Effects of transcutaneous auricular vagus nerve stimulation on cardiovascular autonomic control in health and disease.

Autonomic nervous system (ANS) dysfunction is a well-known feature of cardiovascular diseases (CVDs). Studies on heart rate variability (HRV), a non-invasive method useful in investigating the status of cardiovascular autonomic control, have shown that a predominance of sympathetic modulation not only contributes to the progression of CVDs but has a pivotal role in their onset. Current therapies focus more on inhibition of sympathetic activity, but the presence of drug-resistant conditions and the invasiveness of some surgical procedures are an obstacle to complete therapeutic success. On the

Temporary tricuspid valve detachment for ventricular septal defect closure: is it worth doing it?

BACKGROUND: Tricuspid valve detachment (TVD) may improve the access for closing certain ventricular septal defects (VSDs), but it has some potential risks. We aimed to study the benefits and drawbacks of this technique. METHODS: The midterm outcomes of all 20 patients who underwent a TVD closure for VSD were reviewed and compared with a control group of 15 patients with VSD closure without TVD. RESULTS: There was no significant residual shunt in either group at the last actuarial follow-up. Tricuspid regurgitation occurred in both groups (45% in the TVD group and 27% in the control group, P

Repair of congenital heart disease: a primer--Part 2.

This review, the second of two parts, describes the repair of aortic arch anomalies, left-to-right shunts, valvular disease, tetralogy of Fallot, and truncus arteriosus. Cardiac transplantation is also discussed. Advances in the surgical management of congenital heart disease have led to improved patient survival and quality of life. Improvements in technology in computed tomography and magnetic resonance imaging have resulted in increasing utilization of cross-sectional imaging in these patients. Perioperative care necessitates that radiologists have a basic understanding of the surgical trea

Vascular Surgery in Low-Income and Middle-Income Countries: A State-of-the-Art Review.

BACKGROUND: Cardiovascular disease (CVD) represents 32% of all global deaths. Studies have shown an increase in CVD prevalence and mortality with the most substantial increase in low-income and middle-income countries (LMICs). Within LMICs, we sought to 1) measure the burden of CVD with respect to aortic aneurysm (AA), ischemic stroke (IS), and peripheral arterial disease (PAD); 2) quantify surgical access to vascular surgery services; and 3) identify challenges and solutions to addressing disparities. METHODS: The Institute for Health Metrics and Evaluation Global Burden of Disease Results T

Effect of a standardised heart team protocol versus a guideline-based protocol on revascularisation decision stability in stable complex coronary artery disease: rationale and design of a randomised trial of cardiology specialists using historic cases.

INTRODUCTION: A multidisciplinary heart team approach has been recommended by revascularisation guidelines, but how to organise and implement the heart team in a standardised way has not been validated. Inter-team and intra-team decision instability existed in the guideline-based heart team protocol, and our standardised heart team protocol based on a mixed method study may improve decision stability. The objective of this study is to evaluate the effect of the standardised heart team protocol versus the guideline-based protocol on decision-making stability in stable complex coronary artery di

A new inclusion technique through an upper partial sternotomy for complicated Stanford B-type aortic dissection with an aberrant right subclavian artery: A case report.

RATIONALE: An aberrant right subclavian artery (ARSA), arising from the proximal descending aorta, is a common aortic arch anomaly, with an incidence of 0.5% to 2%. However, coexistence of dissection and an ARSA is extremely rare. We presented the first case of successful management of complicated Stanford B-type aortic dissection combined with an ARSA with a new inclusion technique and stent graft (SG) implantation through an upper partial sternotomy. PATIENT CONCERNS: A 39-year-old woman with a history of severe hypertension was admitted to our hospital because of sudden-onset chest and upp

Acute respiratory distress syndrome following cardiovascular surgery: current concepts and novel therapeutic approaches.

PURPOSE OF REVIEW: This review gives an update on current treatment options and novel concepts on the prevention and treatment of the acute respiratory distress syndrome (ARDS) in cardiovascular surgery patients. RECENT FINDINGS: The only proven beneficial therapeutic options in ARDS are those that help to prevent further ventilator-induced lung injury, such as prone position, use of lung-protective ventilation strategies, and extracorporeal membrane oxygenation. In the future also new approaches like mesenchymal cell therapy, activation of hypoxia-elicited transcription factors or targeting

An ex-vivo quantitative assessment to determine the optimal aortotomy closure technique.

