flocks were located on poorly drained coastal sandy soil. There was evidence of concurrent debilitating dis

Spontaneous Clostridium perfringens Meningitis and Brain Abscess in a Neonate.

This case describes a neonate who presented with spontaneous Clostridium perfringens meningitis and brain abscess. The abscess was drained, and the infant completed a 6-week course of antibiotics. Throughout this time the infant remained well with no need for intensive care. C. perfringens central nervous system infections are associated with trauma and poor outcomes. This case highlights that the spectrum of disease can include spontaneous infection with a relatively mildly clinical course demonstrating the importance of 16s polymerase chain reaction in culture-negative cases and its role in

Renal transplantation: an analysis of operative complications.

Review of 112 renal transplants in 95 patients with end-stage renal disease revealed a wound infection rate of 18 percent. The incidence of infection was unquestionably greater if drains were placed in the wound. An 8.8 per cent incidence of pulmonary infection was represented by lung abscess, bronchopneumonia and necrotizing pneumonitis. Three deaths were attributable to fungal infections-candida sepsis, cryptococcal sepsis, and Nocardia brain abscess. In 14 per cent of this series vascular anastomotic complications occurred, and were attributable to stenosis, thrombosis, or hemorrhage. The c

Prolonged Mild-to-Moderate Hypothermia for Refractory Intracranial Hypertension.

BACKGROUND: Therapeutic hypothermia is an emerging therapy for brain injury and cerebral edema. Hypothermia is known to reduce death and neurologic morbidity in survivors of cardiac arrest from ventricular fibrillation. Traumatic brain injury (TBI) trials studies of short-term hypothermia (24 to 48hours) have had conflicting results. Recent evidence however suggests prolonged hypothermia (48 hours to 14 days) may be beneficial for TBI and select cases of nontraumatic brain injury especially when the duration of cerebral edema and intracranial hypertension is expected to last longer than 24 hou
