[Risk factors in anaphylactoid reactions to muscle relaxants. A retrospective study of 103 cases of shock].

A homogeneous series of 103 cases of shock due to muscle relaxants has been used to identify the risk factors in anaphylactoid shocks due to either true anaphylaxis or to non-specific histamine release. Clinical atopy (asthma) and sub-clinical atopy as shown up by skin tests with mites and pollens, as well as a history of IgE-dependent drug allergy, were present with a significantly greater frequency in the history of patients presenting with anaphylaxis. Abnormal histamine release and reactivity to histamine, as assessed by skin tests with 48/80 and histamine, were often found in those patien

Mechanisms of Forssman-induced bronchospasm and their inhibition.

1 The bronchospasm induced in the guinea-pig by the injection of Forssman antiserum was biphasic in nature in both the sublethal and the lethal reaction. 2 The development of both phases of the bronchospasm in the sublethal reaction was dependent upon the presence of the intact complement system and circulating platelets. In the lethal reaction the phase II bronchospasm did not appear to depend on these factors. 3 The compounds used in this study inhibited phase I bronchospasm of the sublethal reaction in the order, methysergide greater than indomethacin greater than aspirin = sulphinpyrazone

Aspirin intolerance and recurrent urticaria in normal adults and children. Epidemiology and review.

The frequency of aspirin intolerance by history in 2,592 normal individuals was 0.3%. Although the frequency of aspirin intolerance was similar in adults and children, the bronchospastic type predominated in adults, and only the urticarial type was found in normal children. The frequency of recurrent urticaria, regardless of etiology, was significantly greater in adults than in children (3.8% vs. 0.3, P less than 0.001). In adults the frequency of aspirin intolerance was over 20 times greater in individuals with recurrent urticaria than in normal individuals (6.5% vs. 0.3%, P less than 0.001).

A comparison of the actions of different bronchodilators in asthma.

The actions of the following pressurized bronchodilators were compared by administration to 24 asthmatics: (1) Medihaler Iso Forte, (2) Alupent, (3) Medihaler-duo, (4) Bronchilator, and (5) Prenomiser Plus. These contained one or more of the following: isoprenaline, orciprenaline, isoetharine, phenylephrine, atropine methonitrate, and thenyldiamine. The dose was a single discharge from the container. The response was assessed by calculating the mean percentage change in F.E.V. at intervals after inhalation. The mean peak rises were respectively (1) 49·4%, (2) 43·6%, (3) 36·2%, (4) 33·5%, and (

[Anaphylactic shock after tourniquet removal in orthopedic surgery].

Case report of six patients who experienced symptoms of severe anaphylactic shock, after tourniquet release at the end of minor orthopaedic surgery of the limbs under regional anaesthesia. The immunological mechanism of the shock was confirmed by prick tests. The causative agent was rifamycin SV used for cleaning the surgical wound prior to its closure. In case of repetitive surgery and/or in allergic patients the use of another antibiotic or the simple cleaning of the wound with normal saline should be considered. In case of surgery using a tourniquet, an iv access should be maintained for 1

[Bronchial asthma caused by occupational sulfite exposure].

A case is reported of a patient with episodes of bronchospasm requiring hospital admission after handling sodium bisulfite on the job. The patient had a 15-year history of bronchial asthma and concomitant rhinoconjunctivitis and a 6-year history of asthma induced by moderate exercise. His family history included a father with sensitization to mites. Skin tests, measurement of specific IgI, and nasal provocation were positive for domestic dust mites and grass pollen. Skin tests for sodium metasulfite at a concentration of 10 mg/ml were negative. A simple blind oral provocation test of sodium me

Physiological effects of a mouth-borne heat exchanger during heavy exercise in a cold environment.

A mouth-borne heat and moisture exchanger (HME) was tested. Nine healthy subjects performed an incremental-load cycle ergometry test to exhaustion, breathing once through the HME and once through a similar device without heat-exchange function (control). HME substantially increased inspired gas temperatures and decreased expired gas temperatures measured at the mouth; at 260 W (pulmonary ventilation (VE) approximately 1001 min-1) these changes amounted to + 15 degrees C and -5 degrees C, respectively. The breathing resistance was increased by HME but remained well within tolerable levels even

Pulmonary effects of sulfur dioxide exposure and ipratropium bromide pretreatment in adults with nonallergic asthma.