BACKGROUND: After performing an aortotomy, there are a variety of techniques utilized for suture closure. There is no published data comparing the efficacy of various suture techniques. The goal of this study is to provide an ex-vivo quantitative assessment of resistance to leakage and dehiscence for three aortotomy closure techniques. MATERIALS AND METHODS: An ex-vivo model was developed utilizing explanted porcine aorta. Aortotomies were closed using one of three techniques: 1) single layer baseball stitch 2) double layer baseball stitch 3) horizontal mattress stitch with a top layer baseba

Subclavian steal syndrome.

“Subclavian steal” refers to a syndrome of symptoms relating to arterial insufficiency in a branch of the subclavian artery stemming from flow reversal, attributable to occlusive disease in the subclavian artery proximal to that branch that is usually atheroscleroticin cause. Most patients are asymptomatic, but patients with IMA bypass grafts may manifest angina elicited by exercise of the upper extremity ipsilateral to the graft and stenosis (so called“coronary-subclavian steal”).Subclavian steal may also manifest as vertebrobasilar insufficiency or,most commonly, arm claudication.Subclavian st

[Perioperative risk among patients with cirrhosis].

Ten percent of cirrhotic patients with severely compromised liver function will require a surgical procedure. This article reviews the surgical risk associated with liver damage and surgery. The most important prognostic factor is the degree of functional impairment of the liver. It is evaluated using the Child-Pugh score, which has limitations that have been partially overcome by the Model for End Stage Liver Disease (MELD) score. Cardiac surgery has the highest risk, while extra-thoracic and extra-abdominal procedures have the lowest risk. The mortality for abdominal surgery fluctuates betwe

[Pulmonary venous obstruction after the Williams procedure for partial anomalous pulmonary venous connection].

A 54-year-old man was admitted to our hospital because of chest discomfort. Cardiac catheterization revealed partial anomalous pulmonary venous connection with an intact atrial septum. The right upper pulmonary vein (RUPV) drained into the upper segment of the superior vena cava (SVC). Using the Williams procedure, an atrial septal defect (ASD) was created and a fresh autologous pericardial patch was used to fashion a new pulmonary vein return route from SVC to the ASD. Although the patient was stable after the procedure, he was admitted again 6 months later because of obstruction of RUPV. At

Ascending-descending aortic bypass for the treatment of concomitant severe co-arctation, aortic valve disease and hypoplastic aortic arch.

This report describes a one-stage treatment of a 30-year-old patient suffering from severe aortic valve insufficiency, aortic co-arctation, dilatation of the ascending aorta and arcus hypoplasia. The patient underwent aortic valve, ascending aorta and arch replacement through median sternotomy. The aorta was ligated at the level of the co-arctation, which was located in the proximal part of the descending aorta, and an ascending-descending bypass was created using a transhiatic approach. The postoperative course was complicated by a cerebrovascular accident.

Anomalous interarterial left coronary artery: an evidence based systematic overview.

BACKGROUND: Isolated anomalous left main coronary artery (ALMCA) from the right aortic sinus of Valsalva (RASV) with an interarterial course between the pulmonary trunk and aorta is a rare congenital abnormality. We performed an evidence based systematic overview spanning 4 decades to assess the prevalence, clinical features and management of this anomaly. METHODS: A computerized search spanning 40 years was conducted to identify articles describing cases of ALMCA arising from the RASV with an interarterial course. The bibliographies of all relevant articles were also searched. RESULTS: The

Limb ischemia associated with persistent sciatic artery aneurysms-a report of 2 cases.

Persistent sciatic artery (PSA) is a rare developmental anomaly, usually associated with iliofemoral hypoplasia and dependence on the sciatic artery as the dominant inflow to the lower extremity. Although rare, this anomaly is associated with a very high rate of aneurysm formation and frequent thromboembolic complications presenting as lower extremity ischemia, and requires familiarity on the part of the vascular surgeon to properly diagnose the disorder and plan therapy. The authors present 2 cases of PSA presenting with lower extremity ischemia and review the pathophysiology, diagnosis, and
