PURPOSE: Barth syndrome (BTHS), an X-linked disorder caused by defects in TAZ, is the only known single-gene disorder of cardiolipin remodeling. Barth syndrome is an X-linked recessive condition caused by defective remodelling of cardiolipins in mitochondrial membranes because of mutations in the tafazzin (TAZ1/G4.5) gene located at Xq28. The cardinal features of Barth syndrome are cardiac and skeletal myopathy and neutropaenia, reported in the initial description of this condition by Barth et al. (J Neurol Sci 62:327-355) in 1983. Many features of the Barth phenotype have been described but there is no published comment on the facial appearance of these boys, which is consistent and characteristic of this condition.

Skeletal muscle aging results in the gradual suppression of myogenesis, leading to muscle mass loss. However, the specific role of cardiolipin in myogenesis has not been determined. Besides cerebrovascular pathology and livedo, cardiac affections are introduced in clinical manifestations of Sneddon's syndrome. They involve coronary heart disease (up to development of MI), murmur (usually mitral), occasional cardiac arrhythmia. Patients with coronary disease and cardiac murmur show antibodies to cardiolipin significantly more frequently. Wegener's granulomatosis (WG) and microscopic polyangiitis (MP) are "small vessel" vasculitides which can serologically be discriminated measuring autoantibodies directed towards different target antigens (ANCA). In the present study we can demonstrate increased autoimmunity in patients with MP, as evaluated by measurement of thyroid microsomal and cardiolipin antibodies. Studies were carried out on the comparative value of passive haemaglutination, cardiolipin and treponema tests in a total number of 4327 sera syphilitic patients in various stages of the disease and apparently normal subjects or free of syphilis. The results demonstrated the specificity and sensitivity of passive haemagglutination in all the stages of syphilis. In view of the qualities and simplicity of the test, the author recommends it as a screening test combined with a micromethod using the cardiolipin antigen (flocculation test).