In this study we examined the potential short-term effect of sulfur dioxide (SO2) on total respiratory resistance and forced expiratory volume in patients with nonallergic asthma. A group of nine adult subjects with nonallergic asthma, 55 years of age or older, were exposed to SO2 at 0, 0.5, and 1.0 ppm for 20 minutes at rest followed by 10 minutes during light-moderate exercise. The measures of pulmonary function assessed were FEV1, specific total respiratory resistance (SRT), and maximal expiratory flow rates at 50% (Vmax50) and 75% (Vmax75) of expired vital capacity. Measurements were made

Clinical and immunological reactions to Aspergillus niger among workers at a biotechnology plant.

The workforce at a biotechnology plant producing citric acid by fermentation of molasses with a strain of Aspergillus niger was studied. A combination of a respiratory questionnaire and clinical assessment identified 18 subjects (4.9% of the workforce) with work related bronchospasm. In nine of these evidence of sensitisation to A niger was obtained by skin prick tests and radioallergosorbent test (RAST) using as an antigen an extract of the A niger culture fluid from the process. Of the 325 subjects without work related bronchospasm, only nine (2.7%) had a positive prick test. There were no s

Hyperventilation-induced bronchoconstriction in guinea pigs.

The purpose of this study was to investigate the pulmonary effects of hyperventilation in anesthetized, mechanically ventilated guinea pigs. Airway resistance (Raw), dynamic lung compliance (CDyn), blood pressure (BP), heart rate (HR), arterial blood gases (PaO2, PaCO2), pH and arterial plasma HCO3- were measured before and after a 10-min period of hyperventilation produced by increasing the respiratory rate from 60 to 120 breaths/min while maintaining tidal volume at 4 ml. There was a significant increase in Raw and decrease in CDyn lasting up to 20 min after hyperventilation was stopped with

Naloxone does not affect bronchoconstriction induced by isocapnic hyperpnea of subfreezing air.

Although respiratory heat loss with cooling of the tracheobronchial mucosa is responsible for the airway obstruction that develops after inhalation of subfreezing air in asthmatics, the mechanism by which airway cooling results in bronchoconstriction is not known. In order to test whether release of endogenous opiate peptides might play a role in mediating this response, asthmatic subjects were studied before and after isocapnic hyperventilation of subfreezing air after the administration of placebo or naloxone, given in a double-blind fashion. Five asthmatic subjects were tested with low-dose

[Bronchospasm, a sign revealing pulmonary embolism].

As a clinical feature, and sometimes as a feature of the special mechanical tests of ventilation, bronchospasm has been found by the authors to be associated with pulmonary embolus. Bronchospasm was found to be the commonest feature in the 45 patients studied. The sudden onset of bronchospasm must be regarded as strong evidence for pulmonary embolus, especially in patients with a past history of bronchitis. The mechanism of these asthma-like features is discussed. The authors impugn the adrenergic system both to explain the bronchial overactivity of the patients with pulmonary embolus, and to

Oxidant and acid aerosol exposure in healthy subjects and subjects with asthma. Part I: Effects of oxidants, combined with sulfuric or nitric acid, on the pulmonary function of adolescents with asthma.

Both peak flow decrements in children at summer camps and increased hospital admissions for asthma have been associated with summer "acid haze," which is composed of ozone and various acidic species. The objective of this study was to investigate the pulmonary effects of acid summer haze in a controlled laboratory setting. Twenty-eight adolescent subjects with allergic asthma, exercise-induced bronchospasm, and a positive response to a standardized methacholine challenge enrolled in the study; 22 completed the study. Each subject inhaled one of four test atmospheres by mouthpiece on two consec

Inhaled mannitol and cystic fibrosis. Unnecessary bronchial irritation.

Cystic fibrosis is a life-threatening genetic disease mainly characterised by accumulation of viscous secretions in the airways. In the absence of a better alternative, inhaled dornase alfa is used to liquefy bronchial secretions and thereby facilitate their drainage. Mannitol, in the form of capsules of powder for inhalation, is authorised in the European Union for use as a mucolytic in adults with cystic fibrosis. Two double-blind randomised trials have compared two doses of inhaled mannitol (400 mg or 50 mg, twice a day) in a total of 642 patients (57% adults) with cystic fibrosis. After 26
